Hashimoto's encephalopathy- or steroid responsive encephalopathy with autoimmune thyreoiditis (SREAT) - is a disease that can show multiple neurologic manifestations. So far, the pathogenesis of Hashimoto's encephalopathy is not clear. We report on a 47-year-old male patient suffering from a cerebral infarction of the right posterior cerebral artery within the framework of an angiographic confirmed cerebral vasculitis and an autoimmune thyreoiditis. The clinical presentation of the disease could hardly be distinguished from the symptoms of Hashimoto's encephalopathy. The concurrence of the pre…

Small-vessel vasculitis is defined by the presence of blood vessel inflammation involving the arterioles, venules, or capillaries. This can be seen in a broad spectrum of settings, but it is most commonly associated with Wegener's granulomatosis, microscopic polyangiitis, and Churg-Strauss syndrome. Although prednisone combined with cyclophosphamide induces remission and prolongs survival in these diseases, this regimen is toxic and does not prevent relapse. Current therapeutic approaches seek to minimize cyclophosphamide exposure through the use of staged induction-maintenance regimens or cyc…

Thoracic involvement of Behcet's disease is unusual but serious. It is related to the well known vascular tropism of the disease. It may involve the superior vena cava, pulmonary arteries, aorta and subclavian vessels. Imaging is useful for diagnosis and assess the degree of thoracic involvement. CT scan and MRI are obviously more accurate than angiography. The spectrum of thoracic manifestations of the disease is presented based on a review of 22 cases.

OBJECTIVES: To review the most recent information on the incidence, clinical course, pathology, pathogenesis, diagnosis, and treatment of rheumatoid vasculitis (RV), including the still scanty data on the use of biologics.
METHODS: PubMed and MEDLINE databases (1950-2006) were searched for the key words "vasculitis" and "rheumatoid arthritis"; and "rheumatoid arthritis" and "extra-articular manifestations." All relevant articles in English and French were reviewed. Additional words used in follow-up research include "anti-TNF," "rituximab," "IL-1 receptor antagonists," and "CTLA-4 Ig," all in…

One of the most important questions in vasculitis research is not why inflammation of blood vessels occurs but why it persists, often in a site-specific manner. In this review we illustrate how stromal cells, such as fibroblasts and pericytes, might play an important role in regulating the site at which vasculitis occurs. Smooth muscle cells and fibroblasts directly influence the behaviour of overlying vascular cells, amplifying the response of the endothelium to proinflammatory agents such as TNF-alpha and allowing enhanced and inappropriate leucocyte recruitment. An abnormal local vascular s…

We report the case of an adult with rheumatoid arthritis (RA) who developed biopsy-confirmed testicular involvement of vasculitis in the setting of mononeuritis multiplex. This unusual presentation of rheumatoid vasculitis was successfully treated with a combination of corticosteroids and cyclophosphamide.

UNLABELLED: VIRUSES, THE CAUSE OF VASCULITIS: Although the majority of systemic vasculitis are of unknown causes, the responsibility of a viral infection has been formally demonstrated in some of them and specific treatment can permanently cure them. Each virus incriminated accounts for a particular type of vasculitis. HEPATITIS B VIRAL INFECTION (HBV): Is the cause of polyarteritis nodosa in 36 to 50% of cases. The onset of the symptomatology is acute, usually within a few months following the infection; it is comparable to that observed in the absence of HBV infection. CRYOGLOBULINEMIA RELAT…

RESULTS: We found an unexpected lesion: acute necrotizing vasculitis in arterioles located < or =2.05 mm from the target artery. It was characterized by fibrinoid necrosis of the wall, often associated with lymphocytic exudates or thrombosis. Based on the review of perpendicular sections of tissue samples, the arteri …

Antineutrophil cytoplasmic antibodies (ANCA)-associated vasculitis is the most common primary systemic small-vessel vasculitis to occur in adults. Although the etiology is not always known, the incidence of vasculitis is increasing, and the diagnosis and management of patients may be challenging because of its relative infrequency, changing nomenclature, and variability of clinical expression. Advances in clinical management have been achieved during the past few years, and many ongoing studies are pending. Vasculitis may affect the large, medium, or small blood vessels. Small-vessel vasculiti…

Anti-neutrophil cytoplasm antibodies are important markers of certain small vessel necrotizing vasculitides, but the optimal use of laborato … l azurophil granule components indicates a drug-induced syndrome. ANCA-related SVV and drug-induced vasculitis or lupus syndromes have characteristic ANCA profiles that can help distinguish these conditions from other inflammatory diseases.

A dramatic case of ibuprofen-induced bullous leukocytoclastic vasculitis (LCV) is described in a patient with a history of prior sensitization to ibuprofen, a common household nonsteroidal anti-inflammatory drug (NSAID) that has few reported adverse skin reactions. Bullous LCV is a relatively rare clinical presentation of LCV, which requires differentiation from other blistering diseases, including bullous erythema multiforme, bullous fixed drug eruption, linear IgA bullous dermatosis, and bullous pemphigoid. The distinctive histopathologic changes of leukocytoclastic vasculitis readily distin…

Takayasu arteritis, Buerger's diseases, temporal arteritis, vascular Behcet disease and inflammatory abdominal aortic aneurysm are classified in Japan as intractable vasculitides involving mainly large vessels, because their etiologies are not yet elucidated and, therefore, treatments for them were not yet established. Recent experimental and vascular biological studies, however, have focussed on the roles of virus infection in vasa vasorum (vasa vasoritis) and on the subsequent inflammatory vascular changes through HLA and/or other autoimmune mechanisms. Several studies including ours have de
