INTRODUCTION: Vertigo, dizziness, and nausea encompass a spectrum of balance-related symptoms caused by a variety of etiologies. Balance is affected by many systems: Proprioceptive pathways and visual, cerebellar, vestibulocochlear, and vascular / vasovagal systems. Vertigo is a subtype of dizziness, in which a subject, as a result to a dysfunction of the vestibular system, improperly experiments the perception of motion. The most useful clinical subdivision is to categorize vertigo into true vertigo and pseudovertigo, whereas from a pathophysiological point of view, vertigo can be classified …

Vertigo is one of the usual menopausal symptoms. We have often examined some women under the complaint of vertigo related with the menopause. We diagnosed each disease based on neuro-otological examinations and investigated the characteristics of menopausal-associated vertigo. We studied 413 women aged 40-59 years old who complained of vertigo. There were 73 women with menopause symptoms (14 women introduced from the gynecologist in our medical center, 18 women had undergone treatment at another female clinic, and 41 women visited an otorhinolaryngologist first) compared with 340 women without…

Dizziness is among the commonest of chief complaints. It often presents a significant challenge to the attending physician, because the symptoms and signs are often vague and non-specific. However, a robust systematic approach can usually arrive at the diagnosis. Maintaining balance requires sensory inputs from the vestibular, visual, and somatosensory systems and the cerebellum fine-tunes inaccurate motor outputs. Causes of vertigo are most commonly otological, followed by central, somatosensory, and visual. The first question in approaching patients with dizziness is to categorise dizziness …

UNLABELLED: CONCLUSION. The recurrence rate of vertigo due to any cause in patients with vestibular neuritis (VN) was about 26.0% in our study. No clinical characteristics were significantly associated with recurrences.
OBJECTIVE: We aimed to: 1) determine the prevalence of recurrent VN; 2) compare the clinical characteristics and epidemiologic factors of patients with non-recurrent and recurrent VN; and 3) determine the prevalence of benign paroxysmal positional vertigo (BPPV) in patients with VN.
METHODS: In 131 patients diagnosed as having VN (68 males, aged 49.8 ± 14.9 years and 63 femal…

Both migraine and vertigo are common in the general population with lifetime prevalences of about 16 % for migraine and 7 % for vertigo. Therefore, a concurrence of the two conditions can be expected in about 1.1 % of the general population by chance alone. However, recent epidemiological evidence suggests that the actual comorbidity is higher, namely 3.2 %. This can be explained by the fact that several dizziness and vertigo syndromes occur more frequently in migraineurs than in controls including benign paroxysmal positional vertigo, Meniere's disease, motion sickness, cerebellar disorders a…

The concept that reactivation of latent neurotropic viruses (i.e. Herpesviridae group) in the vestibular ganglion is responsible for recurrent vestibulopathies is presented. A similar histopathologic degeneration of vestibular ganglion cells in vestibular neuronitis (VN), Ménière's disease and benign paroxysmal positional vertigo is presented to support this concept. The clinical response (relief of vertigo) to the administration of antiviral medication in these syndromes provides practical evidence of a viral neuropathy in patients with recurrent vertigo. Relief of vertigo after this treatmen…

The symptoms associated with somatoform vertigos take on many forms and often appear to be organic in origin. Only a complete medical history in connection with a clinical neurootological examination and psychiatric/psychosomatic exploration can differentiate between organic (e.g., vestibular) and somatoform vertigos. The patients must be informed about this disorder so that the symptoms are taken seriously. Even though they cannot be ascribed to an organic impairment, the symptoms are still clinically significant. Somatoform vertigo is primarily treated with behavior therapy (if necessary, co…

Hearing loss can be caused by acoustic trauma, aging, ototoxic medications, and various other causes. At the cellular level hearing loss seems to be mediated by reactive oxygen species and ultimately through the activation of apoptotic mechanisms. This article explains the cellular and molecular mechanisms of hearing loss and presents medications that could be used in the intratympanic treatment of hearing loss.

