Multifunctional bioengineered scaffolds and adjunct therapeutic strategies for diabetic foot ulcers.

Diabetic foot ulcers (DFUs) are chronic, non-healing wounds that affect up to 34% of diabetic patients. DFUs are complicated by infection in nearly 60% of cases and frequently progress to amputation. DFU pathology is characterized by a persistent inflammatory state, impaired angiogenesis, and infection. This creates a complex microenvironment refractory to standard care, with fewer than 20% of DFUs healing within 8 weeks. In this review article, normal and pathophysiological processes of wound healing, current clinical management strategies, and adjunct therapeutics in the clinical pipeline ar

Lancing: quo vadis?

Today, lancing fingertips or alternative sites for obtaining a blood sample for self-monitoring of blood glucose (SMBG) is a standard procedure for most patients with diabetes. The need for frequent lancing and associated discomfort and pain can be seen as a key hurdle for patients to comply with SMBG regimens. This article provides an overview of the status quo and future of lancing, focusing on key areas for future developments driven by customer and market needs. We also review technical issues and provide a background for possible improvements. The act of puncturing the skin with a lancet

Liability and Standard of Care in AI-Driven Psychiatric Practice: European Viewpoint.

AI is increasingly incorporated into psychiatric triage, risk prediction, passive monitoring, clinical documentation, and patient-facing conversational systems. These applications may improve access, continuity, efficiency, and pattern recognition, but they also redistribute epistemic authority and complicate responsibility when harm occurs. European regulation is developed in relation to market access, data governance, risk management, and product safety, yet remains fragmented regarding civil liability, organizational negligence, and the psychiatric standard of care. This Viewpoint examines

Next-generation hybrid bioanalytical platforms and clinical integration of TDM technologies for precision monitoring to optimize last-resort antibiotic therapy.

BACKGROUND: The global rise of multidrug-resistant (MDR) infections has intensified the clinical reliance on last-resort antibiotics such as vancomycin, linezolid, and tigecycline, particularly in critically ill patients. These agents possess narrow therapeutic windows, complex pharmacokinetics, and substantial inter-patient variability, making therapeutic drug monitoring (TDM) a cornerstone of individualized therapy. Traditional TDM methodologies though accurate are often limited by centralized processing, slow turnaround times, and cost constraints, which hinder real-time clinical decision-m

Implementing patient reported outcomes in cancer care: Lessons and strategies from a large UK Cancer Centre.

BACKGROUND: Patient-reported outcome measures (PROMs) are valuable for advancing patient-centred cancer care by capturing patients' views on their health, quality of life, and symptoms. However, routine PROM collection remains difficult to implement in clinical settings. This study evaluated a PROM pilot in the prostate cancer radiotherapy setting at Guy's Cancer Centre, identifying multi-level barriers and facilitators to routine implementation. METHODS: A mixed-methods design was used, comprising qualitative interviews with healthcare professionals and a quantitative questionnaire for patie

A qualitative study of stakeholder views on the effects of provider payment on cooperation, quality of care and cost-containment in integrated stroke care.

BACKGROUND: Stroke services are a form of integrated care which have been introduced in many countries, including the Netherlands, to improve health outcomes and processes of care by connecting the acute, rehabilitative, and chronic phases of stroke care. Limited research exists on the effects of payment systems on the functioning of integrated care services from the perspectives of those involved in providing, planning and contracting the care. This qualitative study identified stakeholder views on i) challenges in integrated stroke care associated with fee-for-service systems; ii) other poss

Experiences and support needs of patients receiving home mechanical ventilation and their caregivers: a qualitative meta-synthesis.

BACKGROUND: Home mechanical ventilation (HMV) is an important form of support for the out-of-hospital management of patients with chronic respiratory failure and long-term ventilatory dependence. It plays an important role in prolonging life, relieving symptoms, and supporting home living. As the site of treatment extends from the hospital to the home, patients and caregivers must jointly face challenges related to treatment decision-making, technological adaptation, expansion of caregiving responsibilities, and insufficient support systems. Existing studies have often focused on a single dise

Biomaterials for environmental control of AMR dissemination.

Antimicrobial resistance (AMR) threatens human, animal, agricultural, and environmental health, driven by horizontal and vertical gene transfer. Conventional wastewater treatment removes 63-94% of antibiotic resistance genes (ARGs) but fails to eliminate ARGs and antibiotic-resistant bacteria, while generating toxic sludge. This review evaluates advanced biomaterials, including silver and gold nanoparticles, reactive oxygen species, magnetic nanocomposites, targeted disruption of metabolic pathways, carbon-based sorbents, biopolymers, metal-organic frameworks, functionalized enzymes/peptides,

Triboelectric E-Skin Patch Decoding Silent Speech for Advanced Human-Machine Interaction and Augmented Virtual Communication.

Silent speech decoding (lip reading) represents an innovative hands-free communication method for individuals with laryngeal and lingual impairments, addressing critical daily communication challenges. Herein, a novel silent speech decoding system is proposed, which employs a self-powered, low-cost, and conformable triboelectric-based E‑skin patch and a well-trained multi-layer convolutional neural network (CNN) model. We systematically characterize the structural principles and electrical performance of the E-skin patch. A comparative analysis of labial kinematics during vocalized and silent

Multiparadigm Benchmark of Molecular Docking: From Physics to Co-Folding and Hybrid Models.

