Simulator Pre-Screening of Underprepared Drivers Prior to Licensing On-Road Examination: Clustering of Virtual Driving Test Time Series Data.

BACKGROUND: A large Midwestern state commissioned a virtual driving test (VDT) to assess driving skills preparedness before the on-road examination (ORE). Since July 2017, a pilot deployment of the VDT in state licensing centers (VDT pilot) has collected both VDT and ORE data from new license applicants with the aim of creating a scoring algorithm that could predict those who were underprepared. OBJECTIVE: Leveraging data collected from the VDT pilot, this study aimed to develop and conduct an initial evaluation of a novel machine learning (ML)-based classifier using limited domain knowledge

Hypotension Prediction Index Is Equally Effective in Predicting Intraoperative Hypotension during Noncardiac Surgery Compared to a Mean Arterial Pressure Threshold: A Prospective Observational Study.

BACKGROUND: The Hypotension Prediction Index is designed to predict intraoperative hypotension in a timely manner and is based on arterial waveform analysis using machine learning. It has recently been suggested that this algorithm is highly correlated with the mean arterial pressure itself. Therefore, the aim of this study was to compare the index with mean arterial pressure-based prediction methods, and it is hypothesized that their ability to predict hypotension is comparable. METHODS: In this observational study, the Hypotension Prediction Index was used in addition to routine intraoperat

Validation of Claims-Based Algorithms for Identifying Congenital Urinary Tract, Genital, Gastrointestinal, and Musculoskeletal Malformations.

AIM: Congenital malformations are important outcomes when evaluating medication safety in pregnancy. However, the accuracy of claims-based algorithms for identifying organ-specific malformations remains understudied. We validated algorithms for four malformation groups: urinary tract, genital, gastrointestinal, and musculoskeletal. MATERIALS AND METHODS: Using the Mass General Brigham (MGB, 2007-2020) and Stanford Medicine (2016-2023) databases, we identified infants with potential malformations of interest based on diagnosis and procedure codes within 90 days of birth. A total of 150 cases w

Garden classification of femoral neck fracture using deep-learning algorithm.

The Garden classification, based on X-ray interpretation and established over 50 years ago, remains the standard clinical classification system for femoral neck fractures (FNFs). Yet, this classification has a high interobserver variability of 70%. We sought to develop a deep-learning algorithm capable of accurately predicting FNF types, using only X-ray images, with performance comparable to that of computed tomography (CT). We retrospectively collected data from 1,588 patients who underwent X-ray and 3D-CT scans and were diagnosed with femoral neck fractures at Asan Medical Center. The input

The power of ECG in multimodal patient-specific seizure monitoring: Added value to an EEG-based detector using limited channels.

OBJECTIVE: Wearable seizure detection devices could provide more reliable seizure documentation outside the hospital compared to seizure self-reporting by patients, which is the current standard. Previously, during the SeizeIT1 project, we studied seizure detection based on behind-the-ear electroencephalography (EEG). However, the obtained sensitivities were too low for practical use, because not all seizures are associated with typical ictal EEG patterns. Therefore, in this paper, we aim to develop a multimodal automated seizure detection algorithm integrating behind-the-ear EEG and electroca

Ensemble learning algorithm based on multi-parameters for sleep staging.

The aim of this study is to propose a high-accuracy and high-efficiency sleep staging algorithm using single-channel electroencephalograms (EEGs). The process consists four parts: signal preprocessing, feature extraction, feature selection, and classification algorithms. In the preconditioning of EEG, wavelet function and IIR filter are used for noise reduction. In feature selection, 15 feature algorithms in time domain, time-frequency domain, and nonlinearity are selected to obtain 30 feature parameters. Feature selection is very important for eliminating irrelevant and redundant features. Fe

Deep Learning in Scaphoid Nonunion Treatment.

Background/Objectives: Scaphoid fractures are notorious for a high rate of nonunion, resulting in chronic pain and impaired wrist function. The decision for surgical intervention often involves extensive imaging and prolonged conservative management, leading to delays in definitive treatment. The effectiveness of such treatment remains a subject of ongoing clinical debate, with no universally accepted predictive tool for surgical success. The objective of this study was to train a deep learning algorithm to reliably identify cases of nonunion with a high probability of subsequent union followi

Penalized nonlinear mixed effects model to identify biomarkers that predict disease progression.

Precise modeling of disease progression in neurodegenerative disorders may enable early intervention before clinical manifestation of a disease, which is crucial since early intervention at the premanifest stage is expected to be more effective. Neuroimaging biomarkers are indicative of the underlying disease pathology and may be used to predict future disease occurrence at the premanifest stage. As observed in many pivotal studies, longitudinal measurements of clinical outcomes, such as motor or cognitive symptoms, often present nonlinear sigmoid shapes over time, where the inflection points

Precision diagnosis of GABRA1-associated encephalopathies and epilepsy: optimizing variants classification and molecular subregional effects.

BACKGROUND: GABRA1 variants are associated with a broad spectrum of epileptic phenotypes ranging from mild idiopathic generalized epilepsy to severe developmental and epileptic encephalopathy (DEE). To date, the majority of the identified GABRA1 variants are missense. Evaluating the pathogenicity of missense variants is a great challenge in genetics. This study aimed to explore reliable biological tools to optimize pathogenic classification of variants, thereby improving precision diagnosis of GABRA1-associated encephalopathies and epilepsy. METHODS: The dataset of disease-associated and cont

Artificial Intelligence Prediction Model of Occurrence of Cerebral Vasospasms Based on Machine Learning.

BACKGROUND: Symptomatic cerebral vasospasms are deleterious complication of the rupture of a cerebral aneurysm and potentially lethal. The existing scales used to classify the initial presentation of a subarachnoid hemorrhage (SAH) offer a blink of the outcome and the possibility of occurrence of symptomatic cerebral vasospasms. Altogether, neither are they sufficient to predict outcome or occurrence of events reliably nor do they offer a united front. This study tests the common grading scales and factors that otherwise affect the outcome, in an artificial intelligence (AI) based algorithm to
