[Blood pressure control in hypertensive patients with stable coronary heart disease].

OBJECTIVE: To evaluate systolic blood pressure (SBP) control in hypertensive patients with a stable coronary heart disease (CHD) in general practice in France. METHODS: A survey was conducted in a sample of 206 general practitionners (GP) representative of the French medical population, in 2003 [LHYCORNE survey]. Each GP had to include 3 hypertensive patients, >18 years old, BP > or = 140/90 mmHg and/or treated for hypertension, and with evidence of CHD documented by myocardial infarction (MI) or angina pectoris (AP) [diagnosis previously established by a cardiologist]. Three office BP measur

Neonatal isolated critical aortic valve stenosis: balloon valvuloplasty or surgical valvotomy.

BACKGROUND: Open surgical valvotomy and transcatheter balloon valvuloplasty are recognised treatments for neonatal critical aortic stenosis. METHODS: A retrospective analysis was undertaken of all newborns with critical aortic valve stenosis between 1990 and 2000 presenting to a tertiary centre and who required intervention. The initial catheter and surgical intervention was generally based on the preference of the attending cardiologist and the anatomy of the aortic valve and in consultation with the cardiothoracic surgeon. The two groups were therefore not strictly comparable. Twelve were s

[Cardiac event recorder yields more diagnoses than 24-hour Holter monitoring in patients with palpitations].

BACKGROUND: Palpitations are a common symptom that sometimes results from a substantial cardiac arrhythmia. A 24-hour Holter monitoring is usually used, but the yield of this instrument is low in patients whose symptoms occur infrequently. The aim of this study was to compare the diagnostic yield and the cost-effectiveness of transtelephonic event recorder (TER) with those of Holter monitoring in patients with intermittent palpitations. METHODS: Three hundred and ten patients with intermittent palpitations were allocated to the study and randomly assigned to receive a TER or 24-hour Holter mo

Neurological or cardiological assessment of transient altered consciousness?

Cardiac arrhythmias are a recognized cause of transient alteration of consciousness and unless suspected, a cardiac aetiology for apparent neurological symptoms may be overlooked. We retrospectively assessed the variation in the investigations of 122 patients who were referred to our hospital over a twelve month period for assessment of transient alteration of consciousness and we attempted to identify any factors which might have influenced the clinical outcome for these individuals. Eighteen patients were initially referred for cardiological opinion and 104 for neurological assessment. Sixty

[The mobile coronary unit of Florence: an evaluation of 10 years of prehospital cardiac care].

In order to reduce the high mortality from ischemic heart disease, a mobile coronary care unit manned by a cardiologist has been operating in Florence since 1979. From 1980 to 1989 there were 13,029 interventions, 8,718 (66.9%) of which were cardiac emergencies. 1,718 (19.7%) patients showed acute myocardial infarction while in 2,274 angina was diagnosed (26.1%). Acute pulmonary edema and paroxysmal supraventricular tachyarrhythmias accounted, respectively, for 6.2% and 18.3% of total cardiac emergencies; moreover there were 753 (5.8%) attempted resuscitations in sudden cardiorespiratory arres

Translation and cultural adaptation of the Minnesota Living with Heart Failure Questionnaire for use in Pakistan.

Several tools measure the Health-related quality of life (HRQOL) of people with chronic illnesses but evidence on their cross-cultural adaptation is limited. This study performed the cross-cultural adaptation of the Minnesota Living with Heart Failure Questionnaire (MLHFQ) for use with Urdu speakers in Pakistan, while documenting the adaptation process in detail. This methodological study employed a structured cross-cultural adaptation and linguistic validation design. We incorporated a Delphi Pannel to the MAPI Research Trusts' 5-step linguistic validation process. Two forward translators ind

Foundation models enable wearable signal screening for cardiovascular disease among people living with HIV.

BACKGROUND: Cardiovascular disease screening faces significant challenges in resource-limited settings, where infrastructure and computational constraints preclude advanced assessment. These constraints are particularly acute for people living with human immunodeficiency virus (HIV), who experience elevated cardiovascular risk yet often receive care in clinics without specialist diagnostic capacity. Pretrained physiological foundation models offer potential for low-cost screening using wearable sensors, though their applicability in resource-constrained settings remains unclear. METHODS: We e

Endoscopic Mitral Surgery in Noonan Syndrome-Case Report and Considerations.

Background: Totally endoscopic techniques have become increasingly popular in cardiac surgery, with minimally invasive mitral valve repair emerging as an effective alternative to median sternotomy. This approach could be particularly advantageous for patients with Noonan syndrome, who often present with structural thoracic anomalies and other comorbidities like bleeding disorders. Endoscopic mitral valve surgery is rapidly establishing itself as the new standard of care for mitral valve operations, demonstrating both safety and efficacy. Noonan syndrome is an autosomal-dominant multisystem dis

PATCH (Preferred Attachment Strategy for Optimal Electrocardiograms)-1 Study.

12-Lead electrocardiography (ECG) is among the most frequently performed tests in medical practice. Despite its pivotal role in diagnostic and treatment decisions, baseline artifacts and errors in lead placement are common. The PATCH (Preferred Attachment Strategy for Optimal Electrocardiograms)-1 study enrolled patients with stable cardiovascular disease and a clinical indication for an ECG. Each participant underwent both a standard (S) 12-lead ECG and a patch (P) ECG (EKG-Patch™) during one routine ambulatory clinic visit. The P-ECG has an all-in-one design with built-in lead wires attached