Generalizability of molecular docking predictions across novel protein targets and distinct small-molecule chemotypes is key to the successful application of deep learning co-folding approaches in drug discovery, as highlighted by recent benchmarks, including Runs-n-Poses [Škrinjar, P.; et al.Nat. Struct. Mol. Biol.2026, 33, 782-794.]. To compare generalizability across physics-based, hybrid rescoring models, and co-folding approaches, we systematically benchmarked several molecular docking tools representing these paradigms. Our results support previous observations that co-folding approaches

An Updated Review of the Opportunities and Challenges in Insects as Food and Feed.

The global demand for sustainable protein sources is increasing in response to population growth, climate change and food insecurity. Edible insects are a promising alternative, offering nutritional, environmental and socio-economic advantages. Currently consumed by approximately two billion people worldwide, insects are rich in protein, essential amino acids, unsaturated fatty acids, vitamins and minerals, usually making them nutritionally comparable to or potentially superior to conventional protein sources. Their farming systems require minimal land, water and feed inputs while generating s

[Acute Stroke Care Systems:Pre-Hospital and In-Hospital Management].

Acute ischemic stroke care has evolved substantially since mechanical thrombectomy was established as an effective reperfusion therapy. As clinical outcomes are strongly dependent on the time to reperfusion, the optimization of pre-hospital and in-hospital stroke care systems has become increasingly important. In this review, the key components of an effective acute stroke care pathway are summarized, with a focus on pre-hospital triage using large-vessel occlusion scales, regional stroke networks, and institutional workflow optimization. Effective pre-hospital management includes standardized

Wafer-scale 2D MoS₂ transistors with sub-5 nm channel length and subthreshold performance beyond the silicon limit.

The downscaling of silicon field-effect transistors is approaching fundamental limits, with the international roadmap for devices and systems (IRDS) projecting a 12 nm channel length plateau by 2037. Two-dimensional (2D) semiconductors have emerged as promising candidates for extended transistor scaling. However, no experimental demonstration has thus far surpassed the fundamental switching limits of advanced silicon. Here, we report monolayer MoS₂ transistors with physical channel lengths below 5 nm, achieving a subthreshold swing of 88 mV/dec and an on/off ratio above 106. These results surp

Microalgae-based removal and biodegradation of antibiotics: mechanisms, synergistic consortia, and engineering prospects.

Antibiotic pollution in aquatic environments drives the spread of resistance genes and threatens ecosystem health. Microalgae offer a dual-function platform that both senses antibiotic stress and actively removes these contaminants. This review synthesizes current knowledge on microalgae-based antibiotic removal and biotransformation, emphasizing the integration of biosorption, enzyme-mediated transformation-primarily via cytochrome P450 and nitroreductase-and co‑metabolic processes. We highlight the phycosphere as a dynamic interface where symbiotic bacteria enhance algal survival and polluta

Design principles for tuning selectivity in CO2 reduction by Fe and Co polypyridine complexes.

The development of polypyridine complexes as efficient and selective catalysts for the CO2 reduction reaction (CO2RR) remains a central challenge in molecular catalysis for small-molecule activation. Although considerable efforts have been devoted to identifying active catalysts and optimizing their performance under both electrochemical and light-driven conditions, the fundamental principles governing product selectivity have received comparatively less attention. In this review, we highlight recent advances in the development of iron and cobalt polypyridine complexes, with particular emphasi

Beyond Moisture Content: Time-Domain NMR for Quantifying Molecular Mobility, Water Accessibility and Food Function.

Moisture content is among the most routinely measured properties of foods, yet water quantity alone does not determine processing behavior, stability, texture, fluid release or sensory function. In heterogeneous food matrices, water occupies environments differing in molecular restriction, compartmentalization, exchange, connectivity and translational mobility. Time-domain nuclear magnetic resonance (TD-NMR) provides a rapid and non-destructive means of interrogating these properties through proton signal intensity, longitudinal and transverse relaxation, relaxation-time distributions, self-di

Recent advancements on innovative technologies and applications for non-destructive monitoring of tomato quality.

Tomato quality assessment involves multiple external and internal attributes. Conventional reference methods remain important for accurate characterization but are often destructive, time-consuming, or unsuitable for repeated and online measurements. This review critically examines recent advances in nondestructive sensing technologies for tomato quality evaluation and organizes them according to sensing mechanism. Electromagnetic and optical techniques are assessed for their ability to characterize surface, compositional, spectral, and physiological information, while mechanical and tactile s

Strategies for modelling the intestinal mucus interface: Focus on drug transport and host-microbiota interactions.

The intestinal mucus barrier is a complex, dynamic hydrogel that plays a central role in host-microbiota interactions, nutrient transport, and protection against pathogens. Understanding its physicochemical properties has become essential for developing predictive in vitro and in silico models for drug delivery, microbiome research, and disease modelling. Over the past decade, advances in polymer chemistry, glycoscience, and microfluidics have enabled the development of increasingly sophisticated mucus models that mimic structural, rheological, and biochemical features of native intestinal muc

Highly multiplexed mammalian metabolic engineering with a shotgun approach.

Mammalian metabolic engineering advances basic biology, bioproduction and cell therapy. However, as pathway complexity increases, so does the size of the combinatorial design space and required DNA constructs, rendering unbiased screens intractable. Here, we developed shotgun genetic engineering (SGE), which exploits the ease of delivering many barcoded small constructs-rather than a single large one-into mammalian cells. Each cell serves as an independent experiment, carrying a synthetic pathway that explores gene content, stoichiometry and organellar localization. Functional pathways are ide
