Pain threshold for low energy intracardiac cardioversion of atrial fibrillation with low or no sedation.

UNLABELLED: Recent studies have shown that internal cardioversion of atrial fibrillation is safe and effective. In this randomized prospective study, we have tried to evaluate the influence of different waveforms on the perception of pain during internal cardioversion in patients with chronic atrial fibrillation. METHODS: Internal cardioversion was performed with minimal or no sedation in 31 consecutive patients. R wave triggered, biphasic shocks of 6 ms/6 ms or 3 ms/3 ms duration (randomly selected) and approximately 65% tilt were used starting with a 50-V test shock. The shock intensity was

Early source control with non-intubated uniportal video-assisted thoracoscopic surgery for acute thoracic empyema: a retrospective pilot cohort study.

BACKGROUND: Acute thoracic empyema often develops in older and frail patients, in whom treatment selection can be challenging. This retrospective pilot cohort study evaluated the clinical outcomes of non-intubated uniportal video-assisted thoracoscopic surgery (NI-uVATS) as a potential early source-control strategy for selected patients with acute thoracic empyema. METHODS: We retrospectively reviewed 41 consecutive patients with acute thoracic empyema who underwent NI-uVATS between July 2019 and June 2025. NI-uVATS was performed as the initial procedure in 32 patients (initial group) and aft

Rescue baroreflex activation therapy after Stanford B aortic dissection due to therapy-refractory hypertension.

Clinical trials have demonstrated significant and durable reduction in arterial pressure from baroreflex activation therapy (BAT) in patients with resistant arterial hypertension. There is a lack of data, however, concerning the use of BAT in a rescue approach during therapy-refractory hypertensive crisis resulting in life-threatening end-organ damage. Here, we describe the first case in which BAT was applied as a rescue procedure in an intensive care setting after ineffective maximum medical treatment. A 34-year-old male patient presented with Stanford B aortic dissection and hypertensive cri

GABAB receptor modulation during drug-memory reconsolidation: current evidence and translational priorities for relapse prevention.

Relapse after abstinence is sustained in part by drug-associated memories reactivated by cues, contexts, actions, and interoceptive states. Reconsolidation-based interventions seek to modify a retrieved memory during transient lability. GABAB receptors suppress transmitter release, hyperpolarize neurons, and inhibit adenylyl-cyclase-cAMP-protein-kinase-A signaling, providing biological plausibility for temporally aligned modulation. This structured narrative review organizes the literature using a non-comparative evidence-domain framework. Direct retrieval-window evidence remains limited to tw

Dexmedetomidine Versus Clonidine as an Adjuvant to Lidocaine Spinal Anesthesia in an Ovine Experimental Model.

Sheep are routinely used as orthopedic models due to their similarities to human joints. Spinal anesthesia provides adequate analgesia for these procedures, and its duration can be enhanced with adjuvant drugs. Clonidine is commonly used in human spinal anesthesia, while dexmedetomidine is a newer and more selective α-2 agonist. This study compared the duration and analgesic effect of these two drugs as adjuvants in spinal anesthesia. Thirty-nine sheep undergoing experimental pelvic limb cartilage damage surgery were enrolled. Animals were sedated with diazepam (0.4 mg kg-1) and buprenorphine

Point-of-care ultrasound for the evaluation of dental abscesses in the pediatric emergency department.

BACKGROUND: Facial cellulitis is a common presentation at pediatric emergency departments. Prompt diagnosis of dentoalveolar abscesses is crucial for initiating appropriate treatment and minimizing morbidity. While imaging modalities such as computed tomography (CT) and a panoramic X-ray are available, they are not often used for pediatric patients given the known risks of exposure to radiation and the need for sedation. METHODS: We conducted a prospective diagnostic accuracy study at a tertiary pediatric emergency department to evaluate point-of-care ultrasound (POCUS, index test) for diagno

Neurological monitoring in liver transplantation: innovative approaches and perioperative strategies for managing brain complications.

PURPOSE OF REVIEW: Neurologic disorders in acute and chronic liver disease present a significant challenge in the perioperative management of patients undergoing liver transplantation. Neurologic examination is often limited in comatose or sedated patients, so instrumental monitoring of brain function is essential for prompt diagnosis and management of conditions that could cause brain injury. The purpose of this review is to summarize recent findings and provide an outlook for future developments. RECENT FINDINGS: Encephalopathic patients undergoing liver transplantation are at risk of devel

Visual Evoked Potential Monitoring in Long-term Anesthesia Maintenance and Challenging Environments for Cerebral Arteriovenous Malformation Treatment: A Technical Case Report.

The monitoring of intraoperative optic nerve protection using visual evoked potential has been increasingly used in neurosurgical procedures. Although visual evoked potential is a valuable tool, its application is often hindered by anesthetic limitations and challenges related to measurement and recording environments. This case study indicates the successful use of visual evoked potential monitoring during long-term anesthesia maintenance and in artifact-prone environments for the treatment of a ruptured cerebral arteriovenous malformation. We report the case of a woman in her 30s who underwe

Limited National Potential for Reducing Anesthesia Clinician Staffing for Cataract Surgery: A Historical Cohort Study Using a Large United States Anesthesia Registry.

Background Cataract surgery, the most frequently performed surgical procedure worldwide, is increasing, with many patients receiving intravenous sedation by anesthesia clinicians. Given anesthesia clinician shortages and ambulatory surgery facility scheduling constraints, some ophthalmologists are moving to office-based care without anesthesia services. We examined the potential impact of such a shift in care on reducing anesthesia clinician staffing. Methods We analyzed 622,953 cataract surgeries performed at 672 facilities between 2022 and 2023, using data from the American Society of Anesth

Use of herbal medication in the perioperative period: Potential adverse drug interactions.

Use of herbal medications and supplements has experienced immense growth over the last two decades, with retail sales in the USA exceeding $13 billion in 2021. Since the Dietary Supplement Health and Education Act (DSHEA) of 1994 reduced FDA oversight, these products have become less regulated. Data from 2012 shows 18% of U.S. adults used non-vitamin, non-mineral natural products. Prevalence varies regionally, with higher use in Western states. Among preoperative patients, the most commonly used herbal medications included garlic, ginseng, ginkgo, St. John's wort, and echinacea. However, 50-70
