Necrotizing sialometaplasia is a benign, self-healing disease of salivary gland tissue and is usually confined to the minor salivary glands of the hard palate. It has clinical and histological features that simulate malignancies such as mucoepidermoid and squamous cell carcinomas. Wrong diagnosis has led to unnecessary mutilating surgical procedures. The etiology of the disease is unknown, but an ischaemic process is considered most likely. We describe two patients with necrotizing sialometaplasia, one with midline and one with bilateral symmetrical affection of the hard palate.

A new method for arresting uncontrollable intercostal hemorrhage is presented. It consists of selective transcatheter embolization of the bleeding artery, either by pieces of gel foam or autologous blood clots. In the patient presented in this paper, the hemorrhage was identified and successfully controlled by embolizing the bleeding artery. Thoracotomy was unnecessary, and there were no complications related to the procedure.

We describe the case of a previously healthy teenager who developed third-degree atrioventricular block due to Lyme disease. While Lyme carditis is common in children, severe conduction anomalies remain rare but can be life-threatening. This case highlights the importance of early recognition, prompt testing, and timely intervention to prevent unnecessary procedures, such as permanent pacemaker implantation, and to ensure favorable outcomes. This case also supports Lyme disease testing in pediatric sudden cardiac death in cases with autopsy findings suggestive of myocarditis or pericarditis.

CASE: A 51-year-old woman with diabetes developed wound complications consistent with presumed infection 5 days after undergoing a carpal tunnel release. Despite multiple debridements, the wounds continued to necrose and deteriorate. A diagnosis of pyoderma gangrenosum eventually was made, and the condition improved with the administration of systemic corticosteroids.

Blisters associated with PUVA treatments are an adverse effect of photochemotherapy that has been reported in the literature. Asymptomatic blisters appear spontaneously mainly on the lower limbs and resolve without treatment. The central goal of patients with factitious disorders is to receive medical care. Unnecessary diagnostic procedures and recurrent hospitalizations often ensue. We saw a 39-year-old man with a novel variation of this disorder: factitious cyclic hypersomnia, or the simulation of recurrent episodes of excessive sleep. This case highlights the observations that patients whose illnesses are simulated may have diverse symptoms, that no syndrome is immune to factitious imitation, and that attempts at treatment, though exceedingly challenging, are always contingent upon appropriate recognition.

Non pyogenic discitis is a benign disease of the pediatric age grup. 1st diagnosis is difficult, as the initial symtons may divert the physician to different organs or systems other than the spine. Establishing the diagnosis in the early stage of the disease is important in order to avoid unnecessary and expensive diagnostic procedures. Non pyogenic discitis is diagnosed on radiograms by the demonstration of the narrow disc-space with involvement of the adjacent vertebral bodies. Four phases are recognised in the development of the imaging changes.

We describe two adolescent patients in whom a disorder of the vocal cords associated with emotional factors resulted in acute episodes of stridor. Adduction of the vocal cords on inspiration and abduction on expiration was found on indirect laryngoscopy. The problem responded to either placebo treatment or psychotherapy. The similarity between vocal cord dysfunction presenting as stridor and that presenting as asthma is discussed. The importance of diagnosing these functional problems in children is emphasized in order to avoid unnecessary diagnostic procedures and hazardous treatment.

Vagal reaction occurring after percutaneous needle biopsy can mimic hypovolemic shock. Misdiagnosis has led to unnecessary surgery and death. Both vagal reaction and hypovolemia can produce hypotension, pallor, and diaphoresis. Vagal reaction is characterized by bradycardia or absence of tachycardia. On the other hand, hypovolemia is characterized by tachycardia. Atropine in large doses is the treatment of choice for vagal reaction, when treatment is necessary. Radiologists involved in percutaneous biopsy procedures should be aware of vagal reactions and should known how to treat them.

BACKGROUND: Lichen planus is a rare autoimmune disease primarily affecting the skin and mucous membranes of the oral mucosa, vulva, and vagina.
