Adalimumab therapy for refractory childhood uveitis.

PURPOSE: To report the results of adalimumab therapy in a cohort of children with refractory noninfectious uveitis. METHODS: The medical records of patients diagnosed with uveitis and treated with biweekly adalimumab injections for a period of at least 3 months at the University Hospital of La Paz from 2007-2012 were retrospectively reviewed. Improvement in inflammatory activity was graded according to grading schema of the Standardization of Uveitis Nomenclature Working Group. RESULTS: A total of 15 patients participated in the study (12 girls; mean patient age, 12 years). Diagnoses include…

Interferon-α therapy in noninfectious uveitis.

Interferon (IFN)-α and IFN-β are naturally occurring cytokines which seem to have similar effects on the immune system. One of the effects of IFN seems to increase regulatory T cells. There are numerous case reports and studies reporting about the effect of IFN-α against Behçet's disease (BD), but also against chronic uveitic macular edema and a few other types of uveitis. Within 2-4 weeks, approximately 94% of patients reach complete or partial remission in the case of BD-associated uveitis. So far, IFN-α is the only drug that leads to stable remission even after discontinuation of the treatm…

New treatment options for noninfectious uveitis.

Autoimmune uveitis is a group of sight-threatening inflammatory diseases associated with an exacerbated immunological response to ocular proteins. The Standardization of Uveitis Nomenclature Working Group Guidelines have recommended the use of corticosteroids as the first line of therapy for patients who present with active uveitis. However, long-term use of corticosteroids is associated with numerous adverse effects including cataract, glaucoma and metabolic disorders. In this context, new drugs developed to treat rheumatic diseases, and other autoimmune diseases, are being employed often as …

Central serous chorioretinopathy, corticosteroids, and uveitis.

Any patient with a diagnosis of central serous chorioretinopathy (CSCR) should be questioned to determine any recent corticosteroid use, as the poor visual outcome may be due to unrecognized corticosteroid use. Atypical forms of CSCR may be misdiagnosed as an inflammatory serous retinal detachment, mainly Vogt-Koyanagi-Harada disease. CSCR that develops in association with preexisting uveitis could be a challenging diagnosis as it might be misinterpreted as a worsening of the primary inflammatory condition. Failure to differentiate CSCR from retinal or choroidal inflammatory diseases may resul…

[Differential diagnosis in infectious posterior uveitis].

Infectious aetiologies represent the largest entity in patients affected by posterior uveitis. As a single diagnosis, ocular toxoplasmosis ist the most frequently diagnosed infectious entity, wheres Behçet's uveitis represents the most frequently encountered immunologic aetiology. In acute posterior uveitis, an exact clinical diagnosis is sometimes prohibited by a dense vitreal infiltration. In this situation, the clinical course and progression of the disease may help in the differential diagnosis. Rapidly progressive instances such as viral retinal necrosis may be differentiated from cases o…

Bilateral, simultaneous, uveitis-associated pupillary membranes.

A 48-year-old Caucasian man with an established diagnosis of pathological ankylosing spondylitis with cervical spinal fusion and a lengthy history of acute, recurrent, anterior uveitis presented with decreased vision in both eyes. Biomicroscopic examination revealed bilateral inflammatory pupillary membranes associated with anterior uveitis to be the source of the visual compromise. Aggressive topical anti-inflammatory and mydriatic therapy did not break the pupillary membranes and the patient ultimately underwent surgical resection of the membranes in both eyes. Vision returned to normal in o…

Listerial keratoconjunctivitis and uveitis (silage eye).

Listerial keratoconjunctivitis or silage eye has increasingly been reported in ruminants in recent years. Although the disease has always been associated with silage feeding, its cause, pathogenesis, and epidemiology remain to be fully disclosed. Clinical courses include signs of keratoconjunctivitis and uveitis and cases recover without any residual lesions after antibiotic therapy. More epidemiologic and clinical as well as experimental studies are required to determine this poorly defined condition so that preventive measures could be established.

Tuberculous uveitis after treatment with etanercept.

BACKGROUND: Etanercept is a tumor necrosis factor (TNF) inhibitor that has been licensed in the United States for the treatment of adult and juvenile rheumatoid arthritis as well as psoriatic arthritis. Reactivation of tuberculosis is a complication of its use. We report the first case of tuberculous uveitis due to etanercept. METHODS: We performed a clinical chart review. CASE: A 58-year-old Caucasian woman was referred to our hospital for chronic unilateral granulomatous panuveitis of the right eye (RE). She was on etanercept and methotrexate for rheumatoid arthritis. Since the patient was…

[Therapy of chronic non infectious uveitis].

PURPOSE: Chronic non infectious uveitis represents two-thirds of the causes of chronic uveitis referred in tertiary referral ophthalmology centre. One case out of 5 may evolve towards blindness. Therapy should be discussed on the basis of the uveitis severity and the diagnosis; it uses topics or systemic drugs, mainly corticosteroids and immunosuppressors. CURRENT KNOWLEDGE AND KEY POINTS: Besides corticosteroids and ciclosporin, use of immunosuppressors and biotherapy in chronic non infectious uveitis is not an indication of the Autorisation de Mise sur le Marché. However, immunosuppressors …

Helicobacter pylori infection in anterior uveitis.

