A prospective comparison of computer-navigated and fluoroscopic-guided in situ fixation of slipped capital femoral epiphysis.

BACKGROUND: Slipped capital femoral epiphysis (SCFE) is usually treated with percutaneous in situ screw fixation to prevent further progression of deformity. The purpose of this investigation is to compare computer navigation (CN) techniques with traditional fluoroscopic (fluoro) techniques for in situ fixation of SCFE. METHODS: This study was an IRB-approved prospective study of 39 hip pinnings in 33 children. CN techniques were used in 22 cases and fluoro in 17. The CN and fluoro groups were statistically similar in terms of grade and acuity of the slip. Children were assigned to the groups

Occult distal femoral physeal injury with disruption of the perichondrium.

The authors describe a case of distal femoral physeal injury with disruption of the perichondrium in a 9-year-old girl after a sledding accident. The patient presented with knee pain, limited range of motion, and inability to bear weight. Initial radiographs were normal. A magnetic resonance imaging of the knee demonstrated abnormal signal and widening of the distal femoral physis with elevation of the posterior distal femoral periosteum. This case illustrates the main magnetic resonance imaging findings in an occult Salter Harris type I injury: increased physeal thickness and signal intensity

Prediction of contralateral slipped capital femoral epiphysis using the modified Oxford bone age score.

BACKGROUND: The purpose of the present study was to determine whether the modified Oxford bone score can be used as a predictor for the risk of developing contralateral slipped capital femoral epiphysis (SCFE) in children who present with a unilateral slip. METHODS: We identified 260 patients treated for unilateral SCFE between 1980 and 2002 and followed them up to skeletal maturity or until development of contralateral slip. Exclusion criteria included patients with endocrine or metabolic disorder, Down syndrome, and those with radiographs inadequate to determine the modified Oxford bone sco

The gait function of slipped capital femoral epiphysis in patients after growth arrest and its correlation with the clinical outcome.

PURPOSE: Slipped capital femoral epiphysis (SCFE) represents the preliminary stage of osteoarthritis. Reliable tools for outcome evaluation should be developed to prevent persisting defects. The functional outcome of SCFE-patients after growth arrest is analysed by instrumented 3D-gait analysis (GA). The results are matched to the clinical examination findings. METHODS: A total of 39 SCFE patients after growth arrest (18.8 years; BMI 26.5 kg/m²) with unilateral affection were included. The clinical results were classified according to Harris hip and clinical Heyman Herndon scores. 3D-GA-param

Radiation exposure during in-situ pinning of slipped capital femoral epiphysis hips: does the patient positioning matter?

Multiple radiographic images may be necessary during the standard procedure of in-situ pinning of slipped capital femoral epiphysis (SCFE) hips. This procedure can be performed with the patient positioned on a fracture table or a radiolucent table. Our study aims to look at any differences in the amount and duration of radiation exposure for in-situ pinning of SCFE performed using a traction table or a radiolucent table. Sixteen hips in thirteen patients who were pinned on radiolucent table were compared for the cumulative radiation exposure to 35 hips pinned on a fracture table in 33 patients

A radiological classification of lateral growth arrest of the proximal femoral physis after treatment for developmental dysplasia of the hip.

BACKGROUND: After treatment of developmental dysplasia of the hip, lateral growth arrest of the physis of the proximal femur resulting in disturbance of the growth of the capital femoral epiphysis is not uncommon. The changes are usually not apparent until approximately the age of 9 years. The residual deformity has a variable prognosis at skeletal maturity. The purpose of this study was to assess the long-term impact of these proximal femoral changes on the function of the hips. METHODS: We reviewed the natural history of 22 hips in 21 patients who had a lateral growth arrest of the physis o

Three-dimensional computerized tomographic analysis of the deformity of lateral growth disturbance of proximal femoral physis.

