Multi-target strategies for the improved treatment of depressive states: Conceptual foundations and neuronal substrates, drug discovery and therapeutic application.

Major depression is a debilitating and recurrent disorder with a substantial lifetime risk and a high social cost. Depressed patients genera … road palette of observations, ranging from mechanisms of action of adjunctive drug combinations and electroconvulsive therapy to "network theory" analysis of the etiology and management of depressive states. The review also outlines opportunities to be exploited, and challenges to be addressed, in the discovery and characterization of drugs recognizing multiple targets. Fi …

Catatonia and the neuroleptics: psychobiologic significance of remote and recent findings.

The previously common occurrence of catatonic schizophrenia and catatonic symptoms among schizophrenic patients has diminished sharply; cata … MS, and treatment of catatonia requires avoidance of neuroleptics and the use of benzodiazepines or electroconvulsive therapy (ECT). The extreme negativism and constriction of consciousness in catatonia suggest a primary role of the frontal lobes, with secondary involvement of the extrapyramidal system and its movement disorders. In an attempt to integrate clinical, psycho …

Sex differences in the antidepressant-like potential of repeated electroconvulsive seizures in adolescent and adult rats: Regulation of the early stages of hippocampal neurogenesis.

Age and sex are critical factors for the diagnosis and treatment of major depression, since there is a well-known age-by-sex difference in the prevalence of major depression (being females the most vulnerable ones) and in antidepressant efficacy (being adolescence a less responsive period than adulthood). Although the induction of electroconvulsive seizures (ECS) is a very old technique in humans, there is not much evidence reporting sex- and age-specific aspects of this treatment. The present study evaluated the antidepressant- and neurogenic-like potential of repeated ECS across time in adol

Combining ECT with pharmacological treatment of depressed inpatients in a naturalistic study is not associated with serum BDNF level increase.

BACKGROUND: BDNF blood levels are reduced in MDD. They can be increased with pharmacologic treatment and ECT, but it is not clear whether the combination of treatments promotes an additional increase. The present study aims to evaluate whether combined treatment promotes an increase in BDNF, restoring the level to that of non-depressed controls. METHODS: Ninety-nine adult inpatients were invited to participate in this naturalistic prospective cohort study between May 2011 and April 2013. Diagnosis was made by MINI, and the symptoms were evaluated at admission and at discharge by HDRS-17. Thos

Severe depression masquerading as Creutzfeldt-Jakob disease.

We report a case of melancholic depression with catatonic features presenting as a rapidly progressive organic brain syndrome, initially thought to be probable Creutzfeldt-Jakob disease. The case highlights the fundamental importance of thorough exclusion of treatable pathology masquerading as an irreversible syndrome.

Serial proton spectroscopy in a case of adult-onset subacute sclerosing panencephalitis.

A case of subacute sclerosing panencephalitis in a 27-year-old man was serially evaluated with proton magnetic resonance spectroscopy. Metabolic abnormalities included decreased N-acetylaspartate and elevated choline and myo-inositol in a lesion visible on magnetic resonance imaging and in normal-appearing white matter. Lactate appeared increased within the lesion. Metabolic impairment was persistent after intrathecal interferon-alpha treatment. Spectroscopy pointing to ongoing inflammation, gliosis, and possible membrane turnover was more sensitive than imaging in detecting widespread patholo

Increased cortical GABA concentrations in depressed patients receiving ECT.

OBJECTIVE: Reduced gamma-aminobutyric acid (GABA) concentrations have been reported in the plasma, CSF, and cortex of depressed subjects. Of interest is that ECT, one of the most effective treatments for severe refractory depression, produces considerable anticonvulsant effects that may be related to increased GABAergic transmission. The purpose of this study was to determine if cortical GABA concentrations increase following a course of ECT. METHOD: Occipital cortex GABA concentrations in eight depressed patients were measured by using proton magnetic resonance spectroscopy before and after

The benefits and risks of ECT for patients with primary dementia who also suffer from depression.

BACKGROUND: Major depression afflicts 20-25% of patients with dementia. Of these, about a third do not improve with antidepressant therapy and may be suitable candidates for electronconvulsive treatment (ECT). However, the use of ECT is dementia patients is concerning due to possible adverse effects on memory and cognition. Outcome studies of ECT in patients with primary dementia and depression are very rare. OBJECTIVE: To determine the effectiveness and complications of ECT treatment for depression in dementia. METHOD: A chart review was conducted of all 31 patients wit ha discharge diagnos

Tardive dyskinesia in schizophrenic outpatients.

