A home base for young adults: evidence that a primary care-embedded multidisciplinary rheumatology transition clinic improved care utilization and engagement.

OBJECTIVE: To determine care outcomes and utilization patterns for rheumatology patients seen in a multidisciplinary Young Adult Rheumatology Clinic (YARC) with embedded primary care, compared to patients directly transferred (DT) from pediatric to adult rheumatology clinics. METHODS: Data were collected two years prior to and three years after transfer for YARC and similarly aged DT patients. Difference-in-differences were calculated for show rates and acute care utilization before and after transfer. Vaccination and contraception uptake were compared. Trends in anxiety and depressive sympto

Care transition from a pediatric intestinal rehabilitation program to adult care and the risk of all-cause mortality: A retrospective cohort study.

BACKGROUND: As patients with intestinal failure reach adulthood, transitioning to adult care remains challenging because of a lack of specialized programs. This study evaluated outcomes of patients who transitioned care from a pediatric intestinal rehabilitation program. METHODS: A retrospective cohort study was conducted of intestinal failure patients who turned 20 years old between 2013 and 2022 with follow-up through February 2025. Patient characteristics, including demographics, anatomy, nutrition, and comorbidities, were collected for all patients at the age of 18 years. Patients were cl

Finding a New Normal: Maternal Experiences Transitioning to Home From the Neonatal Intensive Care Unit Caring for Technology-Dependent Infants.

BACKGROUND: Infants discharged from the neonatal intensive care unit (NICU) dependent on medical technology (eg, respiratory or nutritional support) are a growing vulnerable population. These infants are medically fragile, prone to emergency department visits and readmissions, and require increased caregiver demands at home. The experiences of their maternal caregiver's after NICU discharge however, are not well understood. PURPOSE: This qualitative descriptive study addressed this knowledge gap by interviewing mothers of technology-dependent infants about their experience during their first

Involvement of Acute Care Physical Therapists in Care Transitions for Older Adults Following Acute Hospitalization: A Cross-sectional National Survey.

BACKGROUND AND PURPOSE: Recent evidence has suggested physical therapist involvement in care transitions after hospitalization is associated with reduced rates of hospital readmissions. However, little is known about how physical therapists participate in care transitions for older adults, the content of care communications, and the facilitators and barriers of implementing evidence-based care transition strategies into practice. Thus, the purpose of this article is to evaluate participation in care transition activities known to influence readmission risk among older adults, and understand pe

Implementation context, mechanisms and outcomes of a transitional care intervention to prevent delirium: a mixed-methods process evaluation from the TRADE study.

BACKGROUND: While predisposing factors for delirium, like old age or surgery, are well documented, less attention has been paid to environmental factors, including hospital transfer processes and caregiver involvement in transitional care of older patients. To address this gap, we developed a pathway to optimize hospital transfer processes and actively involve caregivers in preventing delirium. This complex intervention was tested in a pilot study using a stepped-wedge design across four hospitals, accompanied by a process evaluation to explore the implementation context, mechanisms and outcom…

Evaluation of a Model of Transitional Care After Preterm Birth on Parents' Mental Health and Self-Efficacy: A Randomized Controlled Pilot Trial.

BACKGROUND/OBJECTIVES: Parents of premature infants experience depression, anxiety, post-traumatic stress disorder, and increased stress, which can negatively impact parent-infant relationships and infant development. To reduce negative consequences and optimally support families, we developed the Transition to Home model (TtH). In this randomized controlled pilot trial (RCT), the feasibility of performing an experimental study to analyse the effects of TtH on parental mental health over time was evaluated. METHODS: The following domains were assessed: recruitment, follow-up and study burden,

Implementation of Transitional Care Planning is Associated with Reduced Readmission Rates in Patients Undergoing Lower Extremity Bypass Surgery for Peripheral Arterial Disease.

BACKGROUND: Patients undergoing lower extremity bypasses (LEB) are at a high risk of developing post-discharge complications requiring readmission. Health systems have developed several strategies to mitigate this risk. One such measure is developing comprehensive Transitional Care Program (TCP), which includes phone calls to patients after being discharged from the hospital. Our study aimed to assess short-term readmission, mortality, and amputation rates of patients who participated in TCP by completing at least one post-discharge follow-up phone call after undergoing LEB for revascularizati…

Use of lung ultrasound to guide surfactant administration and make differential diagnosis during immediate and transitional newborn care in a perinatal hospital.

INTRODUCTION: Lung ultrasound (LU) has proven to be the best method for diagnosing surfactant deficiency (Respiratory Distress Syndrome [RDS]) and continuous positive airway pressure (CPAP) failure; experience in Mexico is limited. METHODS: The objective was to determine the usefulness of a LU program and a lung ultrasound score (LUS) for predicting surfactant need in a perinatal hospital. LUS and FiO2 of patients who received or did not receive surfactant were compared with Mann-Whitney U test. Using LU as the gold standard, FiO2 thresholds were tested using receiver operating characteristic

The unique challenges of childhood-onset systemic lupus erythematosus and lupus nephritis patients: a proposed framework for an individualized transitional care plan.

Childhood-onset systemic lupus erythematosus (cSLE) is a severe lifelong and life-threatening autoimmune disease with multi-organ involvement. Compared to those with adult-onset disease, cSLE patients have more aggressive disease with a higher prevalence of early lupus nephritis (LN) causing worse kidney and patient outcomes. The transfer of adolescent patients to adult healthcare poses several major challenges, from a disease as well as a psychosocial perspective. Transitional care even in tertiary centers can be heterogenous, suboptimal, and often even non-existent. In this comprehensive rev…

Transition of headache care from childhood to adulthood: Focusing needs, barriers, and models of care. A position paper of the IHS Child and Adolescent Committee.

Headache disorders are among the most common neurological conditions in children and adolescents, often continuing into adulthood and causing substantial personal and societal burdens. Yet, the transition from childhood to adult headache care remains under-addressed, with critical clinical practice, policy, and research gaps. This narrative review synthesizes existing evidence and expert perspectives to highlight the urgent need for structured, developmentally appropriate transition models in headache care. It explores the evolving clinical features of headache in adolescence, increased vulner

Transitions of care between pediatric onco-hematology and intensive care units: An exploratory study of healthcare professionals' perceptions and practices in medication management.

BACKGROUND: As in adult care, pediatric patients experience transitions in their care trajectory, such as transfers to intensive care units (ICUs) during episodes of clinical deterioration. In pediatric onco-hematology, the administration of Chimeric Antigen Receptor (CAR) T-cell therapy often necessitates several days of ICU admission for post-infusion monitoring. As a result, systematic collaboration between pediatric onco-hematology and intensive care teams is becoming standard for these advanced therapies. OBJECTIVE: This study aimed to explore healthcare professionals' perceptions, exper

Discharge care quality on transition from hospital to residential aged care after hip fracture: paired next-of-kin and staff responses.

BACKGROUND: Hip fractures are common among residential aged care (RAC) residents and associated with a larger burden of mortality and functional impairment. RAC residents are often prioritised for early discharge back to RAC and are at risk of disjointed care and limited support in their functional recovery. AIM: To compare next-of-kin and RAC staff perspectives on the discharge care transition from hospital following hip fracture surgery, using routinely collected questionnaire data from a geriatrician-led virtual service. A secondary aim was to describe clinical outcomes including mobility,
