Complementary use of computed tomographic angiography in treatment planning for posterior fossa subarachnoid hemorrhage.

OBJECTIVE: The goal of this study was to determine whether the complementary use of computed tomographic angiographic (CTA) assessments would provide additional benefits in the evaluation and treatment of ruptured vertebrobasilar artery aneurysms. METHODS: In the past 4.5 years, 35 patients with an infratentorial dominant pattern of subarachnoid hemorrhage were complementarily examined with CTA scanning, after undergoing one or more three-dimensional rotational projection digital subtraction angiographic (DSA) studies. The results of these studies were interpreted by the treating neurosurgeon

Treatment of basilar artery embolism with a mechanical extraction device: necessity of flow reversal.

BACKGROUND AND PURPOSE: The success of local fibrinolysis in vertebrobasilar thromboembolism depends on the volume and composition of the clot. Since thrombolysis can also be time consuming and cause bleeding, we investigated the feasibility of a mechanical clot retraction device based on a nitinol basket advanced through a microcatheter. METHODS: Five patients with acute embolism of the basilar artery who presented with progressive stroke and impaired consciousness were included in a multicenter study (Neuronet Evaluation in Embolic Stroke Disease [NEED]). In 3 patients flow reversal was ind

Two-dimensional time-of-flight magnetic resonance angiography of renal arteries without maximum intensity projection: a prospective comparison with angiography in 21 patients screened for renovascular hypertension.

PURPOSE: We compared magnetic resonance angiography (MRA) with conventional angiography to establish its value as a screening test in the workup for renal hypertension. METHODS: Twenty-one patients underwent MRA and angiography within a three-day interval. Fifteen patients were suspected of having renovascular hypertension on the basis of clinical findings; the remaining six had multivessel atherosclerosis with renal insufficiency. MRA was performed on a 1 Tesla magnet in three planes: axial, coronal and perpendicular to the axis of each renal artery, by means of several contiguous or overlap

Common Data Elements for Radiological Imaging of Patients with Subarachnoid Hemorrhage: Proposal of a Multidisciplinary Research Group.

INTRODUCTION: Lack of homogeneous definitions for imaging data and consensus on their relevance in the setting of subarachnoid hemorrhage and unruptured intracranial aneurysms lead to a difficulty of data pooling and lack of robust data. The aim of the National Institute of Health/National Institute of Neurological Disorders and Stroke, Unruptured Intracranial Aneurysm (UIA) and Subarachnoid Hemorrhage (SAH) Common Data Elements (CDE) Project was to standardize data elements to ultimately facilitate data pooling and establish a more robust data quality in future neurovascular research on UIA a

Computed Tomography Angiography Recognition of a Hemorrhagic Moyamoya Pattern May Alter Acute Blood Pressure Management.

Moyamoya disease (MMD) is a rare progressive cerebrovascular occlusive disorder characterized by stenosis of the terminal internal carotid arteries and the development of fragile lenticulostriate collateral vessels. In adults, MMD can manifest as intracranial hemorrhage (ICH), carrying significant morbidity and a high risk of recurrent bleeding. When hemorrhagic MMD coexists with bilateral large vessel occlusion, acute management becomes exceptionally complex, as the standard imperative to aggressively lower blood pressure following ICH directly conflicts with the hemodynamic requirements of c

Clinical Insights on My Acute Ischemic Stroke Caused by a Carotid Web.

Acute ischemic stroke, a medical emergency caused by reduced cerebral blood flow, results in brain cell damage. While commonly associated with older individuals, strokes can also occur in young and middle-aged adults, posing significant socio-economic and health challenges due to the long-term impact of the condition. This poses significant socio-economic and health challenges because stroke is a leading cause of disability and mortality. A carotid web (CaW), a rare form of fibromuscular dysplasia, is increasingly recognized as a cause of cryptogenic stroke in young adults. Characterized by a

Endovascular treatment of very small and very large ruptured aneurysms of the anterior cerebral circulation: a single-center experience.

INTRODUCTION: Endovascular treatment of aneurysms has been introduced as a less invasive method for decreasing the rate of aneurysm rerupture and subsequent subarachnoid hemorrhage. The outcome and complication rate for endovascular treatment of very small (≤3 mm) and very large (15-25 mm) intracranial aneurysms has been controversial. Here we report our experience with endovascular coiling of very small and very large ruptured aneurysms of the anterior cerebral circulation. METHODS: Patients were included in the study if the maximum dimension of the intracranial ruptured aneurysm was reporte

[Renovascular hypertension: is it only the top of the iceberg?].

