BACKGROUND: Neoadjuvant chemotherapy (NACT) is a critical component of breast cancer treatment, yet its impact on tissue expander (TE)-based breast reconstruction remains unclear. The purpose of this study was to examine the impact of NACT on complication rates after immediate TE-based breast reconstruction and on the commencement of adjuvant radiotherapy.
METHODS: Female patients who underwent immediate TE-based breast reconstruction between 2017 and 2022 were included. Inclusion criteria consisted of patients who completed NACT within 2 months of mastectomy and those who did not receive che

BACKGROUND: Over the last 15 years, tissue expansion has been used in the treatment of 324 patients at the A. V. Vishnevsky Institute of Surgery. The principal drawbacks of the traditional technique for tissue expansion are the prolonged time needed to complete the process and a high rate of complications. With the traditional technique, the wound created by the implantation of the expander inhibits the expansion of the tissue until healing takes place, which requires 10-14 days. However, attempts to reduce of the length of the incision have been limited by the necessity to ensure good hemosta…

BACKGROUND: In breast reconstruction, symmetry is a vital issue. However, when the original breast is unusually shaped or the patient desires augmentation at the time of reconstruction, obtaining symmetrical breasts becomes difficult.
OBJECTIVES: The authors performed shaping of unaffected breasts by Brava-assisted autologous fat grafting to enhance breast symmetry, and evaluated the clinical results to validate this new approach.
METHODS: Brava-assisted autologous fat grafting was performed to the unaffected breasts of 12 patients who had undergone unilateral breast reconstruction. The proc

Total ear reconstruction in the postburn auricle is one of the most challenging procedures for plastic surgeons. Adverse factors associated with these procedures include reduced or damaged blood supply, poor elasticity of scar tissue, increased risk of infection, and the possible destruction of skin, temporoparietal fascia, or retroauricular fascia. In cases where patients are severely burned, free flaps, such as radial forearm flaps, contralateral temporoparietal fascial flaps, or omental flaps, can be used as framework envelopes. In this work, we introduced a novel method of expanded upper a

BACKGROUND: Penile reconstruction or phalloplasty has always been one of the most challenging problems for plastic surgeons. In 1936, Bogoras performed the first phalloplasty by using traditional tubed pedicle flaps. Many other flaps and methods have been applied since, including lower abdominal flaps, pudendal-thigh flaps, parascapular flaps, paraumbilical flaps and, of course, radial forearm flaps. For each method, reports of both functional and esthetic successes abound. In this case, donor sites for phalloplasty were somewhat limited by the severe electric burn injury. After much considera

BACKGROUND: Despite the long-standing presumption that nighttime surgery could affect adverse outcomes, its association independent of patient's general condition and disease severity remains unclear. We hypothesized that conducting elective surgery for subjects with good physical status at nighttime was not associated with increased risks of postoperative complications and aimed to examine it in tissue-expander-insertion (TEI) operation for breast reconstruction.
METHOD: Patients who underwent immediate unilateral TEI-based breast reconstruction between 2014 and 2019 were enrolled. They were

BACKGROUND: There are several methods for primary breast reconstruction following oncologic resection, including alloplastic and autologous-based reconstruction. Major complications that can lead to re-operation and reconstruction failure occur in up to 25% of the patients and necessitate salvage procedures.
OBJECTIVES: To present the authors' experience using a pedicled latissimus dorsi (LD) flap for the salvage of complicated and impending failed breast reconstruction.
METHODS: A retrospective cohort study was conducted of all patients who underwent breast reconstruction salvage by means o

A simple technique for orbital aperture expansion to facilitate placement of ocular prostheses is described. Both superolateral and inferolateral orbital margins are released by means of a single burr hole craniectomy of the frontosphenoid bone behind the orbital process of the frontal bone. Vertical and horizontal marginal lengthenings are performed by a rotatory displacement of one bone segment alongside the other. The expanded osseous aperture is secured with wire and plate-and-screw fixation following a supraorbital rim craniectomy to allow an adequate fit. The result provides for easier a

We present a case of successful orbital expansion with a tissue expander in 1 patient. This method will be valuable in preventing the facial deformities of anophthalmia and microphthalmia. The method was successful from a technical standpoint because of scleral investment of the expander and because of relative fixation of the expander by passage of the filling tube through the lateral orbital wall. The early experience of this patient confirms our expectations of successful orbital expansion based on the natural history of patients with orbital tumors or congenital glaucoma and on the experim

The gold standard for management of extensive burn has been early excision, temporary allografting and final autografting. However, "lack of donor skin" is a challenge condition when autografting in the treatment of extensive burns. Designing an efficient and easy to apply expansion method may improve burn care quality and shorten the hospital stay period. From December 1998 to May 2004, we have performed fly paper technique postage stamp skin autografting for eight major burn patients in the Kaohsiung Medical University Hospital. By using a quick cutting plate, chessboard tray and petrolatum

BACKGROUND: Acellular dermal matrix (ADM) is commonly used in implant-based breast reconstruction, but the optimal timing of ADM use in prepectoral 2-stage reconstruction remains uncertain.
OBJECTIVES: The authors evaluated whether delaying ADM application to the second-stage implant exchange (after initial tissue expansion without ADM) decreases meaningful capsular contracture without increasing reconstructive failure.
METHODS: The authors performed a retrospective cohort study of adult women undergoing immediate, prepectoral 2-stage tissue expander reconstruction at a single institution. P…

BACKGROUND: Breast reconstruction is a key step in the physical and social recovery of breast cancer patients. Optimal wound healing is crucial not only for aesthetic outcomes but also for the uninterrupted continuation of adjuvant oncologic therapy, which can significantly impact overall prognosis.
PURPOSE: The study compared single-use negative pressure wound therapy (sNPWT) with standard dressings during the second stage of breast reconstruction - tissue expander-to-implant exchanges. Additionally, it aimed to identify factors associated with an increased risk of impaired wound healing and

BACKGROUND: Patients with large or ptotic breasts undergoing mastectomy followed by tissue expander/implant-based reconstruction may benefit from a Wise (inverted-T) pattern reduction mammaplasty incision compared with the traditional horizontal elliptical incision. The authors compared these two groups of patients with regard to complication rates and outcomes.
METHODS: Sixty-nine patients (117 breasts) were identified who underwent Wise pattern mastectomy and two-stage reconstruction. A control group of 89 patients (136 breasts) who underwent reconstruction after horizontal elliptical maste
