[Selective and non-invasive monitoring of the posterior columns and pyramidal tract during surgery of the spine and spinal cord].

For intra-operative neurophysiological monitoring during spine and spinal cord surgery, the challenge is to detect, in real-time, the occurrence of neurological impairment at onset in order to remedy the problem as quickly as possible before it becomes definitely acquired and irreversible. The past three decades were marked by considerable technical progress. Feasibility and reliability have now reached a very high level. A multimodal approach combining simultaneous monitoring of somatosensory and motor pathways has enabled a considerable decline in the rate of false negatives reported in the

Post dural puncture headache in obstetric patients: experience from a West African teaching hospital.

BACKGROUND: This prospective, non-randomised study examined the frequency and severity of post dural puncture headache in 96 Ghanaian women who consented to spinal anaesthesia for caesarean section at the Korle Bu Teaching Hospital, Accra, Ghana. METHOD: Spinal anaesthesia was performed using 22-gauge (n = 22), 25-gauge (n = 46) or 26-gauge (n = 38) Quincke needles. Patients were followed up to determine the incidence and severity of post spinal headache. RESULT: The overall incidence of post dural puncture headache was 8.3%, but was significantly higher (33%) in patients in whom 22-gauge Qu

Somatosensory evoked potential surgical monitoring. Observations during combined isoflurane-nitrous oxide anesthesia.

Somatosensory evoked potentials are used to monitor the integrity of the of the spinal cord during surgical procedures where the spinal cord is at risk. Changes in somatosensory evoked potential latencies and amplitudes can indicate impending structural damage. This use of somatosensory evoked potentials is complicated by the effects of nonsurgical factors, including anesthetics. Fifty patients were studied during surgery to determine the effects of coadministered isoflurane and nitrous oxide on cortical and subcortical somatosensory evoked potentials potentials following stimulation of the ti

Minimally invasive transforaminal lumbar interbody fusion: one surgeon's learning curve.

BACKGROUND CONTEXT: The published literature has not characterized the surgeon's learning curve with the technically demanding technique of a minimally invasive transforaminal lumbar interbody fusion (MIS TLIF). PURPOSE: To characterize based on intra- and perioperative parameters, the learning curve for one spine surgeon during his initial phases of performing an MIS TLIF. STUDY DESIGN/SETTING: Retrospective analysis of a single institution and single surgeon experience with the unilateral MIS TLIF technique between July 2008 and April 2011. PATIENT SAMPLE: Sixty-five consecutive patients,

Use of fast-sequence spine MRI in pediatric patients.

OBJECTIVE: The immediate and long-term risk of anesthesia in the pediatric population is controversial. Traditional spine MRI protocols require the patient to remain still during the examination, and in young children this frequently results in the need for sedation administration. The authors' goal was to develop an abbreviated spine MRI protocol to reduce sedation administration in young patients undergoing spine MRI. METHODS: After IRB approval, the medical records of all pediatric patients who underwent a fast spine MRI protocol between 2017 and 2019 were reviewed. The protocol consisted

Mechanism of action of extradural analgesia.

The factors governing the spread and site of action of local anaesthetics in the extradural space are discussed. Local anaesthetic drugs injected into the extradural space may cause nerve blocks at three possible sites: (a) On the spinal nerves in the paravertebral space. This probably occurs only in young subjects. (b) On the spinal nerves intradurally. This is probably the essential site of blockade. (c) On the spinal cord.

Critical Review and Consensus Statement for Neural Monitoring in Otolaryngologic Head, Neck, and Endocrine Surgery.

BACKGROUND: Enhancing patient outcomes in an array of surgical procedures in the head and neck requires the maintenance of complex regional functions through the protection of cranial nerve integrity. This review and consensus statement cover the scope of cranial nerve monitoring of all cranial nerves that are of practical importance in head, neck, and endocrine surgery except for cranial nerves VII and VIII within the temporal bone. Complete and applied understanding of neurophysiologic principles facilitates the surgeon's ability to monitor the at-risk nerve. METHODS: The American Academy o

INTRODUCTION: Minimally invasive percutaneous CT-guided biopsies of the spine are well established. Although, technical difficulties with CT-guided percutaneous intervention of the C2 body present due to the smaller size of the vertebral elements and potential injury of the critical adjacent structures such as Carotid arteries laterally, vertebral artery, and cervical nerve posterolaterally, pharynx and larynx anteriorly, and spinal cord in the cervical spinal canal. Routinely transoral approach is used for the percutaneous intervention of the C2 body. Literature on the posterolateral approach

Using chloroprocaine for spinal anaesthesia in outpatient knee-arthroscopy results in earlier discharge and improved operating room efficiency compared to mepivacaine and prilocaine.

PURPOSE: Knee arthroscopies are regularly carried out in an outpatient setting. The purpose of this retrospective analysis was to investigate the impact of different local anaesthetics for spinal anaesthesia on operating room efficiency (perioperative process times) and postoperative recovery. This study aims to determine the optimal LA for SPA in patients undergoing knee arthroscopy at a day-surgery centre. METHODS: Anaesthesia records of all patients undergoing knee arthroscopy under spinal anaesthesia from 2010 until 2017 were analysed. Patients were categorised as having received spinal a

[CT-guided electrode placement for sacral nerve stimulation in the treatment of faecal incontinence (cSNS)].

BACKGROUND: The sacral nerve stimulation (SNS) can be performed in the screening phase under local anaesthesia. Implantation of the tined-lead electrodes is usually performed in an inpatient setting under general anaesthesia. An outpatient procedure for both PNE and implantation of the electrodes offers decisive advantages with respect to the accuracy of electrode placement. MATERIALS AND METHODS: From 2006 to 2011 a total of 51 patients was treated with SNS in an outpatient setting. RESULTS: Of 51 patients having the PNE, in four patients the procedure could not successfully be completed. I

ECG and cardiac enzymes after glycine absorption in transurethral prostatic resection.

The electrocardiogram (ECG) was recorded and the serum levels of creatine kinase (CK), aspartate transaminase (ASAT) and prostatic acid phosphatase (PAP) was measured in 22 patients undergoing transurethral resection of the prostate (TURP) under spinal or epidural anaesthesia. The irrigating fluid consisted of 1.5% glycine and 1% ethanol, and absorption of the fluid was monitored by detection of alcohol in the expired breath. The results show that nine of the 11 patients (82%) who absorbed more than 1,000 ml of irrigating fluid had developed ECG changes 24 h after the operations. The most comm

Long-term follow-up of epidural blood patch.

Epidural blood patch (EBP) was performed for the treatment of severe postlumbar puncture cephalalgia in 118 young patients. Following the first EBP, 105 patients had relief of headache. Eleven of the 13 in whom it failed had a second EBP, with adequate relief in 10, giving an overall success of 97.5 percent. Lumbar epidural, caudal, and spinal procedures were successful in 3 patients 105 to 380 days after EBP. Soon after EBP, one patient developed facial paralysis and one complained of episodes of vertigo, dizziness, tinnitus, and ataxia without headaches. Residual complications included backa
