Long-term safety of recombinant human growth hormone in turner syndrome.

CONTEXT: Turner syndrome (TS) affects more than 50,000 girls and women in the United States. The National Cooperative Growth Study (NCGS) has collected efficacy and safety data for 5220 TS children treated with recombinant human GH (rhGH) during the last 20 yr. OBJECTIVES: Our objective was to determine frequencies of specific targeted adverse events (AEs) and additional AEs of interest in TS patients. Corresponding safety data in non-TS patients or normal populations were compared for selected AEs. METHODS: Patients may be enrolled at rhGH initiation and followed until discontinuation. Inve

Osteotomy does not improve early outcome after slipped capital femoral epiphysis.

We performed a retrospective, nonrandomized cohort study of unilateral, chronic, severe, stable slipped capital femoral epiphysis comparing five girls and five boys who underwent in-situ screw fixation alone with five girls and five boys who underwent in-situ screw fixation combined with staged flexion intertrochanteric femoral osteotomy to restore proximal femoral alignment. Functional outcome was measured by the Harris hip score, with 20% selected as a goal for improvement in functional outcome after corrective osteotomy. While flexion intertrochanteric femoral osteotomy improved hip range o

Controversies in slipped capital femoral epiphysis.

Slipped capital femoral epiphysis (SCFE) is a common adolescent hip disorder. This article reviews the major controversies in SCFE as of the year 2005. These are (1) treatment of the unstable SCFE, (2) the role of osteotomy in the treatment of SCFE, (3) prophylactic fixation of the contralateral hip in children presenting with unilateral SCFE, and (4) methods of fixation in the very young child with SCFE.

Percutaneous osteotomy for deformity correction in adolescents with severe slipped capital femoral epiphysis.

Various methods of performing proximal femoral realignment in adolescents with severe slipped capital femoral epiphysis exist. We report the technique and early results of a percutaneous, opening wedge subtrochanteric femoral osteotomy using an external fixator for correcting multiplanar deformities in such patients. Nine adolescents with severe slipped capital femoral epiphysis underwent a percutaneous osteotomy at an average age of 14.5 years. Mean operative blood loss was 61 ml, with 2 days of inpatient stay and 129 days of external fixation time. At an average follow-up of 23 months, hip f

Slipped capital femoral epiphysis. The mechanical function of the periosteum: new aspects and theory including bilaterality.

This particular ailment has many designations in the literature but none is quite adequate. Here we use the terminology slipped capital femoral epiphysis (SCFE). The anatomy of the proximal femur in all mammals reflects their growth and function. The main forces acting on the head are perpendicular to the growth plate. The thick tear-proof perichondrium-periosteum on the femoral neck is like a stocking attached to the epiphysis and the trochanter region. Growth in length causes a strong tensile stress in the periosteum, pressing the epiphysis against the metaphysis and thus stabilizing the vul

[Epiphyseolysis capitis femoris (ECF) in monozygotic twins].

AIM: The frequency of bilateral slipped capital femoral epiphysis (SCFE) is 5-80%. A set of boy twins developed left-sided SCFE within 2 years. We tried to find a relationship between SCFE and HLA typing. METHOD: We obtained the HLA typing and compared it to previously reported cases. RESULTS: By comparing 14 gene loci we made sure that they are identical twins. In our patients as well as in may of previously reported cases of sets of boy twins HLA phenotype A2 were found, while in the sets of girl twins HLA phenotype A11 and B12 were found. CONCLUSIONS: HLA phenotyping is needed in a large

Acetabular development in developmental dysplasia of the hip complicated by lateral growth disturbance of the capital femoral epiphysis.

BACKGROUND: Lateral growth disturbance of the capital femoral epiphysis is the most common type of physeal arrest complicating the treatment of developmental hip dysplasia. Although this type of physeal damage has been assumed to result in poor acetabular development, the natural history of dysplastic hips affected by this pattern of growth disturbance is still unclear. To investigate this issue, we evaluated acetabular development in a retrospective study of fifty-eight hips in forty-eight patients who had lateral physeal arrest after management of developmental hip dysplasia. METHODS: Of th

[Computer-assisted planning of corrective osteotomies in cases of epiphysiolysis capitis femoris].

Slipped capital femoral epiphysis (SCFE) leads to an unphysiologic function in adolescent hips. Evaluation of the slippage as well planning of corrective osteotomies is a major three-dimensional (3D) problem. Therefore, the current clinical evaluation, which is based on biplanar plain radiographs, cannot be satisfying. More is needed than simply measuring the femoral geometry to evaluate the impact of a slippage onto the physiologic hip function. We have developed a computer-based system for planning and evaluation of reorienting osteotomies in severe cases of SCFE. In our system, CT-based 3D

Orthopedic aspects of child abuse.

Fractures are one of the most common manifestations of child abuse. Pediatricians and other physicians caring for children must have a clear understanding of the injury patterns sustained by children in abusive situations and be able to recognize those injuries. A thorough history and physical examination and accurate radiographic interpretation are essential. Early identification of nonaccidental trauma is imperative because proper intervention has been shown to be effective.

