Association of development quotient with nutritional status of vitamins B6, B12, and folate in 6-59-month-old children: Results from the Brazilian National Survey on Child Nutrition (ENANI-2019).

BACKGROUND: Vitamins B6, B12, and folate are essential for the formation and maintenance of the human brain, but studies evaluating these vitamins with early childhood development (ECD) in children under 5 y are limited and controversial. OBJECTIVES: To evaluate the association between vitamins B6, B12, and folate concentrations/status and ECD. METHODS: Data regarding 6520 children aged 6-59 mo from the ENANI-2019 (the Brazilian National Survey on Child Nutrition) were analyzed. ECD was assessed using the Survey of Well-being of Young Children's milestones questionnaire. Vitamin B6 concentra

Vitamin B12 deficiency and metabolism-mediated thrombotic microangiopathy (MM-TMA).

Thrombotic microangiopathies (TMA) are characterized by microangiopathic hemolytic anemia, thrombocytopenia and organ damage resulting from mechanical factors, accumulation of the ultra-large von Willebrand factor multimers or complement-mediated abnormalities. Severe acquired vitamin B12 (Cobalamin - Cbl) deficiency or congenital defective Cbl metabolism could lead to a picture that mimics TMA. The later has been termed metabolism-mediated TMA (MM- TMA). This confusing picture is mediated partly by the large red cell fragmentation coupled with reduced platelet production in the absence of vit

Update on vitamin B12 deficiency.

Vitamin B(12) (cobalamin) deficiency is a common cause of megaloblastic anemia, a variety of neuropsychiatric symptoms, and elevated serum homocysteine levels, especially in older persons. There are a number of risk factors for vitamin B(12) deficiency, including prolonged use of metformin and proton pump inhibitors. No major medical organizations, including the U.S. Preventive Services Task Force, have published guidelines on screening asymptomatic or low-risk adults for vitamin B(12) deficiency, but high-risk patients, such as those with malabsorptive disorders, may warrant screening. The in

[Vitamin B12 deficiency in the context of obesity surgery].

Bariatric surgery is an efficient treatment option allowing long term weight loss as well as mortality risk reduction and cardiovascular comorbid conditions profile improvement. The number of procedure performed each year is on a constant rise worldwide. Obesity surgery techniques combining malabsorption with a restrictive procedure are associated with a greater excess weight reduction but will also carry more metabolic complications amongst which vitamin B12 deficiency is particularly frequent. The purpose of the present work is to review the hematologic and neurologic complica. tions associa

Comparison of vitamin B12, vitamin D and folic acid blood levels in patients with schizophrenia, drug addiction and controls.

Schizophrenia (SCH) and drug addiction are chronic disorders that are frequently accompanied by physical diseases, poor nutrition and reduced self-care, all of which are likely to result in vitamin deficiencies. The objective of this study was to compare vitamin levels in SCH patients, substance use disorder (SUD) patients and healthy controls (HCs). The study included 189 SCH patients, 119 SUD patients and 109 HCs. Information on vitamin B12, folic acid and vitamin D levels were retrieved from the hospital's database, and mean values and deficiency/insufficiency were evaluated. Vitamin D defi

The Utilization of Serum Folate and Homocysteine Tests and the Prevalence of Folate Deficiency in Reproductive-Age Korean Women during the COVID-19 Pandemic.

We investigated the prevalence of folate deficiency and associated factors in a large population of Korean women of reproductive age during the COVID-19 pandemic. We utilized different cut-offs and evaluated age, year of testing, geographical region, and the utilization of serum homocysteine levels. Out of the 27,758 women evaluated, the overall prevalence of folate deficiency was 12.5% (<4 ng/mL, metabolic indicator) and 5.4% (<3 ng/mL, hematologic indicator). Homocysteine testing was observed in 8.4% of women, with 2.7% having elevated homocysteine levels (>15.4 µmol/L). According to our mul

Brain function in the elderly: role of vitamin B12 and folate.

Vitamin B12 (cobalamin) deficiency associated neuropathy, originally called subacute combined degeneration, is particularly common in the elderly. The potential danger today is that with supplementation with folic acid of dietary staples such as flour, that the incidence of this disease could rise as folic acid, as opposed to natural folate (N5CH3HFGlu1), enters the cell and the metabolic cycle by a cobalamin independent pathway. This chapter briefly describes the clinical presentation of the disease, which unless treated will induce permanent CNS damage. The biochemical basis of the interrela

The Controversial Role of HCY and Vitamin B Deficiency in Cardiovascular Diseases.

Plasma homocysteine (HCY) is an established risk factor for cardiovascular disease CVD and stroke. However, more than two decades of intensive research activities has failed to demonstrate that Hcy lowering through B-vitamin supplementation results in a reduction in CVD risk. Therefore, doubts about a causal involvement of hyperhomocysteinemia (HHcy) and B-vitamin deficiencies in atherosclerosis persist. Existing evidence indicates that HHcy increases oxidative stress, causes endoplasmatic reticulum (ER) stress, alters DNA methylation and, thus, modulates the expression of numerous pathogenic

[Thrombotic microangiopathy caused by vitamin B12 and folate deficiency (pseudo-TMA)].

Thrombotic microangiopathies (TMA) are characterized by microangiopathic hemolytic anemia, thrombocytopenia, and organ damage. As a rare but reversible cause, vitamin B12 and folate deficiency may present as thrombotic microangiopathy (pseudo-TMA) with hemolytic uremic syndrome (HUS)-like clinical features, posing a significant diagnostic challenge. A 56-year-old woman developed severe macrocytic anemia, thrombocytopenia, acute kidney injury, and laboratory signs of hemolysis. Investigations revealed marked folate deficiency, pronounced macrocytosis (MCV>120 fL), low haptoglobin levels, and ma

What ultimately underlies vitamin B12 and folate deficiency in subjects treated with biguanides?

PURPOSE: Metformin, the main biguanide in clinical settings, is the most common antidiabetic agent worldwide and has for years been regarded as a safe treatment option mostly for subjects with type II diabetes mellitus. Nevertheless, long-term metformin treatment leads to vitamin B12 and/or folic acid/folate deficiency in a significant portion of metformin recipients. As these deficiencies, and particularly in the case of vitamin B12, may lead to severe suboptimal health outcomes and are frequently encountered, the current review provides a comprehensive insight into the underlying mechanisms.
