Functional results and survival after surgery for peripheral skeletal metastasis: A 434-case multicenter retrospective series.

INTRODUCTION: Peripheral skeletal metastasis (PSM) has a negative impact on quality of life. New treatments for the primary tumor or the osteolysis hold out hope of improved survival. The few published French series were small, and we therefore undertook a multicenter retrospective analysis of PSM surgery between 2005 and December 2016, with the aim of assessing: 1) rate and type of complications, 2) functional results, and 3) overall survival and corresponding risk factors. HYPOTHESIS: The French data for clinical results, survival and complications are in agreement with the international li

Suitability of the Oxford Shoulder Score for measuring pain in clinical trials evaluating interventions for people with shoulder disorders according to the OMERACT filter 2.2.

BACKGROUND: Pain is a mandatory domain in Outcome Measures in Rheumatology (OMERACT) core outcome sets for shoulder disorder trials. We evaluated the Oxford Shoulder Score (OSS), a composite measure comprising four pain and eight function items but no separate subscales for these domains, for measuring pain using the OMERACT Filter 2.2. METHODS: Following the OMERACT Handbook, we assessed domain match and feasibility, then systematically reviewed OSS measurement properties in shoulder disorders (rotator cuff disease, adhesive capsulitis, instability, osteoarthritis, dislocation, humeral head

Risk factors for poor physical and psychological outcomes after traumatic brachial plexus injury: a multicenter retrospective cohort study.

BACKGROUND: Traumatic brachial plexus injury (BPI) is associated with upper limb dysfunction, neuropathic pain (NP), and depressive symptoms … . Most injuries were closed (81.65%), and 81.04% patients had concomitant injuries, most frequently humeral fractures (36.70%). The most common injury pattern was C5-C6 (27.22%). Surgery was performed within 3 months in 195 patients. Over a mean follow-up of 45.78 months, 216 patients achieved muscle strength of M3 or higher. Independent risk factors for poor funct …

Treatment outcomes for rotator cuff arthropathy: A systematic review of reverse total shoulder arthroplasty with or without augmentation.

BACKGROUND: Rotator cuff arthropathy (RCA) involves shoulder joint degeneration due to rotator cuff insufficiency, leading to pain, limited motion, and functional decline. Reverse total shoulder arthroplasty (RTSA) with rotator cuff repair, with tendon transfer, and without tendon repair or transfer are all commonly used to restore function if conservative treatments fail. This study systematically compares outcomes between the three methods. METHODS: A literature review was conducted using PubMed, Embase, MEDLINE, and Cochrane. Inclusion criteria: patients 18 years or older who were treated

Deciphering acute shoulder trauma with normal initial X-ray: Contributions of ultrasonography and MRI.

INTRODUCTION: Shoulder trauma with normal radiographs is a frequent emergency presentation. The predominance of pain makes clinical examination unreliable, and complementary imaging is often needed for diagnosis. The aim of the present study was to assess the contribution of ultrasonography (US) and magnetic resonance imaging (MRI) in deciphering such trauma. The study hypothesis was that MRI can confirm most diagnoses and reveal occult lesions. METHODS: 48 patients with a median age of 33 years (IQR, 22-48.75), admitted to the Emergency Department between February and September 2016, were pr

Joint-preserving treatment for type 3 sequelae following fracture of the proximal humerus with small head fragments.

BACKGROUND: Although nonunions of the proximal humerus are rare, they cause significant disability to patients. Surgical reconstruction is challenging, especially with small and excavated head fragments. A promising surgical option is open reduction and stabilization using the Humerusblock device along with tension wires. The aim of this retrospective investigation was to evaluate the clinical and radiological results of this procedure. MATERIALS AND METHODS: Fifteen patients with symptomatic surgical neck nonunions were treated with open reduction and internal fixation using the Humerusblock

Epidemiology and risk factors of humerus fractures among skiers and snowboarders.

BACKGROUND: The incidence of humerus fractures while participating in snowboarding and skiing is undefined. Very little is known about the risk factors associated with these fractures. HYPOTHESIS: Snowboarders are at increased risk for sustaining humerus fractures when compared with skiers. In addition, the types of fractures, laterality, and risk factors differ between the 2 groups. STUDY DESIGN: Case-control study; Level of evidence, 3. METHODS: At a major ski area clinic, 318 humerus fractures were evaluated over 34 seasons. Radiographs were classified according to the AO and Neer system

Low-energy arterial injury at the shoulder with progressive or delayed nerve palsy.

