Leiomyoma of the bladder is a rare diagnosis and accounts for 0.43% of all bladder tumors. We herein present a case of a thirty-nine-year-old female diagnosed with bladder leiomyoma presenting with urinary retention. Workup of her urinary retention revealed a midline bladder neck mass. Transurethral resection of her tumor revealed pathology consistent with bladder leiomyoma. The urachus is an embryological remnant that normally regresses before birth. Failure in this regression may give rise to different pathologies. Among these, a urachal abscess may be difficult to diagnose based on the clinical presentation alone since this condition can mimic different pathologies. We report the first case of concurrent urachal abscess and invasive low-grade urothelial carcinoma of the urinary bladder.

The analysis of long-term results of complex surgical correction of urinary bladder extrophy in 83 patients has shown that 21 (25.3%) patients had concrements in the urethra. Sarcomas with muscular differentiation (eg, rhabdomyosarcoma, leiomyosarcoma, pleomorphic sarcoma) are uncommon in the adult urinary bladder.