Differentiating vertigo from other forms of dizziness presents a diagnostic challenge. Extensive investigation, however, can help reveal vertigo's true etiology: life-threatening central lesions or debilitating, but benign, peripheral causes. Here, learn to assess vertigo using physical examination, history, and diagnostic procedures. Also, manage symptoms of common vestibular disorders, such as benign paroxysmal peripheral vertigo, labyrinthitis, and Ménière's disease.

Findings of electronystagmography (ENG) in 645 patients with vertigo were analysed. Two hundred and twelve cases showed vestibular peripheral vertigo, 394 cases vestibular central vestigo, and 39 cases other types of vertigo. The ENGs in patients with vestibular peripheral vertigo were charazterized mainly by the vestibular abnormal caloric responses, spontaneous and benign positional nystagmuses were also obsedrved at the same time. Different degrees and various combinations of the abnormal responses shown in optokinetic central test were the most characteristic of ENG in patients with vestib…

Three-dimensional analysis of nystagmus was carried out in patients with peripheral vestibular diseases using a computerized image recognition technique developed by us. In the present study, we analyzed data from patients with Meniere's disease and vestibular neuritis with the central premise of localizing the pathology in the peripheral vestibular organs. In Meniere's disease, the recordings of all subjects showed two components of eye movements, namely the horizontal and torsional components. On the other hand, most of the patients with vestibular neuritis exhibited all three components of

We report a family with dominantly inherited migraine headaches, episodic vertigo, and essential tremor. All symptoms improved with the use of acetazolamide. Linkage analysis ruled out linkage to markers on chromosome 19p, known to be linked to the genetic defect in families with the clinically similar syndromes of hemiplegic migraine and periodic ataxia. This genetic heterogeneity of migraine syndromes could result from defects in a family of genes coding proteins with similar properties.

Isolated vertigo is usually a distressing though benign clinical entity. Recently, however, it has been recognized that in a subset of patients isolated vertigo may be a harbinger of vertebrobasilar stroke. Recognition of this "at risk" subgroup may be difficult. Brainstem auditory evoked potentials (BAEPs) are usually normal in patients with vertigo from labyrinthine disease. We describe a patient with isolated vertigo, in whom BAEPs were abnormal, and who subsequently developed an anterior inferior cerebellar artery territory infarct. BAEP testing might lead to early recognition of those pat…

The outcome of unilateral sudden deafness with and without vertigo was evaluated according to the severity of the initial hearing loss, the shape of the audiogram, and other variables. The subjects for this investigation were 1313 patients who had presented to the authors' university hospital from 1972 to 1990 within 2 weeks of the onset of hearing loss. Thirty percent of the patients had accompanying vertigo. Vertigo occurred frequently in patients with severe hearing loss in the high-tone frequencies. Hearing recovery of high-tone frequencies was worse in patients with vertigo than in those …

There are many transient neurologic disturbances associated with various types of migraine. Visual symptoms, such as scintillating scotomata are most common, but somatosensory, motor, cranial nerve, and brain stem symptoms also occur. Among the brain stem symptoms, vestibular manifestations are quite common and include nonspecific dizziness, disequilibrium, vertigo, and motion intolerance. Auditory symptoms are less common. These transient neurologic symptoms can precede the headache as an aura, can occur during the headache, or, uncommonly, can immediately follow the headache. It is also well…

Since episodic vertigo in the pediatric and adolescent age group is unusual and therefore not well known to most otolaryngologists, we present six cases to show some of the various presentations and different underlying causes. These cases covered a wide diagnostic spectrum: meningioma, medulloblastoma, childhood migraine with vestibular symptoms, childhood Meniere's syndrome (one case due to perilymph fistula), and benign paroxysmal vertigo of childhood. The two patients with tumor and the patient with perilymph fistula were treated surgically; the other patients are being managed conservativ…