We describe 20 patients, aged between 43 and 88 years, with delayed nerve palsy or deepening of an initial palsy caused by arterial injury from low-energy injuries to the shoulder. The onset of palsy ranged from immediately after the injury to four months later. There was progression in all the patients with an initial partial nerve palsy. Pain was severe in 18 patients, in 16 of whom it presented as neurostenalgia and in two as causalgia. Dislocation of the shoulder or fracture of the proximal humerus occurred in 16 patients. There was soft-tissue crushing in two and prolonged unconsciousness

Treatment of The Posterior Unstable Shoulder.

BACKGROUND: It is estimated that approximately 5% of glenohumeral instabilities are posterior. There are a number of controversies regarding therapeutic approaches for these patients. METHODS: We analyse the main surgery alternatives for the treatment of the posterior shoulder instability. We did a research of the publications related with posterior glenohumeral instability. RESULTS: There are conservative and surgical treatment options. Conservative treatment has positive results in most patients, with around 65 to 80% of cases showing recurrent posterior dislocation. There are multiple sur

[Fractures of the elbow joint].

Fractures around the elbow joint comprise fractures of the distal humerus, the radial head, the olecranon and the coronoid process. Combined lesions are particularly demanding for the surgeon. Accurate knowledge of the anatomy and of the biomechanics is an essential requirement for a specific diagnosis and therapy. A stable and painless movable elbow joint is essential for most of the activities of daily living. Risk factors for the development of posttraumatic elbow joint arthrosis are non-anatomically reconstructed joint surfaces, axial malalignment of the joint axis and untreated concomitan

Nerve injury in revision total elbow arthroplasty: a systematic review and meta-analysis.

BACKGROUND: Revision total elbow arthroplasty (TEA) is technically demanding and carries a substantial risk of postoperative neurological complications because of scarring, altered anatomy, implant removal, and repeated humeral and ulnar exposure. The incidence, nerve distribution, and recovery profile of nerve injury after revision TEA remain incompletely defined. This study aimed to systematically review the literature to define the incidence, recovery profile, and risk factors for nerve injury after revision TEA. METHODS: A systematic review of the literature was performed in accordance wi

Asfotase alfa improved skeletal mineralization and fracture healing in a child with MCAHS.

Tissue non-specific alkaline phosphatase (TNSALP) is an enzyme that is tethered to the cell membrane by glycosylphosphatidylinositol (GPI) and converts inorganic pyrophosphate to inorganic phosphate. Inorganic phosphate combines with calcium to form hydroxyapatite, the main mineral in the skeleton. When TNSALP is defective, conversion of inorganic pyrophosphate to inorganic phosphate is impaired and the skeleton is at risk of under-mineralization. Phosphatidylinositol glycan anchor biosynthesis class N (PIGN) is one of more than 20 genes in the GPI-biosynthesis family. Pathogenic variants in P

Obstetric neonatal brachial plexus and facial nerve injuries: A 17 years single tertiary maternity hospital experience.

OBJECTIVES: To establish the incidence of obstetric neonatal brachial plexus and facial nerve injuries in a tertiary maternity hospital in the United Kingdom and to identify associated risk factors with an emphasis on the time of delivery. STUDY DESIGN: From our hospital electronic data bases we identified all live births born between 2000 and 2016 and those neonates who sustained a nerve injury during delivery. We performed a logistic regression analysis linking "facial nerve injury" and "brachial plexus injury" with variables for which we had complete cohort data including "breech", "gestat

Anatomy of the median nerve and its clinical applications.

The median nerve is a mixed sensory and motor nerve. It is classically described as the nerve of pronation, of thumb, index finger, middle finger and wrist flexion, of thumb antepulsion and opposition, as well as the nerve of sensation for the palmar aspect of the first three fingers. It takes its name from its middle position at the end of the brachial plexus and the forearm. During its course from its origin at the brachial plexus to its terminal branches, it runs through various narrow passages where it could be compressed, such as the carpal tunnel or the pronator teres. The objective of t

Force, fatigue, and the cross-sectional area of wrist extensor muscles after radial nerve grafting.

The force and fatigue of the wrist extensor muscles during maximal voluntary and tetanic contractions were measured and compared in the injured and noninjured extremities of 11 patients with radial nerve gap injury and in 9 normal volunteers. The cross-sectional area (CSA) of the wrist extensor muscles was determined by magnetic resonance imaging and was correlated with force. In the patient group, an average of three (range, 2-4) sural nerve cable grafts, measuring 11.5 +/- 5 cm (range, 5-20 cm) in length, were sutured to the nerve stumps at least 9 years before this study.
