Does Virtual Reality Improve Procedural Completion and Accuracy in an Intramedullary Tibial Nail Procedure? A Randomized Control Trial.

BACKGROUND: Artificial reality technologies are currently being explored as potential options to improve surgical education. Previous studies have primarily examined the efficacy of artificial reality in laparoscopic procedures, but to our knowledge, none have been performed in orthopaedically relevant procedures such as intramedullary tibial nailing, which calls for more versatile large-scale movements. QUESTIONS/PURPOSES: Does a virtual reality simulator with or without a standard technique guide result in (1) a higher proportion of participants who completed the insertion of an intramedull

Medium-term evaluation of leg lengthening by ISKD® intramedullary nail in 28 patients: Should we still use this lengthening system?

INTRODUCTION: Lower-limb lengthening presently uses intramedullary nailing. There are motorized systems and mechanical systems, each with their specific complications. The aim of the present study was to assess the efficacy of ISKD® mechanical nails (Orthofix Inc., Texas, USA) on 3D EOS® imaging, and also complications and functional impact. HYPOTHESIS: The study hypothesis was that nail lengthening is effective but should no longer use the ISKD® system, due to the high rate of specific complications. MATERIAL AND METHODS: A single-center retrospective study included 28 patients (14 female,

Cerebral fat emboli: a trigger of post-operative delirium.

Accumulating evidence implicates cerebral fat embolism (CFE) as a causative agent in post-operative confusion (POC). CFE occurs following orthopaedic procedures including, intra-medullary (IM) nailing and total joint arthroplasty (TJA). The incidence of CFE is high (59-100% TJA) and the resulting POC is associated with higher overall complication rates. Cognitive dysfunction improves in many patients but can persist - with potentially disastrous outcomes. The pathomechanics of CFE implicate circulating lipid micro-emboli (LME) that are forced from IM depots by instrumentation/nailing. Passage

[Chronic sclerosing osteomyelitis. Apropos of 4 cases and the value of the treatment by closed intramedullary reaming].

Chronic sclerosing osteomyelitis is a rare variety of primary osteomyelitis located principally on the diaphysis of the long bones in adults. Pain is the main symptom, but biological signs are negative or normal, and histological examination is non-specific. X-rays are very stereotyped with enlargement of the cortex of diaphyseal bone. This disease is note worthy by its difficult diagnosis, and is often misdiagnosed as bone tumor, malignant or benign, especially osteoid osteoma. Four observations are reported and in one case, after 13 years, an abscess developed, bearing out the oneness of chr

Outcome of supracondylar fractures of the humerus in children treated by closed reduction and percutaneous pinning.

Supracondylar fractures of humerus in children are common injuries. Displaced fractures are inherently unstable. Conservative treatment results in malunion. Open reduction and internal fixation (ORIF) is more invasive and recovery is prolonged. From September 2004 to September 2005, 102 displaced supracondylar fractures of humerus, aged between one and half year to 13 years, were treated using close reduction and percutaneous Kirschner (K) wire fixation under c-arm fluoroscopy. Seventy nine patients were treated by cross K-wires and in twenty three cases lateral two K-wires were put. Above elb

Complications and imaging artifacts related to MRI in patients with intramedullary osteosynthesis after proximal femur fracture.

BACKGROUND: Fractures of the proximal femur is one of the most frequent human injuries, and most of the patients are treated with osteosynthesis, such as intramedullary nails. These patients often require magnetic resonance imaging (MRI) scans in their further lives due to various reasons. This raises the question of whether complications with implanted osteosynthesis material such as implant loosening, burning, dislocation, or other complications are to be expected or whether an MRI examination is even suitable with regard to imaging artifacts. METHODS: The aim of this retrospective study wa

Intramedullary nailing of tibial shaft fractures in the semi-extended position using a suprapatellar approach: A retrospective case series.

Tibial shaft fractures are extremely common injuries. High rate of union, acceptable alignment in all planes, and a low complication rate have been reported when diaphyseal fractures are treated by intramedullary nailing. However, knee pain after tibial nailing has historically been problematic. While the exact aetiology of the knee pain is still unknown, surgical approach relative to the patellar tendon, nerve transection, violation of the fat pad or joint capsule as well as nail diameter and implant prominence have been claimed as possible causes. Therefore, establishing an appropriate start

Surgical approaches to intramedullary nailing of the tibia: Comparative analysis of knee pain and functional outcomes.

INTRODUCTION: Post-operative knee pain is common following intramedullary nailing of the tibia, regardless of surgical approach, though the exact source is controversial. Historically, the most common surgical approaches position the knee in hyperflexion, including patellar tendon splitting (PTS) and medial parapatellar (MPP). A novel technique, the semi-extended lateral parapatellar approach simplifies patient positioning, fracture reduction, fluoroscopic assessment, and implant insertion. It also avoids violation of the knee joint capsule. However, this approach has not yet been directly com

Fluoroscopic freehand and electromagnetic-guided targeting system for distal locking screws of humeral intramedullary nail.

PURPOSE: The current techniques used to lock distal screws for the nailing of long bone fractures expose the surgeons, radiologists and patients to a hearty dose of ionizing radiation. The Sureshot™ Distal Targeting System is a new technique that, with the same results, allows for shorter surgery times and, consequently, less exposure to radiation. MATERIALS AND METHODS: The study was performed on 59 patients (34 males and 25 females) with a simple humerus fracture diagnosis, type 1.2.A according to the AO classification, who were divided into two groups.
