Impact of Hip Versus Knee Pain as the Presenting Symptom of Slipped Capital Femoral Epiphysis on Time to Imaging, Surgery, and Complications.

BACKGROUND: Slipped capital femoral epiphysis (SCFE) is a pediatric hip disorder affecting roughly 1 in 10,000 children and adolescents, with delayed diagnosis and treatment leading to poor outcomes. This study compares the association between presenting hip or knee pain symptomatology and its effects on time to diagnosis and treatment of SCFE, mid- to long-term complications, and risk of requiring hip reconstructive surgeries. METHODS: A retrospective cohort study was conducted comparing SCFE presentations with chief complaints of either hip or knee pain that led to SCFE surgery. Propensity

Finite element analyses for predicting anatomical neck fractures in the proximal humerus.

BACKGROUND: Proximal humerus fractures which occur as a result of a fall on an outstretched arm are frequent among the elderly population. The necessity of stabilizing such fractures by surgical procedures is a controversial matter among surgeons. Validating a personalized FE analysis by ex-vivo experiments of humeri and mimicking such fractures by experiments is the first step along the path to determine the necessity of such surgeries. METHODS: Four fresh frozen human humeri were loaded using a new simple experimental setting, so to fracture the humeri at the anatomical neck. Strains on hum

Treatment for Post-Slipped Capital Femoral Epiphysis Deformity.

In situ pinning of slipped capital femoral epiphysis (SCFE) is a safe and effective treatment modality, but often results in residual deformity leading to femoroacetabular impingement, which may limit patient activities and predispose to early onset arthritis. Enhanced understanding of the implications of femoroacetabular impingement and new surgical techniques have prompted interest in treating post-slipped capital femoral epiphysis deformity to both improve current symptoms and delay or prevent hip arthrosis.

Outcome following excision of the radial head in children with open physes for impaired elbow motion.

BACKGROUND: Only few reports have described the outcome of pediatric patients following radial head resection. Therefore, the aim of the present study was to assess clinical and radiologic outcome of patients with open physes following resection of the radial head. MATERIALS AND METHODS: Skeletally immature patients treated with resection of the radial head were included. Range of motion (ROM) of the elbow joint was compared with preoperative values. Grip strength, pronation and supination strength, and carrier angle were compared with the unaffected side. Radiographs were assessed for signs

Corrective osteotomy and in situ fusion for late-presenting nonunion of lateral condyle fractures of the humerus in adults.

BACKGROUND: Late presentation of lateral condylar fractures of the humerus is not an uncommon problem in children, and to lesser extent, in adults. The various surgical interventions for repairing the fracture, correcting the deformity, or even performing anterior transposition of the ulnar nerve as a single procedure or in combination, especially in adults, is still a controversial topic, with a paucity of literature regarding this. There is a risk of loss of the available preoperative range of motion, nonunion, and avascular necrosis. METHODS: In 19 patients with late presentation of latera

Relationships Between Severity of Deformity and Impingement in Slipped Capital Femoral Epiphysis.

BACKGROUND: In situ pinning, a low-risk treatment for slipped capital femoral epiphysis (SCFE), leaves the slipped femoral head in place and may reduce range of motion (ROM) and cause impingement. It is unclear when a more complex surgery should be considered, because the relationships between severity, slip stability, remodeling, impingement, and ROM are unknown. RESEARCH QUESTIONS: (1) Do more severe acute SCFE deformities (no bony remodeling) result in a greater loss of flexion ROM?(2) Does the presence or location of impingement on the pelvis vary with severity of acute SCFE deformity? Coxa vara in postseptic arthritis of the hip in children.

The behavior and treatment of coxa vara and pseudarthrosis of the proximal femur secondary to sepsis is not well described. The aim of this study is to describe the pathoanatomy for coxa vara and pseudarthrosis in postseptic hips, evaluate progression of neck shaft angle (NSA), and discuss treatment. This is a retrospective case series of 20 patients (21 hips). There were 11 hips with predominant avascular necrosis of the capital femoral epiphysis without pseudarthrosis (type 1) and 10 with pseudarthrosis (type 2). The interobserver κ value was 0.79. There was a decrease in NSA from 110.3° to

Common paediatric conditions of the lower limb.

Growing children are susceptible to a number of disorders to their lower extremities of varying degrees of severity. The diagnosis and management of these conditions can be challenging. With musculoskeletal symptoms being one of the leading reasons for visits to general practitioners, a working knowledge of the basics of these disorders can help in the appropriate diagnosis, treatment, counselling, and specialist referral. This review covers common disorders affecting the hip, the knee and the foot. The aim is to assist general practitioners in recognising developmental norms and differentiati

Deficiency of circadian clock protein BMAL1 in mice results in a low bone mass phenotype.

The circadian clock is an endogenous time keeping system that controls the physiology and behavior of many organisms. The transcription factor Brain and Muscle ARNT-like Protein 1 (BMAL1) is a component of the circadian clock and necessary for clock function. Bmal1(-/-) mice display accelerated aging and many accompanying age associated pathologies. Here, we report that mice deficient for BMAL1 have a low bone mass phenotype that is absent at birth and progressively worsens over their lifespan. Accelerated aging of these mice is associated with the formation of bony bridges occurring across th

Comparison of alternate references for femoral rotation in female patients undergoing total knee arthroplasty.

