A Preliminary Observation of Increased Glial Cell Line-Derived Neurotrophic Factor in Manic Switch due to Electroconvulsive Treatment in Depressive Patients.

OBJECTIVE: Neurotrophic factors are known to be involved in the pathogenesis of mood disorders. However, the precise neurobiology underlying relapse into depression or switch to mania under antidepressant treatment is not fully understood. Evidence suggests the role of neuroplasticity in these processes. METHOD: Brain-derived neurotrophic factor and glial cell line-derived neurotrophic factor (GDNF) serum levels were measured concomitantly during electroconvulsive treatment (ECT) in 30 depressive patients (25 patients with unipolar, 5 with bipolar depression), including those who relapsed int

Cell signaling, receptors, electrical effects and therapy in circadian rhythm.

Circadian rhythm has been the object of much attention. This review addresses the aspects of cell signaling, receptors, therapy and electrical effects in a multifaceted fashion. The pineal gland, which produces the important hormones melatonin and serotonin, exerts a prominent influence, in addition to the supraschiasmatic nucleus. Many aspects involve free radicals which have played a widespread role in biochemistry.

Response to ECT in bipolar I, bipolar II and unipolar depression.

OBJECTIVES: A significant body of evidence indicates the efficacy of electroconvulsive therapy (ECT) in unipolar depression but mixed results have been reported in bipolar depression. We explored difference of response to ECT in unipolar (UP), bipolar I (BP I) and bipolar II (BP II) depression, in a sample of patients resistant to pharmacological treatment. METHODS: One hundred and thirty depressive patients (17 with Major Depression (UP), 67 with bipolar disorder II (BP II) and 46 with bipolar disorder I (BP I) according to DSM-IV criteria) were included in the study and treated with bilater…

A 25-year longitudinal, comparison study of the outcome of depression.

BACKGROUND: There is still a relative paucity of information about the long-term course of depression. METHODS: Consecutive patients admitted to a teaching hospital psychiatry unit with symptoms of depression, previously assessed at 6 months and 2, 5 and 15 years after index admission, were reviewed at 25 years (N = 49, including eight informants of deceased probands, of an original 145 with major depression (DEPs)). Prospective psychiatric (N = 22) and retrospective surgical (N = 50) control groups assessed after 25 years were used for comparison. RESULTS: A further decade of follow-up conf

Efficacy of carbamazepine compared with other agents: a clinical practice survey.

BACKGROUND: To gain an impression of the experience with and efficacy of carbamazepine relative to other agents and relative to its use in t … cute and long-term treatment of bipolar illness. It was rated slightly less effective than lithium, electroconvulsive therapy, or neuroleptics, but more effective than several other agents. The results of the survey highlight many areas in need of further systematic investigation.

Mood-dependent learning. I. Endogenomorphic and psychoreactive depressions--influence of positive and negative feedback on expectancy shifts.

Influenced by Seligman's definition of 'learned helplessness' we studied the change of expectancy after positive and negative reinforcement before the onset and in the course of treatment of endogenomorphic and psychoreactive depressions. Before the onset of treatment patients do not exhibit any systematic change of expectancy after positive or negative reinforcement, in the course of treatment reaction to reinforcement is demonstrable, the type of treatment administered during the observation period causes differences in behavior.

Interventions for suicide and self-injury: A meta-analysis of randomized controlled trials across nearly 50 years of research.

Self-injurious thoughts and behaviors (SITBs) are major public health concerns impacting a wide range of individuals and communities. Despite major efforts to develop and refine treatments to reduce SITBs, the efficacy of SITB interventions remains unclear. To provide a comprehensive summary of SITB treatment efficacy, we conducted a meta-analysis of published randomized controlled trials (RCTs) that have attempted to reduce SITBs. A total of 591 published articles from 1,125 unique RCTs with 3,458 effect sizes from the past 50 years were included. The random-effects meta-analysis yielded surp

Peyronie's disease: review of nonsurgical treatment options.

The purpose of this article is to review the contemporary literature on nonsurgical therapies for Peyronie's disease (PD); focus on randomized, placebo-controlled trials; and review the latest guidelines for the management of PD from the International Consultation on Sexual Medicine. A combination of oral agents or intralesional injection with traction therapy may provide a synergy between the chemical effects of the drugs and the mechanical effects of traction. Until a reliable treatment emerges, some of the nonsurgical treatments discussed can be used to stabilize the scarring process and ma

Pediatric catatonia across diagnostic contexts: Presentation, treatment course, and factors associated with ECT use.

Objective Catatonia in young people is increasingly recognized, but pediatric cohorts remain limited. We characterized clinical presentation and treatment course and examined factors associated with electroconvulsive therapy (ECT) use and exploratory inflammatory correlates of psychomotor dimensions. Methods We retrospectively reviewed 57 consecutive patients aged <18 years treated at a tertiary referral center; one patient with a medical etiology was excluded from inferential analyses, leaving 56. DSM-5-TR signs were operationalized as hypokinetic, hyperkinetic, and parakinetic scores. Associ…

Peripheral immune cell markers as predictors of electroconvulsive treatment response.

BACKGROUND: Research indicates that higher levels of inflammation in patients with depression could be associated with a better response to electroconvulsive therapy (ECT). However, studies on the usefulness of immune markers as a potential biomarker for ECT response are scarce. AIM: This study sought to investigate whether peripheral immune cell markers are associated with ECT response in patients with depression. METHOD: Twenty-seven patients who received ECT treatment with a diagnosis of major depression in the Karadeniz Technical University psychiatry inpatient clinic were evaluated retros…

Catatonic Features That May Be Mistaken for Worsening Autism Show Treatment-Associated Improvement: Implications for Later Regression.

Catatonia in autistic individuals may resemble worsening autism yet represent a potentially treatable departure from baseline functioning. We compared item-level catatonic signs in autistic and non-autistic patients and evaluated treatment-associated change during electroconvulsive therapy (ECT). This single-center observational cohort included patients with catatonia who received ECT from May 2022 through April 2026. The 23-item Bush-Francis Catatonia Rating Scale (BFCRS) was assessed longitudinally. Pretreatment BFCRS data were available for 104 patients, including 41 autistic and 63 non-aut…

Clozapine-Memantine Therapy in Catatonia: Clinical Insights From a Case Report and Literature Review.

OBJECTIVES: To present a case of catatonia in a patient with schizophrenia successfully managed with clozapine and memantine in the absence of standard treatment options. A focused literature review was also conducted to contextualize the findings within the existing evidence. METHODS: A 26-year-old man with schizophrenia presented with classic catatonic symptoms, including mutism, posturing, and negativism. Although a lorazepam challenge confirmed benzodiazepine responsiveness, further use was limited by drug unavailability, and electroconvulsive therapy (ECT) was contraindicated due to a ce…
