Associated aneurysms in supratentorial arteriovenous malformations: impact of aneurysm size on haemorrhage.

BACKGROUND: Associated aneurysms (AAs) are presumed to represent an additional risk factor for intracranial haemorrhage from cerebral arterio-venous malformations (AVMs). To date, efforts to capture their natural history, as well as to identify aneurysms with the potential capability of regression after AVM treatment remain incomprehensive. As the aneurysm size represents an important aspect for the treatment indication of incidental saccular aneurysms, this factor has rarely been encountered for the treatment of AAs so far. The present study aims to determine the angiographic and clinical cha

Neuropsychological outcomes of stereotactic radiotherapy for cerebral arteriovenous malformations.

To our knowledge, this is the first study to investigate cognitive outcome in patients with large or surgically inaccessible cerebral arteriovenous malformations (AVMs), who were treated with hypo-fractionated stereotactic radiotherapy (HSRT). A sample of 10 patients with AVMs was assessed up to 3.5 years post-HSRT. All patients were treated with HSRT to a total dose of 55 Gy in 11 fractions over a treatment period of 2.5 weeks. Neuropsychological assessments were given prior to radiotherapy and then at three time points following radiotherapy: 6 weeks, 6 months and 2.5-3.5 years post-treatmen

Complex dural arteriovenous fistula in Bannayan-Riley-Ruvalcaba syndrome.

In this paper the authors report the case of a complex dural arteriovenous fistula (dAVF) with high-risk features in a 14-year-old girl with Bannayan-Riley-Ruvalcaba syndrome (BRRS), a phosphatase and tensin homolog-associated syndrome, presenting with signs and symptoms of increased intracranial pressure (ICP) that had previously been attributed to pseudotumor cerebri. This fistula was obliterated following 2 stages of embolization, and the patient experienced immediate symptomatic improvement. At the 2-month follow-up evaluation, the fistula remained angiographically occluded, and her sympto

Vasospasm after spontaneous angiographically negative subarachnoid hemorrhage.

BACKGROUND: Vasospasm, delayed infarcts and clinical deterioration due to delayed cerebral ischemia (CD-DCI) following digital subtraction angiography (DSA)-negative, spontaneous subarachnoid hemorrhage (SAH) are infrequently and inconsistently described in the literature. METHODS: To quantify and characterize rates of each, we reviewed our own series of 77 patients with DSA-negative, spontaneous SAH. Employing a PubMed search, we reviewed the literature and calculated directly adjusted overall rates of vasospasm, delayed infarcts and CD-DCI. RESULTS: In our own patient cohort, 26 % of patie

The course and outcome of unilateral intracranial arteriopathy in 79 children with ischaemic stroke.

Arteriopathies are the commonest cause of arterial ischaemic stroke (AIS) in children. Repeated vascular imaging in children with AIS demonstrated the existence of a 'transient cerebral arteriopathy' (TCA), characterized by lenticulostriate infarction due to non-progressive unilateral arterial disease affecting the supraclinoid internal carotid artery and its proximal branches. To further characterize the course of childhood arteriopathies, and to differentiate TCA from progressive arterial disease, we studied the long-term evolution of unilateral anterior circulation arteriopathy, and explore

Bilateral vertebral artery dissection and infratentorial stroke complicated by stress-induced cardiomyopathy.

The Global Programme to Eliminate Lymphatic Filariasis (GPELF) was launched in 2000. To understand why some national programs have been more successful than others, a panel of individuals with expertise in LF elimination efforts met to assess available data from programs in 8 countries. The goal was to identify: 1) the factors determining success for national LF elimination programs (defined as the rapid, sustained reduction in microfilaremia/antigenemia after repeated mass drug administration [MDA]); 2) the priorities for operational research to enhance LF elimination efforts. Of more than 40

CT angiography: application to the evaluation of carotid artery stenosis.

PURPOSE: To determine whether computed tomographic (CT) angiography can be used to accurately quantify carotid stenoses and differentiate severe stenoses from occlusions. MATERIALS AND METHODS: CT angiography was used to evaluate 50 carotid arteries in 27 patients who were referred for surgical evaluation after diagnosis of carotid stenosis and who had undergone standard angiography. Four observers read each study separately in a blinded fashion, then all four observers together reviewed those studies in which the individual readings differed to reach a consensus reading. RESULTS: The consen

Contrast-enhanced ultrasound reveals free-floating thrombus in carotid artery: The cause of stroke is surprisingly plaque rupture.