PURPOSE: Accurate rotational alignment of the femoral component is of vital importance for successful total knee arthroplasty (TKA). Two anatomical references located on the anterior femur were recently introduced. To determine which is more reliable reference axis for the femoral component rotation in female patients receiving TKA, the trochlear anterior line was compared with the femoral anterior tangent line. MATERIALS AND METHODS: Preoperative computed tomography in 76 patients receiving TKA for varus deformity was performed, and the images were reconstructed into three-dimensional models

Two cases of avascular necrosis after prophylactic pinning of the asymptomatic, contralateral femoral head for slipped capital femoral epiphysis: case report and review of the literature.

BACKGROUND: In a child with a unilateral slipped capital femoral epiphysis (SCFE), the treatment of the radiographically normal, asymptomatic contralateral hip remains controversial. The risks of a subsequent slip have to be measured against the risks involved with an additional surgical procedure. Proponents of prophylactic pinning believe that fixation with a single cannulated screw is a safe method to prevent secondary arthrosis from a missed minor slip. Others argue that in most cases it is an unnecessary procedure and with careful follow-up, any issues with the asymptomatic, normal hip ca

Expression of matrix Gla protein and osteocalcin in the developing tibial epiphysis of mice.

This study aimed to investigate the expression of matrix Gla protein (MGP) and osteocalcin (OCN) in the tibial epiphysis of developing mice. At 1, 2, 3, and 4 weeks after birth, tibiae were removed and processed for histochemical observations and western blot analyses under anesthesia. To evaluate bone volume, the specimens were scanned with Micro CT Scanner from the articular cartilage through the growth plate, along the long axis of tibia. At 1 week after birth, OCN reactivity was faint in the region of vascular invasion, while hardly any MGP reactivity was discernible. Subsequently, MGP rea

Surgical hip dislocation: techniques for success.

Surgical hip dislocation (SHD) is a versatile approach used to address both intra-articular and extra-articular pathology around the hip joint in both pediatric and adult patients. It allows anterior dislocation of the femoral head for direct visualization of the hip joint while preserving femoral head vascularity and minimizing trauma to the abductor musculature. Previously described indications for SHD include femoroacetabular impingement, deformity resulting from Legg-Calve-Perthes disease, slipped capital femoral epiphysis, periarticular trauma, benign lesions of the hip joint, and osteoch

Biomechanical analysis of screw fixation vs. K-wire fixation of a slipped capital femoral epiphysis model.

INTRODUCTION: Previous data have shown that due to the technical ease, low-morbidity, and lower complication rates, the in situ single-implant fixation is the current standard for stabilization of slipped capital femoral epiphysis (SCFE) fixation. Multiple-implant fixation is thought to be combined with a higher incidence of serious complications. The purpose of the current study was to evaluate single- vs. multiple-implant fixation regarding strength and stiffness. Furthermore, different screw designs, including telescopic screw, were evaluated regarding the stiffness, strength, and especiall

Two novel COL2A1 mutations associated with a Legg-Calvé-Perthes disease-like presentation.

BACKGROUND: Abnormal development and growth of the capital femoral epiphysis and acetabulum are associated with a wide variety of underlying etiologies, one of which is Legg-Calvé-Perthes disease. CASE DESCRIPTION: We report the cases of two children who presented with abnormal development of both hips and in whom novel mutations in the COL2A1 gene were found. These cases illustrate the importance of identifying individuals with a type II collagen abnormality, as it informs management, allows investigation for other complications, and provides the opportunity for accurate genetic counseling a

Elevated cross-talk between subchondral bone and cartilage in osteoarthritic joints.

Osteoarthritis (OA) is a degenerative joint disease and one of the leading causes of disability in the United States and across the world. As a disease of the whole joint, OA exhibits a complicated etiology with risk factors including, but not limited to, ageing, altered joint loading, and injury. Subchondral bone is hypothesized to be involved in OA development. However, direct evidence supporting this is lacking. We previously detected measurable transport of solute across the mineralized calcified cartilage in normal joints, suggesting a potential cross-talk between subchondral bone and car

Complete slippage of capital femoral epiphysis: total hip arthroplasty with custom-made stem.

Slipped capital femoral epiphysis occurs in adolescents, most often shortly after the onset of puberty. In patients suffering form renal osteodystrophy, the incidence is higher and the condition usually occurs at a younger age. Metabolic changes induce weakening of the bone, which makes the hip joint vulnerable. The initial treatment consists of phosphate-restricted diet, lanthanum carbonate, cinacalcet (a calcimimetic), calcium carbonate and oral calcitriol, aiming at restoration of bone quality. The use of several surgical treatment options has been reported. In this case, which was diagnose

Simultaneous biplanar fluoroscopy for the surgical treatment of slipped capital femoral epiphysis.

BACKGROUND: The current standard of care for treatment of slipped capital femoral epiphysis (SCFE) is in situ placement of a single, cannulated screw across the physis under direct fluoroscopic guidance. Previous studies have reported the theoretical advantages of shorter operative time and improved accuracy of screw placement when 2 fluoroscopy units are used simultaneously. METHODS: A retrospective review was performed to compare the use of 1 versus 2 C-arms in the surgical stabilization of SCFE. Data analysis, including demographics, surgical setup times, operative times, and precision of