[Course of bone tissue after bone marrow allograft in adolescents with sickle cell disease].

PURPOSE OF THE STUDY: Allogenic B.M.T. has been investigated as a curative treatment of anemia in S.C.D. Furthermore, correction of functional asplenia has been observed after B.M.T.; it remains to be determined if such a treatment may reserve other organ damage and particularly bone abnormalities and osteonecrosis frequently associated with S.C. hemoglobinopathies. MATERIAL AND METHODS: The aim of this retrospective review was to evaluate the radiologic and MRI results of 2 patients with avascular necrosis before the B.M.T. Roentgenographic diagnosis of osteonecrosis before B.M.T. included,

Growth disturbances of the proximal femur after pinning of juvenile slipped capital femoral epiphysis.

Premature closure of the capital femoral physis after pin fixation of slipped capital femoral epiphysis (SCFE) in the juvenile population (at least 1 year less than the established mean) can lead to growth disturbances of the proximal femur. A retrospective review of 21 patients (33 hips) who had a pinning of a juvenile SCFE was performed. Growth disturbances including greater trochanteric overgrowth, coxa vara, and coxa breva were noted in 64% of the hips. An 80% incidence of bilateral involvement was noted in the juvenile group. Consideration should be given to prophylactic pinning of the co

Transtrochanteric rotational osteotomy in the treatment of idiopathic and steroid-induced femoral head necrosis, Perthes' disease, slipped capital femoral epiphysis, and osteoarthritis of the hip. Indications and results.

Transtrochanteric rotational osteotomies were successful in 342 hips (274 patients) for the treatment of idiopathic and steroid-induced osteonecrosis of the femoral head, Perthes' disease, slipped capital femoral epiphysis, osteoarthritis, and other disorders. One hundred fifty-eight hips (113 patients) with idiopathic and steroid-induced necrosis were followed up for two to 11 years. In 121 of 158 hips, excellent results were obtained clinically and roentgenographically. In 42 Grade I or II hips, excellent results were obtained in every respect (success rate, 86%). Eighty-eight of 93 hips in

Southwick osteotomy for severe chronic slipped capital femoral epiphysis: results and complications.

During an eight-year period, twenty-four Southwick osteotomies were performed in twenty-one patients with severe chronic slipped capital femoral epiphysis. After follow-ups ranging from two to ten years (average, four and one-half years), correction of the deformity had been maintained in all but one patient, who has loss of fixation of the osteotomy. One patient had joint-space narrowing, avascular necrosis of the femoral head, and significant restriction of motion which was still present three years after operation. We concluded that this osteotomy gives good correction of the deformity but

Replacement of the femoral head by open operation in severe adolescent slipping of the upper femoral epiphysis.

The stages in adolescent slipping of the upper femoral epiphysis are classified in relation to treatment. The operation of open replacement of the displaced femoral head is described, and the results of a personal series of seventy-three such operations are presented. Open replacement is excellent treatment for severe chronic slipping so long as the growth plate is still open. The greater incidence of avascular necrosis in acute-on-chronic cases is probably due to damage to the blood supply of the head at the time of the acute slip or kinking of the vessels before replacement. Prolonged tracti

A comparative analysis of axial and appendicular skeletal maturity staging systems through assessment of longitudinal growth and curve modulation after VBT surgery.

PURPOSE: Appendicular skeleton markers are commonly used for maturity assessment for Adolescent Idiopathic Scoliosis (AIS) patients. Traditionally, Risser has been a standard skeletal maturity assessment method. More recently, Sanders classification (SSMS), as a more comprehensive system, became popular, especially in decision-making for Vertebral Body Tethering (VBT). Thumb-Ossification Composite Index (TOCI), using ossification of thumb epiphyses, has been claimed to more accurately stage patients around their peak height velocity. However, growth peaks may occur separately at lower limbs an…

Modified Dunn Procedure for Open Reduction of Chronic Slipped Capital Femoral Epiphysis.

BACKGROUND: Abnormal femoral head anatomy following moderate-to-severe slipped capital femoral epiphysis (SCFE) can lead to femoroacetabular impingement and premature osteoarthritis4-10. Surgical correction at the deformity site through capital reorientation has the potential to fully ameliorate this but has traditionally been associated with high rates of osteonecrosis11-15. The modified Dunn procedure has the potential to restore anatomy in hips with SCFE while protecting the blood supply to the femoral head. DESCRIPTION: A surgical dislocation of the hip is performed according to the techn

Osteopoikilosis and multiple exostoses caused by novel mutations in LEMD3 and EXT1 genes respectively--coincidence within one family.

BACKGROUND: Osteopoikilosis is a rare autosomal dominant genetic disorder, characterised by the occurrence of the hyperostotic spots preferentially localized in the epiphyses and metaphyses of the long bones, and in the carpal and tarsal bones 1. Heterozygous LEMD3 gene mutations were shown to be the primary cause of the disease 2. Association of the primarily asymptomatic osteopokilosis with connective tissue nevi of the skin is categorized as Buschke-Ollendorff syndrome (BOS) 3. Additionally, osteopoikilosis can coincide with melorheostosis (MRO), a more severe bone disease characterised by …
