Results of attempted radical tumor removal and venous repair in 100 consecutive meningiomas involving the major dural sinuses.

OBJECT: Radical removal of meningiomas involving the major dural sinuses remains controversial. In particular, whether the fragment invading the sinus must be resected and whether the venous system must be reconstructed continue to be issues of debate. In this paper the authors studied the effects, in terms of tumor recurrence rate as well as morbidity and mortality rates, of complete lesion removal including the invaded portion of the sinus and the consequences of restoring or not restoring the venous circulation. METHODS: The study consisted of 100 consecutive patients who had undergone sur

Pathogenetical consideration of spontaneous dural arteriovenous fistulas (DAVFs).

Sinus thrombosis has been regarded as an aetiological factor in DAVFs. However, this claim has been disputed in the literature; because it is not possible to prove that DAVFs arise as a result of sinus thrombosis in all cases, and there is evidence that venous thrombosis can be acquired from a DAVF. The purpose of this study is to examine the hypothesis that the different angiomorphologies of DAVFs depend on the location of their venous outflow, and that a territorial classification of spontaneous DAVFs can be created which is based on their acquired development as a consequence of the breakdo

Lemierre's Syndrome With Cavernous Sinus Thrombosis: A Life-Threatening Complication of Peritonsillar Abscess.

Introduction: Lemierre's syndrome is a rare and life-threatening form of septic thrombophlebitis that can complicate head and neck infections. In most cases, septic thrombophlebitis affects the internal jugular vein. Its extension into the cerebral venous sinus is extremely rare, especially in the cavernous sinus. Herein, we report a rare and fatal case of peritonsillar phlegmon complicated by extensive cervicofacial fasciitis, with both internal jugular vein and cavernous sinus thrombophlebitis. Observation: It was a 72-year-old patient with a history of type II diabetes and chronic renal fai

[On the anatomical measurements of the posterior fossa using dry skulls--from neurosurgical standpoint].

For the purpose of studying anatomy of the posterior fossa from neurosurgical standpoint, detailed observation and measurements were performed on dry skulls. In addition, literatures were reviewed concerning size and shape of the tentorial incisura, as well as anatomical locations of dural penetration of the cranial nerves, three to twelve, and numbers of their rootlets. As a cardinal landmark for posterior fossa craniotomy, junction of the transverse and the sigmoid sinuses was chosen. This corresponds to the crossing point of the following two lines: A) parieto-mastoid suture B) a line drawn

Classification and management of antral septa for maxillary sinus augmentation.

The antral septum, a commonly found anatomical variation, has been related to the occurrence of membrane perforation during sinus augmentation. The aims of this study were to review features of antral septa and to propose a classification system and options for managing antral septa during sinus augmentation. A literature search of the PubMed database was performed to identify articles investigating antral septa. Manuscripts using three-dimensional computed tomography, providing direct measurements of human subjects or cadavers, and reporting features of antral septa besides the prevalence wer

Risk factors for failed transverse sinus stenting in pseudotumor cerebri patients.

BACKGROUND: Idiopathic intracranial hypertension (IIH) when no underlying etiology is found, is a clinical syndrome characterized by elevated intracranial pressure (ICP) (>25 cm H2O), which may lead to headaches and visual symptoms. In patients with IIH who are found to have transverse sinus stenosis, placement of a venous stent across the stenosis has been shown to lower ICP and to resolve the symptoms in several case series, with generally favorable results. In this study, we examine common risk factors associated with failure of transvenous stenting for IIH. If venous sinus stenting fails,

Endovascular mechanical thrombectomy for the treatment of extensive postpartum cerebral venous sinus thrombosis: a case report.

BACKGROUND: Cerebral venous sinus thrombosis (CVST) is a rare but potentially life-threatening complication of pregnancy and the postpartum period. Although systemic anticoagulation remains the standard first-line treatment, evidence supporting endovascular mechanical thrombectomy in postpartum patients is limited. CASE PRESENTATION: A 22-year-old woman presented 12 days after an uncomplicated vaginal delivery with progressive headache and vomiting. Neuroimaging confirmed extensive CVST involving the superior sagittal, cortical, left transverse, and sigmoid sinuses, extending to the left jugu

Idiopathic hypertrophic pachymeningitis masquerading as CNS neoplasm: case report and literature review.

IHP is a rare inflammatory disorder characterized by dural thickening. Its nonspecific presentation often leads to diagnostic challenges and potential misdiagnosis as a neoplasm. Literature review and illustrative case example. PubMed search using terms related to IHP yielded 272 candidate citations, 50 of which met study criteria and were included. A 40-year-old woman presented with headache, dizziness, and blurred vision. Surgical intervention via right craniotomy was recommended due to diagnostic uncertainty, symptomatic mass effect, and the potential for a malignant diagnosis. A near-total

Persistent Oscillating Vertigo From Extracranial Venous Compression.

BACKGROUND: Persistent oscillating vertigo (POV) can be triggered by motion, when it is called mal de débarquement syndrome (MdDS) but can also occur from nonmotion triggers such as neck injury, inflammation, or homeostatic derangements (nonmotion POV [nmPOV]). The pathology underlying MdDS and nmPOV is unknown but shared symptoms include rocking/bobbing/swaying vertigo, headache, cognitive slowing, and fatigue. METHODS: We present a case series of patients with MdDS and nmPOV whose symptoms were relieved after treatment of extracranial venous compression in the neck and thoracic outlet. RES

New Horizons for Diagnostic Pitfalls of Cerebral Venous Thrombosis: Clinical Utility of a Newly Developed Cerebral Venous Thrombosis Diagnostic Score: A Case Report and Literature Review.

BACKGROUND Diagnosing cerebral venous thrombosis (CVT) poses significant challenges owing to a nonspecific clinical presentation, poorly correlated laboratory biomarkers, and low sensitivity of non-contrast head computed tomography (CT). We describe a case of missed CVT diagnosis, due to low clinical suspicion and nonrecognition of anemia as a prothrombotic factor, especially during an ulcerative colitis (UC) flare. A recently proposed CVT clinical probability score can guide clinicians in pursuing further neurovascular imaging. CASE REPORT A 35-year-old man, with treatment-naive UC, presented
