Swallowing occurs preferentially in the expiratory phase of the quiet breathing cycle and at mid-to-low tidal volume. This coordinative pattern imparts important biomechanical advantages to swallowing and airway protection and facilitate laryngeal elevation, laryngeal vestibular and vocal fold closure, and cricopharyngeal sphincter opening. This preferred coordinative relationship between breathing and swallowing is impaired in a variety of patient populations, including head and neck cancer survivors with dysphagia. We developed a training protocol to re-establish more optimal phasing of swal

ObjectivePreliminary findings suggest that different kinds of meditation could work on diverse cognitive and psychological processes. The present study aimed at disentangling the effects of three mindfulness techniques on mental rumination and subjective time perspective. Design: 75 young healthy participants were randomly assigned to one of four conditions: mindful breathing, body scan, observing-thoughts meditation and an active control condition. The meditation groups practiced mindfulness daily and attended weekly group meetings for 8 weeks, while the control group was involved in reading

Osteopathic manipulation has been used to treat a wide range of diseases in largely outpatient settings. The authors describe the emergent use of osteopathic manipulative treatment to improve respiratory mechanics in a critically ill patient with acute respiratory failure. High-velocity mobilization of cervical and thoracic dysfunctions resulted in a decreased work of breathing, improved arterial oxygenation, resolution of tachycardia, and an overall improvement in the patient's clinical condition.

The difficulties inherent in the 'Look, Listen and Feel' method of identifying respiratory arrest prompted the authors to develop a simple mechanical breathing indicator that can show clearly, at a glance, whether or not a patient is breathing. The novel indicator was designed to be highly visible so that its interpretation should be obvious to medical personnel and the lay public, and yet sufficiently simple so that it could be easily and inexpensively incorporated into the type of pocket rescue masks currently in use. The indicator needs no power source, works indoors and outdoors and does n

We studied 31 consecutive mechanically ventilated patients with acute respiratory failure in two stages: (1) During spontaneous breathing through the respirator, switching from full mechanical assistance to continuous positive airway pressure mode with 0 cm H2O pressure. We measured maximum inspiratory pressure and continuously monitored the pattern of breathing. After 8 to 25 min, none of the patients were able to sustain spontaneous breathing and mechanical ventilation was required to resume. (2) Subsequently, during mechanical ventilation, by manipulating the variables of the ventilator we

Little is known about maturation of peripheral chemoreceptor tone (PCT) during growth. We recently demonstrated that the increase in PCT was 49% greater during hypoxic (15% O2) exercise in children compared to adults. As the PCT is a major determinant of ventilatory (VE) response at the onset of exercise (measured by the time constant tau), we hypothesized that hypoxia would affect tau VE (and tau VCO2) to a greater extent in children. Nine healthy children (6-10 y old) and nine healthy adults (18-40 y old) performed multiple transitions from rest to constant work rate on the cycle ergometer.

Tracheal sound sensors provide multiple respiratory signals that are valuable for studying upper airway characteristics. This chapter reviews the original work and ongoing research on tracheal sound analysis in relation to upper airway obstruction during sleep. Past and current research suggest that being associated with other sleep study recording sensors and advanced signal processing techniques, tracheal sound analysis can extensively contribute to the diagnosis and assessment of sleep-disordered breathing.

Chest wall disorders represent deformities and/or injuries that alter the rib cage geometry and result in pulmonary restriction, increased work of breathing, exercise limitations, and cosmotic concerns. These disorders are congenital or acquired and affect all ages. Disorders affecting the spine (kyphoscoliosis, ankylosing spondylitis), ribs (flail chest), and sternum (pectus excavatum) are discussed in this article, with emphasis on clinical presentations, pulmonary function abnormalities, diagnosis, and treatment.

BACKGROUND: The endotracheal tube resistance dominates the total airway resistance in most intubated patients. Mucus deposition and biofilm formation can rapidly increase tube resistance and thereby contribute to serious ventilatory impairments, including dynamic hyperinflation, intrinsic PEEP build-up, added work of breathing, and patient-ventilator asynchrony. During controlled mechanical ventilation, an increased tube resistance can be inferred from the difference between peak and plateau pressure, but this approach fails during pressure-supported spontaneous breathing.
METHODS: We present…

Ketones are well-known biomarkers of fat oxidation produced in the liver as a result of lipolysis. These biomarkers include acetoacetic acid and β-hydroxybutyric acid in the blood/urine and acetone in our breath and skin. Monitoring ketone production in the body is essential for people who use caloric intake deficit to reduce body weight or use ketogenic diets for wellness or therapeutic treatments. Current methods to monitor ketones include urine dipsticks, capillary blood monitors, and breath analyzers. However, these existing methods have certain disadvantages that preclude them from being