BACKGROUND: Lateral growth disturbance of the proximal femoral physis after treatment of developmental dysplasia of the hip, also known as type 2 avascular necrosis, has been described in a 2-dimensional (2D) plane with standard radiographs. It is not well described in the 3-dimensional plane using 3D computed tomography (CT) scan. The purpose of this study was to define the anatomy of the proximal femur after the lateral growth disturbance of the proximal femoral physis with 3D CT scans. METHODS: Ten patients (11 hips) with lateral growth disturbance of the proximal femoral physis after trea

Proximal femoral physis shear in slipped capital femoral epiphysis--a finite element study.

The following finite element study was conducted to determine whether increased body weight, femoral retroversion, and varus hip loading could sufficiently raise physeal shear strains and stresses above the yield point and predispose an adolescent hip to a slip. A computer tomography scan of a 13-year-old child with slipped capital femoral epiphysis was used to generate a solid model of the proximal femur and physis. The model was parameterized using 3-dimensional software to generate three difference angles of femoral neck version-neutral, 15 degrees retroversion, and 15 degrees anteversion.

[MRI in the diagnosis and treatment of Perthes disease and epiphysiolysis of the head of the femur].

In a 10-year prospective study, we were able to examine 93 children with 109 hips suffering from Perthes' disease and 6 hips with Meyer's dysplasia epiphysealis by magnetic resonance imaging (MRI) follow-up studies. Routine imaging was done by coronary T1- and T2-enhanced sequences and sagittal T1-enhanced sequences. In total of 400 MR images, the following criteria were evaluated: staging of the disease according to MRI signal changes, extent of the necrosis, prognostic criteria, differential diagnosis of Meyer's dysplasia epiphysealis, and proposals for therapy. Typical signal changes in MRI

Safety issues in children and adolescents during growth hormone therapy--a review.

The action of growth hormone (GH) via its receptor involves many organ systems and metabolic pathways. These diverse actions are reviewed in this paper in the context that they may represent unwanted side-effects of GH therapy for growth promotion. The monitoring of GH therapy in large multicentre international databases has demonstrated a low frequency of adverse events. Tumour recurrence or new malignancy are not increased. Headaches, especially in the first few months of therapy, require close evaluation as benign intracranial hypertension is found infrequently, especially in children with

[Differential diagnosis of pediatric hip pain in MR tomography].

The efficiency of spin echo tomography in the differential diagnosis of hip joint pain in children in comparison with other imaging methods was tested in 115 spin echo tomography examinations of 68 patients. By means of the different modes available it was possible to determine joint effusion, necrosed areas and the formation of abscesses with great accuracy. In the case of Legg-Calve-Perthes disease the MRT is especially in the early diagnosis superior to the other Methods, already in the early stage a classification in the four Catterall groups was possible. Furthermore the "head at risk" si

Slipped capital femoral epiphysis: an analysis of 80 patients as to pin placement and number.

A retrospective study of 80 patients with chronic slipped capital femoral epiphysis was performed, analyzing the clinical results with regard to pin placement and pin number. Follow-up averaged greater than 2 years, with 86% of the 80 patients obtaining a satisfactory clinical outcome. Serious complications occurred in 10 patients. The severity of complications increased as the number of pins used increased (p less than 0.07). A varus pin position resulting in a more inferior pin placement in the proximal femoral epiphysis was found to be associated with the fewest complications. When the pin

Hip problems in children. Aids to early recognition.

Awareness of the age at which various pediatric hip disorders present and familiarity with their characteristics greatly aid in early detection. A properly performed physical examination is still the mainstay of early diagnosis. The earlier a hip disorder is discovered and treated, the less the chance of lasting harm to the child.

The treatment of severe fractures of the lower end of the femur with Judet's screw-plate.

Severe fractures of the lower end of the femur have increased in frequency as road accidents have become more common. Good anatomical reconstruction is essential in order to conserve useful joint function, and this can only be achieved by internal fixation. Judet's screw-plate has been used with success in these cases and is presented in its technical and clinical aspects. The quality of the results obtained is documented in an analysis of 22 operations.
