Reduction of cervicothoracic spondyloptosis in an ambulatory patient: when traction fails.

INTRODUCTION: Cervical spondyloptosis is a rare complication of high-energy trauma which often results in significant patient morbidity and mortality. The authors present a case of spondyloptosis of C7 over T1 with minimal radicular symptoms and otherwise complete spinal cord sparing. This case highlights the surgical challenges faced with cervical spondyloptosis and the techniques used when traction fails. CASE PRESENTATION: A 21-year-old man with no significant past medical history presented after a high-speed motor vehicle collision with cervicothoracic pain and mild hand grip weakness in …

Review of traction alopecia in the pediatric patient: Diagnosis, prevention, and management.

Hairstyles and practices that frequently apply tension to the hair follicle can result in traction alopecia (TA). Many of the hairstyles and hair practices associated with a higher risk of TA begin at a young age, making early intervention and education advantageous in the pediatric population. Recognizing TA at its reversible stage in the pediatric population is critical as early interventions, and counseling will prevent permanent scarring alopecia. While TA can affect any pediatric patient, this review highlights the reported literature on specific populations with a higher reported prevale…

Reversal of diabetic tractional retinal detachment attributed to keto diet.

A 40-year-old woman, known type 2 diabetes mellitus, obese (100 kg), on insulin (80 units), was under treatment for unstable proliferative diabetic retinopathy with extramacular tractional retinal detachment (TRD) in the left eye. At 1-year follow-up, she developed progression of TRD involving the macula in the left eye for which she was advised surgery. She did not follow-up for 6 months during which time she had adopted a coconut oil-rich ketogenic diet. She reported losing 25 kg body weight along with reversal of diabetes (Glycosylated Haemglobin (HbA1C) of 5.3% without insulin) in the inte…

Pins and rubbers traction system for fractures of the proximal interphalangeal joint.

PURPOSE: We aimed to evaluate the clinical results using the pins and rubbers traction system (PRTS) as described by Suzuki et al. for unstable intra-articular fractures of the proximal interphalangeal (PIP) joint and to assess the efficacy and the indication of the technique of the PRTS. METHODS: Thirty-nine fingers in 39 patients (mean age 46.0 years) with unstable fractures of the PIP joint were treated by the PRTS. The mean interval between injury and surgery was 30.8 days. Active and passive exercise of range of motion of all fingers was started immediately after surgery. Radiographic an…

PREDICTIVE FACTORS OF SURGICAL OUTCOMES IN VITRECTOMY FOR MYOPIC TRACTION MACULOPATHY.

PURPOSE: To assess predictive factors and surgical outcomes for myopic traction maculopathy. METHODS: This retrospective observational case study enrolled 73 patients who underwent vitrectomy for myopic traction maculopathy. The 79 eyes obtained from our study sample were divided into 4 types: retinoschisis, lamellar macular hole (lamellar MH), foveal retinal detachment (FRD), and FRD + lamellar MH, or into 2 types according to the presence of FRD preoperatively. Dependent variables of interest were age, sex, pre- and postoperative best-corrected visual acuity (BCVA) at 6 months, and axial le…

Numerical modeling of the traction process in the treatment for Pierre-Robin Sequence.

INTRODUCTION: The goal of this numerical study was to identify the results of modulated growth simulation of the mandibular bone during traction in Pierre-Robin Sequence (PRS) treatment. METHODS: Numerical simulation was conducted in the Ansys 16.2 environment. Two FEM (finite elements method) models of a newborn's mandible (a spatial and a flat model) were developed. The procedure simulated a 20-week traction period. The adopted growth measure was mandibular length increase, defined as the distance between the Co-Pog anatomic points used in cephalometric analysis. RESULTS: The simulation ca…

Halogravity traction in the preoperative treatment of scoliosis in twins with Marfan syndrome.

We report on the influence of the duration of halogravity traction for achieving curve correction in monozygotic twins with Marfan syndrome who underwent posterior spinal fusion. Review of the medical charts and standard radiograph analysis of twin girls treated at our department was performed. Halogravity traction with a four-pin skull construct was applied for 3 weeks in twin A and for 2 weeks in twin B with a maximum of 20% body weight used. Both were on a 24-hours-day halogravity traction regime. Achieved thoracic curve correction after halogravity traction was 31% in twin A and 18% in twi…

Magnetic resonance arthrography of the wrist with axial traction: An iconographic review.

Stress maneuvers inspired by arthroscopic techniques have been previously studied for MRA of shoulder, hip, knee and wrist. Axial traction in MRA of the wrist is advantageous to study intrinsic ligaments and cartilage, but seems useless to assess tendons or nerves disorders. Based on our experience and a well-chosen iconography, we would like to emphasize the contribution of axial traction in MRA of wrist disorders.

Bilateral tractional retinal detachment in a patient with hemolytic-uremic syndrome.

Hemolytic uremic syndrome (HUS) is a thrombotic microangiopathy characterized by hemolytic anemia, acute kidney injury, and thrombocytopenia. Ophthalmic manifestations of HUS range from mild, including scattered intraretinal hemorrhages, to severe, including a Purtscher-like retinopathy and iris and optic disk neovascularization (NVD). We report the case of a 23-month-old girl with severe HUS who presented with bilateral dense vitreous hemorrhage. During vitrectomy, optic disk neovascularization and funnel-shaped, tractional detachments of both retinas were observed. To our knowledge, this is …

Biomechanical analysis of two-step traction therapy in the lumbar spine.

Traction therapy is one of the most common conservative treatments for low back pain. However, the effects of traction therapy on lumbar spine biomechanics are not well known. We investigated biomechanical effects of two-step traction therapy, which consists of global axial traction and local decompression, on the lumbar spine using a validated three-dimensional finite element model of the lumbar spine. One-third of body weight was applied on the center of the L1 vertebra toward the superior direction for the first axial traction. Anterior translation of the L4 vertebra was considered as the s…

The Use of Intraoperative Traction in Pediatric Scoliosis Surgery: A Systematic Review.

STUDY DESIGN: Systematic review. OBJECTIVE: To study the morbidity and the potential benefits of using intraoperative skeletal traction as an adjunct method for correcting scoliosis. SUMMARY OF BACKGROUND DATA: Cotrel et al., used intraoperative skeletal traction in all patients in their original series of segmental spinal instrumentation for scoliosis correction (Cotrel et al., Clin Orthop Relat Res 1988;227:10-23). However, the use of intraoperative traction may introduce other forms of surgical morbidity, which has not been systematically studied. METHODS: Two independent reviewers searc…

Widespread vitreoretinal traction simulating retinal vasculitis in a patient with uveitis.

PURPOSE: To describe a case of uveitis with widespread vitreoretinal traction simulating retinal vasculitis angiographically. METHODS: Interventional case report. RESULTS: A 62-year-old woman was examined for low vision in her left eye that persisted for ∼1 year. She had been treated for polymyalgia rheumatica for 1 year. Visual acuity was 20/20 in the right eye and 20/100 in the left eye. There were a few aged vitreous cells in both eyes. On fundus examination, there was mild epiretinal membrane in the left eye. Fluorescein angiography revealed diffuse fluorescein leakage in the late phase …
