Total elbow arthroplasty in obese patients.

BACKGROUND: The prevalence of obesity in the United States has increased in recent decades. The aim of this study was to evaluate the influence of obesity in patients undergoing primary total elbow arthroplasty. METHODS: From 1987 to 2006, 723 primary semiconstrained, linked total elbow arthroplasties were performed in 654 patients. The average patient age (and standard deviation) at the time of surgery was 62.3 ± 13.7 years, with 550 total elbow arthroplasties (76%) performed in women. Total elbow arthroplasties were used to treat inflammatory conditions in patients undergoing 378 total elbo

Outcome of supracondylar fractures of the humerus in children treated by closed reduction and percutaneous pinning.

Supracondylar fractures of humerus in children are common injuries. Displaced fractures are inherently unstable. Conservative treatment results in malunion. Open reduction and internal fixation (ORIF) is more invasive and recovery is prolonged. From September 2004 to September 2005, 102 displaced supracondylar fractures of humerus, aged between one and half year to 13 years, were treated using close reduction and percutaneous Kirschner (K) wire fixation under c-arm fluoroscopy. Seventy nine patients were treated by cross K-wires and in twenty three cases lateral two K-wires were put. Above elb

A biomechanical comparison of intramedullary nailing systems for the humerus.

This study evaluated the rotational and bending stability of three interlocking nail systems in paired cadaveric humeri. The Russell-Taylor Humeral Interlocking Nail, the Seidel Humeral Locking Nail, and the True-Flex Humeral Nail were mechanically tested in torsion and four-point bending. The Russell-Taylor and the Seidel interlocking nails are reamed systems that rely on proximal interlocking screws and distal screws or phalanges respectively for rotational stability. However, axillary nerve damage may result during proximal screw placement, and these systems exhibit low resistance to rotati

Interpretation of Post-operative Distal Humerus Radiographs After Internal Fixation: Prediction of Later Loss of Fixation.

PURPOSE: Stable fixation of distal humerus fracture fragments is necessary for adequate healing and maintenance of reduction. The purpose of this study was to measure the reliability and accuracy of interpretation of postoperative radiographs to predict which implants will loosen or break after operative treatment of bicolumnar distal humerus fractures. We also addressed agreement among surgeons regarding which fracture fixation will loosen or break and the influence of years in independent practice, location of practice, and so forth. METHODS: A total of 232 orthopedic residents and surgeons

Iliac bone-block autograft for posterior shoulder instability.

INTRODUCTION: Posterior shoulder instability is a rare condition, representing only 4% of all shoulder-joint instabilities. Numerous surgical techniques are used to treat it when conservative functional treatment proves to be insufficient. This retrospective study relates to 8 patients, presenting recurrent posterior shoulder instability, all treated with a posterior iliac bone-block procedure. The results were assessed both clinically and with contemporary imaging techniques. MATERIALS AND METHODS: A unique identical surgical technique was used in all these cases including a posterior deltoi

Pectoralis major tendon repair outcomes: comparison of transosseous tunnel and cortical button fixation techniques.

BACKGROUND: The traditional gold standard for repairing pectoralis major tendon ruptures involves the utilization of a cortical trough and transosseous bone tunnels. However, it has been shown to create stress risers that could increase the risk of developing humeral fractures. Modern repair techniques have evolved using either anchors or cortical button fixation. There is currently no clinical outcome data comparing the outcomes between the 2 repair methods. This retrospective cohort study aims to compare the clinical and patient-reported outcomes between the cortical button and transosseous …

Intelligent anatomic design of porous radial head prosthesis and microscopic-macro biomechanical finite element analysis in the long-term after replacement surgery.

BACKGROUND: The microporous structure of porous titanium alloy may affect osteoblast differentiation and reduce effective elastic modulus (EEM) of the prostheses. Therefore, the biomechanics-based anatomic design of porous radial head prosthesis (PRHP) may help promote bone healing and reduce postoperative complications. METHODS: A microscopic-macro virtual testing platform (VTP) was built to design cells with excellent mechanical properties, further, to construct the PRHP. An intelligent anatomic platform of healthy human elbow-forearms was developed to construct finite element (FE) models o

Plate osteosynthesis of proximal humerus fracture is associated with significant complications and poor functional score for patients older than 45 years: a cohort study.

BACKGROUND: Despite the introduction of new implants and biomechanical improvements for proximal humerus fractures (PHF), the complication rate remains significant, particularly in older adults. This study aimed to identify risk factors for complications and failure following plate osteosynthesis for PHF and discuss an age cut-off that surgeons should consider for complications. METHODS: Adults with PHF treated with the proximal humerus internal locking system at our large tertiary academic referral center between 2015 and 2020 were retrospectively identified. Univariate analysis was used to
