Extracorporeal shockwave therapy (ESWT) ameliorates healing of tibial fracture non-union unresponsive to conventional therapy.

Tibial non-unions are common cause of demanding revision surgeries and are associated with a significant impact on patients' quality of life and health care costs. Extracorporeal shockwave therapy (ESWT) has been shown to improve osseous healing in vitro and in vivo. The main objective of present study was to evaluate the efficacy of ESWT in healing of tibial non-unions unresponsive to previous surgical and non-surgical measures. A retrospective multivariant analysis of a prospective open, single-centre, clinical trial of tibia non-union was conducted. 56 patients with 58 eligible fractures wh

An Innovative Approach to the Repair of Distal Tibia Nonunion Using a Retrograde Buried Tibio-Talar-Calcaneal Nail: A Case Report.

We report a case using retrograde tibial nailing as treatment of nonunion of a distal tibial osteotomy, which was performed as part of a complex reconstruction of distal tibial malunion with ankle arthritis. Although retrograde nailing has classically been used for tibial-talar-calcaneal arthrodesis, this method spares the subtalar joint. Preservation of some hindfoot motion by subtalar mobility allows for a decrease in the loss of function typically seen with tibial-talar-calcaneal arthrodesis.

Influence of Immobilization Time on Functional Outcome in Radial Neck Fractures in Children.

Background Radial neck fractures represent 20 to 30% of elbow fractures in children. Incorrect treatment can lead to significant permanent functional impairment. Posttraumatic avascular necrosis may cause a deformity of the radial head and neck. Deformation of the radial head and neck can be more severe after open rather than closed reduction or orthopedic treatment without reduction. The aim of our study was to analyze the influence of immobilization time on functional outcome. Patients and Methods Retrospective, descriptive study of all children who had been treated for a radial neck fractur

Results of non-union of humerus treated with retrograde humeral nail.

Treating non-union of humerus fracture is a surgical challenge with variable outcome. We report series of 51 adult patients of aseptic non-union of humerus from 1998 to 2010 treated retrograde humeral nail. The mean age of patient was 54 years with 33 females and 18 males. The mean duration of non-union was 8 months. In 48 out of 51 cases (94 %), union was achieved at mean duration of 10.1 months. Thirty-two out of 51 patients needed bone grafting. Three patients had post-operative radial nerve neuropraxia which fully recovered. At last visit, mean Constant score for shoulder was 83 and mean M

Minimal invasive treatment of proximal femur fractures with the long gamma nail: indication, technique, results.

BACKGROUND: The surgical treatment of complex unstable proximal femur fractures from the trochanteric region to the middle shaft area is difficult and often highly invasive, especially in older patients with osteoporotic bones. METHODS: In 1993, we began to treat all unstable proximal femur fractures from the trochanteric region to the middle third of the shaft with the long gamma nail (LGN) and allowed the patients immediate full weightbearing after surgery. Perioperative, postoperative, and follow-up data were analyzed. RESULTS: During a 3-year period, we treated 37 patients with unstable

Girl predominance in trampoline-related forearm shaft fractures and their increasing incidence since 2000.

BACKGROUND: There are reports of increasing incidence of forearm shaft fractures in children. Their treatment has been preferably nonoperative but surgical fixation has gained popularity due to elastic stable intramedullary nailing. We aimed to study whether the incidence of pediatric both-bone forearm shaft fractures and their operative care have changed since year 2000. Trampoline injuries, in particular, and their treatment, re-displacement and short-term outcomes were the secondary outcomes of the study. METHODS: A population-based study in the geographic catchment area of Oulu University

Risk Factors for Nonunion After Femoral Rotational Osteotomy for Idiopathic Anteversion and Retroversion in Adolescents.

BACKGROUND: Lower extremity rotational abnormalities can cause difficulty with ambulation, patellofemoral pain and instability, hip and ankle pain, as well as differences in self-image in children and adolescents. Rotational osteotomies of the femur to correct these torsional differences have been shown to improve function, pain, and self-image. There are no studies to date evaluating risk factors for nonunion after femoral rotational osteotomies in children and adolescents. METHODS: Patients 10 to 18 years old treated with femoral rotational osteotomy fixed with intramedullary nailing for id

Complications and risk factors of intramedullary bone lengthening nails: a retrospective multicenter cohort study of 314 FITBONE and PRECICE nails.