BACKGROUND: Despite intensive research, the etiology of acute anterior uveitis (AAU) remains poorly defined. Infection with gram-negative bacteria such as Yersinia, Salmonella, Shigella, and Chlamydia have already been suggested as a possible trigger event for AAU. Helicobacter pylori is also a gram-negative bacterium, shares the lipopolysaccharides, but did not attract the attention of many ophthalmologists until recently. Having in mind the relatively high incidence of H. pylori infection in the population, we propose that H. pylori may also be a trigger factor for AAU. PATIENTS AND METHODS…

[Tubulointerstitial nephritis with uveitis (TINU) syndrome].

Authors present an 11-year old girl with non-specific clinical symptoms with increase of inflammatory indices and acute non-oliguric renal failure: creatinine clearance - 55 ml/min/1.73 m2. An anterior uveitis was observed after 7 weeks of illness. Diagnosis of tubulointerstitial nephritis with uveitis (TINU) was confirmed by interstitial infiltrate (lymphocytes, neutrophils, eosinophils) and interstitial fibrosis in renal biopsy. Steroid therapy was given for 7 months. Uveitis without kidney involvement relapsed 1 month after discontinuation of steroid therapy. The full recovery of uveitis wa…

[Uveitis in children: about 20 cases].

INTRODUCTION: Uveitis in children is rare: 3%-8% of all cases. It requires the same diagnostic and etiological processes as uveitis in adults, with additional difficulties at examination and a sometimes insidious progression. MATERIAL AND METHODS: Retrospective study of 20 cases of childhood uveitis from 1995 to 2000. All patients received an ophthalmologic examination and an etiological search, with specific and/or symptomatic treatment and follow-up lasting from 6 months to 5 years. RESULTS: The 20 children presenting uveitis were 4-16 years old, with etiologies as follows: 8 cases of Behç…

[Specific features of tuberculous uveitis pathomorphology].

There have been great changes in the clinical picture and course of ocular tuberculosis in the 90s versus the 70-80s. There has been a upward trend in the incidence of ocular tuberculosis in the population, in children and teenagers in particular. Patients with tuberculous uveitis involving the posterior eye makes up over 70%. A specific eye tissue inflammatory process is mainly exudative and infiltrative with rapid development of the complications masking a tuberculous focus, which hampers treatment and causes a significant decrease in visual functions.

[Clinical immunological classification of tuberculous uveitis].

Clinical and immunological studies carried out in 251 patients with tuberculous uveitis (TU) revealed three types of inflammatory reactions in the uveal tract in this patient population: productive, exudative, and productive-exudative (mixed), differing by the type of immune disorders. The findings helped develop a clinical immunological classification of TU, taking account of the clinical form of uveitis, type of inflammatory reaction of the uveal tract, disease pattern, involvement of one or both eyes, complications, and the immunopathological variants of TU course.

Uveitis and anti-neutrophil cytoplasmic antibodies.

Serum samples of 485 uveitis patients were screened for the presence of anti-neutrophil cytoplasmic antibodies using a standardized immunofluorescence test (IIF) on neutrophil granulocytes. Seventeen of these sera contained cytoplasmic (C)-ANCA antibodies, while two of the sera contained perinuclear (P)-ANCA antibodies (both antinuclear antibody (ANA)-positive, one anti-myeloperoxidase (MPO)-positive). None of the C-ANCA-positive sera reacted with proteinase-3 in ELISA using a highly purified proteinase-3 preparation. Four C-ANCA and one P-ANCA-positive serum reacted with MPO. The majority of …

Acute anterior uveitis and HLA-B27.

Acute anterior uveitis is a common ocular disease characterized by inflammation of the iris and ciliary body. In the majority of patients presenting with an acute attack of anterior uveitis, the only clues to the pathogenesis of this disease are its close association with the genetic marker HLA-B27 and the likely triggering role of a variety of gram negative bacteria. HLA-B27 acute anterior uveitis appears to be a distinct clinical entity frequently associated with the seronegative arthropathies, such as ankylosing spondylitis and Reiter's syndrome. Recent advances in our understanding of the …

Serum immunoglobulins in acute anterior uveitis.

Serum levels of immunoglobulin A, G, and M were examined in 99 patients with acute anterior uveitis (AAU). The mean level of serum IgA was raised (p less than 0.001) in AAU patients when compared with healthy control subjects. The levels of serum IgG and IgM were not significantly different from those in the control group. The raised levels of serum IgA were found to occur predominantly in the HLA-B27 positive group of patients (p less than 0.001) and were highest in HLA-B27 positive patients with sacroiliitis or associated spondylarthritic diseases (p less than 0.001). The HLA-B27 negative gr…