A study of the side effects of phenothiazine drug treatment on 211 schizophrenic patients was carried out using the Simpson Scale. There were 21 cases of tardive dyskinesia, giving a prevalence of 10% which is low compared with other studies. Compared with the control patients without tardive dyskinesia, there was no significant relationship between tardive dyskinesia and variables like sex, age group, duration of drug treatment, ECT, benzhexol, and anticholinergic and other side effects, as reported in Western countries.

Residual learning capability in organic amnesia.

This article provides a review of residual learning capability in patients suffering from the organic amnesic syndrome. It is shown that organic amnesics are able to learn a considerable number of laboratory tasks, many to a level comparable with normals. It shown that the tasks on which they succeed invariably involve testing situations which do not refer to specific preceding events (e.g. pursuit rotor, maze learning, picture completion, conditioning). It is concluded that this pattern of preserved learning capability can be interpreted as an impairment of episodic storage with the relative

Hormones, electrolytes, and cerebrospinal fluid proteins in manic-melancholic patients.

The concentrations of insulin and thyroid hormones, tryptophan, electrolytes, urea, plasma proteins in the cerebrospinal fluid, and glucose in blood and cerebrospinal fluid in manic-melancholic patients were studied. As control groups served patients suffering from other psychiatric disorders as well as neurological and orthopedic patients. Apart from the blood values of thyroid hormones, the results showed no differences between the various diagnostic groups, neither in the abnormal states nor when recovered. For blood thyroxine and free thyroxine index, a statistically significant difference

Variation in public health reimbursement policies of ECT and rTMS across 28 European countries: A call for evidence-based policy reform from the ECNP Neuromodulation Network.

Mental disorders contribute significantly to the disease burden in Europe. Brain stimulation treatments such as electroconvulsive therapy (ECT) and repetitive transcranial magnetic stimulation (rTMS) are supported by strong evidence and are recommended in clinical guidelines in many countries. However, access to these interventions remains highly inconsistent across Europe. Reimbursement by public health services plays a critical role in enabling equitable access to these therapies. In this study, we examined disparities in national reimbursement policies for ECT and rTMS for the treatment of …

Comparing the temporal pattern of symptom change across multiple treatments for depression.

BACKGROUND: Depression is increasingly understood as a complex dynamic network of individual depressive symptoms that change over time and are causally related to each other. Directed dynamic time warping (DTW) is a novel method for examining the temporal pattern of symptom change, hereby identifying symptoms that tend to remit earlier and those that improve at later stages of treatment. We compared directed depressive symptom dynamics across psychotherapy, pharmacotherapy and electroconvulsive therapy (ECT). METHODS: Time series data from three observational studies (on ECT) and two randomiz…

Impact analysis of expanded access to ketamine for treatment-resistant depression.

Aim: This study aimed to estimate the economic impacts of expanded access to ketamine relative to electroconvulsive therapy (ECT) by offering intravenous ketamine to US patients with nonpsychotic treatment-resistant depression (TRD) and moderate-to-severe depression. Materials & methods: A population-level Markov simulation model with key parameters from a randomized trial was used to simulate the economic impacts of managing TRD with intravenous ketamine versus ECT over a 5-year horizon. Health states included response of depression in the acute treatment phase and continued treatment and rel…

The influence of age on ECT efficacy in depression, mania, psychotic depression and schizophrenia: A transdiagnostic analysis.

AIM: ECT use is variable across age groups. We aim to investigate the effect of age on ECT response among patients with depression, psychotic depression, mania and schizophrenia. METHODS: Our retrospective observational study included patients from the Institute of Mental Health (Singapore) who were initiated on ECT (March 2017-February 2023). MADRS and BPRS scores were assessed 1-2 days before the first ECT and after the 6th session. The association between age group and ECT response was analyzed by ANOVA and generalized linear regression. A one-way sensitivity analysis was performed with ag

Psychiatric, Motor, and Autonomic Effects of Bifrontal ECT in Depressed Parkinson's Disease Patients.

OBJECTIVE: Depressive symptoms are a source of significant morbidity in Parkinson's disease (PD). Electroconvulsive therapy (ECT) is a promising treatment for depression in PD (dPD); however, data remain limited, including data on optimal electrode placement. In this retrospective study, the investigators aimed to characterize the effects of bifrontal ECT for dPD on psychiatric and motor symptoms, as well as autonomic response. METHODS: Clinical data were retrieved from a university-affiliated ECT service in Vancouver, British Columbia, for patients with dPD receiving bifrontal ECT between 20…