Neuronal signaling requires high rates of ATP production via the oxidative metabolism of glucose. The American bullfrog is intriguing, as this species has typical brain energy requirements for an average vertebrate but modifies synaptic physiology and metabolism after hibernation to maintain function during hypoxia and ischemia. Given the importance of the respiratory system in restoring metabolic homeostasis during emergence from underwater hibernation, work to date has addressed this response in the brainstem respiratory network. Thus, metabolic plasticity has been interpreted as an adaptati

Carotid bodies (CBs) are main peripheral chemoreceptors involved in breathing regulation. Despite the well-known role played by CBs on breathing control, the precise contribution of CBs on the regulation of lung mechanics remains controversial. Accordingly, we study changes in lung mechanics in normoxia (FiO2 21%) and hypoxia (FiO2 8%) in mice with or without functional CBs. For this, we used adult male mice that underwent sham or CB denervation (CBD) surgery. Compared to sham-operated mice, we found that CBD induced an increase in lung resistance (RL) while breathing normoxic air (sham vs. CB

A case is described of a 67 year old obese white woman who had a history of multiple medical problems and who was in chronic respiratory failure but responded poorly to intermittent positive pressure breathing, chest physiotherapy, and supplementary oxygen. She was treated successfully with a 600 k.cal diet and a 26-day physical reconditioning programme. Reconditioning techniques included free and treadmill walking, stair climbing, bench stepping, light calisthenics, and breathing retraining. Improvements were noticed in blood gases, spirometry, electrocardiogram, motor coordination, and physi

We describe the underlying theory, design and experimental evaluation of an electromechanical analogue infant lung to simulate spontaneous breathing patterns of preterm infants. The aim of this work is to test the possibility to obtain breathing patterns of preterm infants by taking into consideration the air compressibility. Respiratory volume function represents the actuation pattern, and pulmonary pressure and flow-rate waveforms are mathematically obtained through the application of the perfect gas and adiabatic laws. The mathematical model reduces the simulation interval into a step short

We investigated cardiovascular function and plasma catecholamine response during incremental exercise and recovery in diabetic patients with (DAN+) and without autonomic neuropathy (DAN-). The former group was divided according to the presence of parasympathetic (DAN+PH-) or associated parasympathetic and sympathetic (DAN+PH+) damage to the autonomic nervous system. A group of healthy volunteers was studied as a control group. All the patients and control subjects underwent a submaximal or symptom-limited incremental exercise test using a cycle-ergometer. Air flow and respiratory gas fractions

Orofacial muscles are critical for life-sustaining behaviors, such as feeding and breathing. Centuries of work by neuroanatomists and surgeons resulted in the mapping of bulbar motor neurons in the brainstem and the course of the cranial nerves that carry their axons. Despite the sophisticated understanding of the anatomy of the region, the molecular mechanisms that dictate the development and maturation of facial motor neurons remain poorly understood. This fundamental problem has been recently revisited by physiologists with novel techniques of studying the rhythmic contraction of orofacial

Exhaled breath has recently been identified as a matrix for the detection of drugs of abuse. This work aims to further document this application using a new and simple collection device in patients following recovery from acute intoxication. Breath, plasma and urine samples were collected from 47 patients (38 males, age range 25-74) together with interview data. Analysis of breath and plasma samples was done by liquid chromatography-mass spectrometry methods. Urine was screened using immunochemical reagents and positive findings confirmed with liquid chromatography-mass spectrometry methods. T

The purpose of this work is to evaluate the feasibility of lung imaging using 3D electrical impedance tomography (EIT) during spontaneous breathing trials (SBTs) in patients with acute hypoxic respiratory failure. EIT is a noninvasive, nonionizing, real-time functional imaging technique, suitable for bedside monitoring in critically ill patients. EIT data were collected in 24 mechanically ventilated patients immediately preceding and during a SBT on two rows of 16 electrodes using a simultaneous multicurrent source EIT system for 3D imaging. Dynamic 3D EIT images of conductivity were computed,

This longitudinal study examines changes to speech production and speech breathing in older adults with and without Parkinson’s disease (PD) … higher recoil pressures to generate subglottal pressure for speech production, potentially reducing work of breathing. Participants with PD displayed significant decreases in lung volume initiation and termination. Thus, unlike older adults, people with PD exert more expiratory muscle pressure during speech production, leading to increased effort. Speech-language p …

Some have observed that developed world is fat and getting fatter. This is even extending into the developing world, and it is important to appreciate that the consequences of childhood obesity last into adulthood and are associated with premature death. From the paediatric respiratory perspective, the deposition of excess adipose tissue in the thoraco-abdominal region begins early in life and is believed to alter diaphragm mobility and chest wall expansion, reduce lung compliance, and result in a rapid shallow breathing pattern with an increased work of breathing and reduction in maximum vent…