Essential hypertension accounts for 95% of all cases of hypertension. A small number of patients (between 2% and 5%) have a reversible disease as the cause for raised blood pressure. Unilateral and bilateral renal artery stenosis may be responsible for secondary hypertension. Diagnosis and treatment of renal artery stenosis are of a great importance. Revascularization of ischemic kidney may correct blood pressure control and preserve renal function. Much data suggest close pathophysiological relation between renal artery stenosis, ischemic nephropathy and development of hypertension. However,

Optic Nerve MRI Signal Alterations in Dural Arteriovenous Fistulae: Correlations With Papilledema and Visual Decline.

BACKGROUND: Visual decline in dural arteriovenous fistulae (DAVF) patients is attributed to venous hypertension, raised intracranial pressure and resultant optic nerve axoplasmic flow stasis. Objective imaging biomarkers for optic nerve involvement are however limited. PURPOSE: To determine whether 3D fluid attenuated inversion recovery (FLAIR) optic nerve signal intensity ratio (SIR) is associated with papilledema and visual deterioration in DAVF patients. STUDY TYPE: Single-center retrospective observational study. POPULATION: 54 digital subtraction angiography (DSA) confirmed DAVF patien

Radiation exposure in neurosurgical intensive care unit patients: Balancing diagnostic benefits and long-term risks.

BACKGROUND: X-ray, computed tomography (CT), and digital subtraction angiography (DSA) techniques are indispensable in managing critically ill neurosurgical patients. However, repeated diagnostic imaging leads to cumulative radiation exposure, raising concerns about long-term risks such as malignancies. This study evaluates the frequency, dosage, and implications of radiation exposure in a neurosurgical intensive care unit (NICU) patient cohort. METHODS: A retrospective analysis was conducted on 589 patients admitted to the NICU between 2013 and 2018 with the diagnosis of traumatic brain inju

Use of Prospective Radiobrachial Angiography in Transradial Cardiac Catheterization and Intervention.

OBJECTIVES: This study examined the utility of prospective radiobrachial angiography (pRBA) in transradial coronary angiography and intervention as a method for reducing procedural complications. BACKGROUND: A growing body of evidence has supported the transradial approach (TRA) as superior to the transfemoral approach (TFA) due to advantages such as reduced bleeding and improved outcomes in high-risk patients. However, TRA has a higher failure rate than TFA, and has seen slow rates of adoption among United States operators. METHODS: This was a retrospective, single center, case-control anal

Entrapment of the basilar artery within a clivus fracture: Case report and literature review.

A review of the literature revealed that basilar artery (BA) entrapment is a very rare (17 cases published) and severe pathological condition, which often leads to death. We report the case of a 72-year-old man who presented with a longitudinal clivus fracture associated with a basilar artery entrapment. This entrapment was responsible for a basilar artery dissection, which led to an ischemic stroke in the pons. The patient was managed with medical treatment, mainly to avoid a progression towards an ischemic stroke. It consisted of heparin therapy followed by antiplatelet therapy, which finall

Induced-Wedge Technique to Improve Liquid Embolic Agent Penetration into Spinal Dural Arteriovenous Fistula.

OBJECTIVE: To promote embolization of spinal dural arteriovenous fistula (SDAVF) with a small long tortuous feeder that may impede successful application of an embolic agent. METHODS: An induced-wedge technique was developed in which blood flow was blocked by coiling at the proximal part of the microcatheter for secure injection of N-butyl cyanoacrylate. We used this technique in 11 patients with SDAVF (male/female ratio, 8:3): 7 thoracic, 2 lumbar, 1 sacral, and 1 cervical. Clinical outcome was evaluated by the Aminoff-Logue Disability Scale. RESULTS: Penetration of embolic agent through th

Endovascular treatment of cystic adventitial disease of the popliteal artery.

Cystic adventitial disease of the popliteal artery is an uncommon cause of intermittent claudication in young patients. Several treatment options are available, oriented to either drainage of the cyst and/or arterial reconstruction. Endovascular techniques have been used in exceptional cases to treat this condition, mostly balloon angioplasty and only 2 cases of stenting, with mixed to poor results. We report the case of a 36-year-old female patient with claudication who was treated with primary stenting with long-term symptomatic relief and arterial patency.
