A national geostatistical survey for schistosomiasis and soil-transmitted helminths in Zimbabwe following interruption of preventive chemotherapy.

BACKGROUND: Schistosomiasis and soil-transmitted helminthiases (STH) are targeted for elimination as public health problems (EPHP) in Zimbabwe by 2030. A 2018 impact survey demonstrated prevalence below EPHP thresholds following seven rounds of annual preventive chemotherapy (PC). However, the COVID-19 pandemic caused a three-year suspension of PC. We conducted a nationwide geostatistical impact assessment to estimate prevalence at ward-level and evaluated impact of PC interruption relative to 2018 results using a quasi-experimental design. METHODS: The cross-sectional survey in 2021 targeted…

Point-of-care screening for curable sexually transmitted infections in antenatal care in Zimbabwe: a costing study.

BACKGROUND: Sexually transmitted infections (STIs) including Chlamydia trachomatis, Neisseria gonorrhoeae and Trichomonas vaginalis during pregnancy are associated with adverse birth outcomes. However, there are limited published data available on cost estimates for implementing such screening. The aim of this study was to estimate the cost of integrating point-of-care STI screening into antenatal care in Harare, Zimbabwe, compared with a modelled standard of care (syndromic management). METHODS: We implemented screening for C. trachomatis, N. gonorrhoeae and T. vaginalis in antenatal care at…

Risk factors for contracting malaria in six wards of Mudzi District, Zimbabwe: A case control-study.

Despite a significant decline in burden in the last two decades, malaria remains a significant global public health threat. Vector behaviour, climate, ecology, human economic and social behaviour, and quality of housing are some of the established predictors for contracting malaria. Zimbabwe has recorded a significant decline in malaria burden, however, districts like Mudzi continue to experience persistent malaria transmission despite well-performing indoor residual spraying programs. Persistent malaria transmission despite high IRS coverages point to human socio-economic behavior and health …

Morphological and molecular identification of amphistomes of wild ruminants from selected conservancies of Matebeleland region of Zimbabwe.

Amphistomosis is a poorly understood trematode disease of domestic and wild ruminants whose effect has been underestimated. To date, there is limited information on the taxonomic status of amphistome species, particularly in sub-Saharan Africa which is home to diverse species infecting domestic and wild ruminants. In view of this, a survey was conducted in wildlife conservancies and national parks located in Beitbridge, Nyamandlovu, and Hwange in the Matebeleland region of Zimbabwe during the 2019-2021 game hunting seasons. Gastrointestinal tract of 329 carcasses of wild ruminants were examine…

Prevalence of HIV-associated osteoporosis and fracture risk in midlife women: a cross-sectional study in Zimbabwe.

Antiretroviral therapy roll-out has dramatically reduced HIV-related mortality; more women are living to reach menopause. Menopausal estrogen loss causes bone loss, as does HIV and some of its treatments. However, data describing HIV's impact on osteoporosis prevalence and fracture risk are scarce in southern Africa. A cross-sectional study of women aged 40-60 years (49% women with HIV [WLH]) was conducted in Harare, Zimbabwe. Menopause, fracture, and HIV history were collected, and anthropometry and BMD (by DXA) measured, and FRAX 10-year fracture probabilities quantified. The FRAX probabilit…

Stigma, Social Cohesion, and HIV Risk Among Sexual and Gender Minorities in Two Cities in Zimbabwe.

Though stigma is a recognized contributor to the disproportionate HIV burden among sexual and gender minorities (SGM) in sub-Saharan Africa, data describing this association among Zimbabwean SGM are limited. We examined relationships between SGM stigma and HIV and the potential for social cohesion to moderate the association among Zimbabwean men who have sex with men, transgender women, and genderqueer individuals. Consenting participants (n = 1511) recruited through respondent-driven sampling for a biobehavioral survey in Harare and Bulawayo completed structured interviews and received HIV te…

Should disability-inclusive health be a priority in low-income countries? A case-study from Zimbabwe.

The National Disability Policy was launched in Zimbabwe in June 2021 and includes a range of commitments for the provision of disability-inclusive health services and rehabilitation. Fulfilment of these pledges is important, as at least 7% of the population have disabilities, and people with disabilities face greater challenges accessing healthcare services and experience worse health outcomes. However, it will require financial investment which is challenging as the needs of people with disabilities are set against a background of widespread health systems failures in Zimbabwe, exacerbated by…

Effects of repeated blood donation on iron status of blood donors in Zimbabwe: A cross-sectional study.

INTRODUCTION: Iron deficiency is a major complication of repeated blood donation. However, most of the blood screening methods employed by blood collection agents do not include iron status markers, leading to possible subclinical iron deficiency. The aim of this study was to evaluate the effects of repeated blood donation on the iron status of this vulnerable population in Zimbabwe. METHODS: All donors were categorized into groups based on number of donations made in the previous 2-year period prior to enrolment into the study. Serum iron, total iron-binding capacity (TIBC), and ferritin wer…

Harnessing of Social Capital as a Determinant for Climate Change Adaptation in Mazungunye Communal Lands in Bikita, Zimbabwe.

The livelihoods of rural people have been plagued by the precarious impacts of climate change-related disasters manifesting through floods, heat waves, droughts, cyclones, and erratic temperatures. However, they have not remained passive victims to these impacts. In light of this, rural people are on record of employing a plethora of adaptation strategies to cushion their livelihoods from climate change impacts. In this vew, the role of social capital as a determinant of climate change adaptation is underexplored. Little attention has been paid to how social capital fostered through trust and

Estimated costs for the delivery of safer conception strategies for HIV-discordant couples in Zimbabwe: a cost analysis.

BACKGROUND: In recent years, safer conception strategies have been developed to help HIV-serodiscordant couples conceive a child without transmitting HIV to the seronegative partner. The SAFER clinical trial assessed implementation of these strategies in Zimbabwe. METHODS: As a part of the SAFER study, we estimated the costs (in 2017 $US) associated with individual and combination strategies, in the trial setting and real-world practice, from a healthcare system perspective. Safer conception strategies included: 1) ART with frequent viral load testing until achieving undetectable viral load (…

Impact of the adolescent and youth sexual and reproductive health strategy on service utilisation and health outcomes in Zimbabwe.

Poor reproductive health among youth and adolescents threatens their future health and economic wellbeing in Zimbabwe amidst a high HIV/AIDS prevalence. This study evaluates the impact of a multi-pronged adolescent sexual and reproductive health (ASRH) strategy implemented by government of Zimbabwe between 2010 and 2015 to improve ASRH in terms of the uptake of condoms and HIV testing as well as outcomes in terms of sexually transmitted infection (STI) prevalence and HIV prevalence. We combine the difference in difference and propensity score matching methods to analyse repeated Zimbabwe demog…

Evaluation of the early warning indicators of HIV drug resistance surveillance system in Manicaland province, Zimbabwe.

More than 1.1 million people currently receive lifelong antiretroviral treatment in Zimbabwe following the adoption of the test and treat strategy in 2017. The huge numbers of people on antiretroviral therapy (ART), combined with HIV's error-prone replication, increases the probability of HIV drug resistance (HIVDR) developing. HIVDR in resource-limited settings like Zimbabwe has significant human and financial implications. The early warning indicators of HIV drug resistance surveillance system was set up to monitor the ART programme and to identify factors that could raise the risk of HIVDR.…
