Recovery of vestibular evoked myogenic potentials after a vertigo attack due to vestibular neuritis.

CONCLUSIONS: Inferior vestibular nerve functions could recover in patients with vestibular neuritis (VN). OBJECTIVES: Although the recovery of superior vestibular nerve functions has been reported, there is little information about the recovery of inferior vestibular nerve functions in patients with VN. This study was done to clarify if inferior vestibular nerve functions recover after an attack of VN. METHODS: Neuro-otological tests including vestibular evoked myogenic potential (VEMP) testing and caloric testing were sequentially performed in 13 patients with VN, who showed absence of VEMP

Vestibular evoked myogenic potentials: history and overview.

Vestibular evoked myogenic potential (VEMP) testing is a relatively new diagnostic tool that is in the process of being investigated in patients with specific vestibular disorders. In this review, we will outline the history and provide a current review of VEMP research. Briefly, the VEMP is a biphasic response elicited by loud clicks or tone bursts recorded from the tonically contracted sternocleidomastoid muscle. Current data suggest that the VEMP is a vestibulo-collic reflex whose afferent limb arises from acoustically sensitive cells in the saccule, with signals conducted via the inferior

[Function disorders of otoliths: clinical aspects and therapy options].

In the clinical routine, the diagnostic of the lateral gait is often comparable with a complete peripheral vestibular function examination. Diseases of the otolithic organs, with the saccule and utricle are not identified with conventional clinical examination methods. In more than 50% of patients with thermal hypostimulation, orientation tests show a simultaneous pathology of the utricle function. Participation of the saccule has been described for vestibular neuropathy, Menière's disease, bilateral vestibulopathy and acoustic neurinoma. Both otolithic organs are included in posttraumatic ver

Application of the vestibular disorders activities of daily living scale.

OBJECTIVE: Existing scales of functional performance are either insufficiently sensitive or omit some important daily life tasks. This paper demonstrates that a new scale of self-perceived disablement in the vestibularly impaired population-the Vestibular Disorders Activities of Daily Living Scale (VADL)-differentiates between disabled and healthy persons and evaluates the associations of this assessment with other measures of vestibular disorders. STUDY DESIGN: Prospective. METHODS: Subjects were 1) asymptomatic, healthy adults, 2) patients with benign paroxysmal positional vertigo, 3) pati

Clinical-pathologic correlation in a patient with selective loss of hair cells in the vestibular endorgans.

We found a selective loss of vestibular hair cells in a patient followed for more than 10 years with imbalance and oscillopsia due to idiopathic progressive loss of vestibular function. Hearing function and cochlear hair cells were normal. The vestibulo-ocular reflex (VOR) gain at high frequencies was relatively maintained despite marked shortening of the dominant VOR time constant (to less than 500 ms). Ultrastructural examination of remaining hair cells showed mitochondrial abnormalities. The ultrashort VOR time constant probably resulted from changes in firing patterns of the primary affere

Acute Unilateral Vestibular Failure Does Not Cause Spatial Hemineglect.

OBJECTIVES: Visuo-spatial neglect and vestibular disorders have common clinical findings and involve the same cortical areas. We questioned (1) whether visuo-spatial hemineglect is not only a disorder of spatial attention but may also reflect a disorder of higher cortical vestibular function and (2) whether a vestibular tone imbalance due to an acute peripheral dysfunction can also cause symptoms of neglect or extinction. Therefore, patients with an acute unilateral peripheral vestibular failure (VF) were tested for symptoms of hemineglect. METHODS: Twenty-eight patients with acute VF were as

Correlation between caloric and ocular vestibular evoked myogenic potential test results.

CONCLUSION: The ocular vestibular evoked myogenic potential (o-VEMP) test results correlate significantly with caloric test results for patients with acoustic neuroma (AN), but not for patients with Meniere's disease (MD), indicating that the o-VEMP test may replace the caloric test for evaluating the vestibular nerve from which the AN arises. Conversely, the caloric, o-VEMP, and cervical VEMP (c-VEMP) tests should be performed to map lesion sites in the vestibular labyrinth. OBJECTIVE: This study performed caloric, o-VEMP, and c-VEMP tests on patients with central and peripheral vestibular d

Vestibular impairment and cochlear implantation.

CONCLUSION: Cochlear implantation (CI) may induce vestibular impairment soon after surgery as well as after implant activation. This impairment seems to be independent from the cause of deafness and can be considered a possible complication from the intra-operative trauma and, to minor degree, from the ongoing electric stimulation. It would also seem that vestibular damage occurs independently from the likelihood of post-operative hearing deterioration. In unilateral selected CI cases, vestibular examination can be proposed as additional pre-operative exam for selection of the ear to be implan

Prediction of Vestibular Imbalance in Acute Peripheral Vestibulopathy by Measuring Horizontal Ocular Deviation on Magnetic Resonance Imaging.

OBJECTIVES: The aim of the study is to evaluate whether horizontal ocular deviation (OD) from MR imaging in the emergency room (ER) reflects vestibular imbalance, by comparing the horizontal OD in patients with acute vestibulopathy to controls. STUDY DESIGN: Retrospective review. PATIENTS AND METHODS: A total of 69 patients with acute unilateral peripheral vestibulopathy and 30 healthy subjects were included. Horizontal OD was quantified by using the axial T2-weighted fast-spin echo (FSE) images of the brain at 1.5 T. In the study group, the results of VFTs (videonystagmography [VNG], calori

[Dynamic vestibular compensation in vestibular peripheral diseases].

Vestibular compensation, or neuronal plasticity in the central vestibular system, is quite an important process in patients with acute unilateral peripheral vestibular disease, allowing them to lead a comfortable daily life when medical treatments fail to cure the peripheral vestibular function. Is the residual unilateral vestibular input from damaged vestibular endo-organs a positive or negative factor for the development of dynamic vestibular compensation in the central nervous system? To elucidate the true mechanism of vestibular compensation, we examined the ENG findings and dizziness hand

Noise exposure and its effect on the labyrinth, Part II.

The neurootological data of 12 patients attending our ear, nose, and throat clinic were analyzed. The patients submitted to a neurootological routine evaluation consisting of patient history recording and audiometric and equilibriometric investigations (i.e., electronystagmography). The conclusion was that many tinnitus patients--even those who had no vestibular symptoms--showed some disturbance in the vestibular tests, such as spontaneous and positional nystagmus and nystagmus alternans. Laurikainen came to the same conclusion in 1995, after examining 10 patients with sudden deafness but no v

Sensory interaction testing in platform posturography.

Mostly techniques measuring the vestibulo-ocular reflex (VOR) have been used for the evaluation of patients with dizziness problems. Some investigators, however, have also tried to take into account the vestibulospinal reflex (VSR). So recording techniques for the Romberg-test have been proposed and called posturography (PG). By interfering with the visual and proprioceptive sensory inputs during this PG-testing one tries to find out how 'sensory interaction' is organized in the balance performance of the patient examined. To interfere with vision, closure of the eyes has been commonly used an

[Characteristics of vestibulo-ocular interactions in patients with unilateral lesions of the peripheral part of the vestibular system. Sinusoidal rotation. 3].

In 40 patients with unilateral injury of the peripheral compartment of the vestibular system in the stage of decompensation (Group 1) and subdecompensation (Group 2), the vestibulo-ocular reflex (VOR) was investigated during sinusoidal rotation with the eyes closed. Group 1 patients showed a significant asymmetry of the VOR coefficient and a distinct shift of the VOR displacement towards injury during sinusoidal stimulation of any intensity. Group 2 patients displayed similar but less significant changes.