BACKGROUND AND PURPOSE: Intramedullary bone-lengthening nails have become increasingly popular. The 2 most used and successful nails are the FITBONE and the PRECICE nails. Uniform reporting is lacking on complications of intramedullary bone-lengthening nails. The purpose was therefore to assess and categorize the complications of lower limb bone-lengthening nails and investigate risk factors. PATIENTS AND METHODS: We performed a retrospective review of patients operated on with intramedullary lengthening nails at 2 hospitals. We included only lower limb lengthening with FITBONE and PRECICE na

Minimal Out-Toeing and Good Hip Scores of Severe SCFE Patients Treated With Modified Dunn Procedure and Contralateral Prophylactic Pinning at Minimal 5-year Follow up.

BACKGROUND: Slipped capital femoral epiphyses (SCFE) is associated with out-toeing of the foot and external rotation gait. But it is unknown if SCFE patients treated with the modified Dunn procedure have out-toeing at follow up.Therefore, we used instrumented gait analysis and questioned (1) do severe SCFE patients treated with a modified Dunn procedure have symmetrical foot progression angle (FPA) compared with contralateral side and compared with asymptomatic volunteers (2) what is the prevalence of out-toeing gait and what are the outcome socres at follow up. METHODS: Gait analysis of 22 p

Lateral insertion points in antegrade femoral nailing and their influence on femoral bone strains.

OBJECTIVES: Insertion of rigid uniplane bent femoral nails through the piriform fossa has been reported to cause neurovascular complications. New nails were designed for more lateral entry points. However, these may be associated with a higher risk of iatrogenic fractures. This study investigated if two differently bent nails with more lateral entry points induce higher cortical bone strains than a uniplane bent nail introduced through the piriform fossa. METHODS: Three groups of 8 cadaveric femurs were instrumented using the following nail systems and entry points: Cannulated Femoral Nail, p

Enhancement of Local Osseointegration and Implant Stability of Titanium Implant in Osteoporotic Rats by Biomimetic Multilayered Structures Containing Catalpol.

This study examined the surface modification of titanium (Ti) implants to enhance early-stage osseointegration, which reduced the failure rate of internal fixation in osteoporotic fractures that inherently decrease in bone mass and strength. We employed a layer-by-layer electroassembly technique to deposit catalpol-containing hyaluronic acid/chitosan multilayers onto the surface of Ti implants. To evaluate the in vitro osteoinductive effects of catalpol-coated Ti implants, the robust osteoblast differentiation capacity of the murine preosteoblast cell line, MC3T3-E1, was employed. Furthermore,

Personalized surgery approach in severe form of osteogenesis imperfecta type III: point of view.

Osteogenesis imperfecta (OI) is a genetic disorder characterized by fragile bones. It is our aim to illustrate variability in clinical presentation of severe form of OI. As an example of personalized surgery approach we present an 11-year-old girl with OI type III. Prior to referral to our hospital, she was treated with 18 cycles of bisphosphonates as well as with several different surgical procedures. Due to no improvement in her mobility status she underwent two additional surgeries at our hospital with a 5-month interval between them. Prior to the surgery, molecular genetic analysis was per

[Spectrum of indications for intramedullary or external fixators for axis correction and limb lengthening].

Corrective osteotomy is a well established operative option for correction of predominantly congenital skeletal deformities on the growing skeleton. The indications for the use of a suitable corrective instrument result partly from the location, severity of the deformity and the presence of concomitant leg length discrepancies. External fixators are predominantly used for correction of combined deformities. For isolated leg length differences limb lengthening nails can be implanted but only after growth plates have become closed; however, only a few combined deformities can be addressed in thi

The trochanteric nail versus the sliding hip screw for intertrochanteric hip fractures: a review of 93 cases.

BACKGROUND: The trochanteric nail, a redesigned short gamma nail, (Howmedica, Rutherford, NJ) was introduced in the United States during 1998 for the treatment of intertrochanteric fractures. METHODS: We retrospectively reviewed 93 patients who were treated for an intertrochanteric fracture with either a sliding hip screw (SHS) or the short trochanteric nail (TN). RESULTS: In all, 94% of the patients in the sliding hip screw group healed without complication. There was one case of femoral head necrosis, one lag screw cutout, and one hardware removal for pain. Eighty-nine percent of the patie