BACKGROUND: Acute stroke poses a serious threat to people's health. The occurrence of a thrombus following the rupture of vulnerable plaques in the carotid artery is a significant contributor to the development of stroke. In previous case reports, it has been challenging to visualize tiny ulcerations within carotid artery plaques using computed tomography angiography (CTA) and digital subtraction angiography (DSA), even when the rupture of the plaque leads to the formation of a free-floating thrombus (FFT). However, in this particular case, contrast-enhanced ultrasound (CEUS) was able to overc

Sagittal splitting of the temporalis muscle for encephalo-myo-synangiosis to prevent ischemic complications due to a swollen temporalis muscle without inhibiting collateral developments in patients with moyamoya disease.

OBJECTIVE: Encephalo-myo-synangiosis (EMS) is an effective revascularization procedure for the treatment of moyamoya disease (MMD). However, the temporalis muscle used for EMS sometimes swells and causes ischemic complications by compressing the underlying brain. This study aimed to elucidate the effect of sagittal splitting (SS) of the muscle for prevention of ischemic complications and its impact on the postoperative development of collateral vessels. METHODS: In this historical case-control study, we analyzed 60 hemispheres in adult patients with MMD who underwent EMS using the temporalis

Application of contrast-enhanced ultrasound before inferior vena cava filter recovery.

BACKGROUND: This study aims to investigate the clinical value of contrast-enhanced ultrasound (CEU) before temporary inferior vena cava filter (IVCF) recovery in patients with deep venous thrombosis, in order to provide ultrasound signs for the recovery of IVCF in clinical practice. METHODS: The CEU manifestations of patients with deep vein thrombosis before temporary IVCF recovery were retrospectively analyzed. With the manifestations of digital subtraction angiography (DSA) or results of the surgical recovery of IVCF as the standard, the detection rate of a thrombus in IVCF was compared bet

A Compact Unified Model for Moderate-to-Severe Carotid Artery Stenosis Using Image Segmentation: A Multicenter Study.

BACKGROUND: This study aimed to evaluate the diagnostic performance of a new artificial intelligence model, the compact unified model (CU-Model), for moderate-to-severe carotid artery stenosis using digital subtraction angiography (DSA) images compared to clinical reference standards. METHODS: In this retrospective multicenter study, 156 patients with confirmed moderate-to-severe carotid artery stenosis were included. A pretrained CU-Model (trained on public non-clinical datasets for optical flow and stereo matching) was applied to raw DSA images without any fine-tuning on clinical data to av

Drug-coated balloon versus bare-metal stent for the treatment of symptomatic vertebral artery origin stenosis: a systematic review and meta-analysis.

BACKGROUND: Symptomatic vertebral artery origin stenosis (VAOS) is associated with high recurrent stroke risk despite medical therapy. While bare-metal stents (BMS) are established for endovascular treatment, in-stent restenosis remains common. Drug-coated balloons (DCB) offer a stent-free alternative, but comparative effectiveness is uncertain. METHODS: We searched PubMed, Embase, the Cochrane Library, Web of Science, and Scopus for comparative studies of DCB vs. BMS in symptomatic VAOS. The primary outcome was imaging-confirmed restenosis; secondary outcomes included symptomatic restenosis,

Endovascular therapy for symptomatic non-acute middle cerebral artery occlusion: impact on Lenticulostriate arteries reperfusion and clinical safety.

BACKGROUND: Non-acute middle cerebral artery occlusion (NAMCAO) is a significant cause of ischemic stroke, often resulting in persistent neurological deficits and a high risk of recurrence. Endovascular therapy (EVT) shows potential benefit but faces challenges related to microvascular reperfusion and procedural safety, particularly in the perforator territories supplied by Lenticulostriate arteries (LSAs). METHODS: A retrospective cohort of 112 consecutive patients with symptomatic NAMCAO treated by EVT at three centers was analyzed. Clinical assessments included NIHSS and modified Rankin Sca

Efficacy of preoperative venography in patients undergoing placement of totally implantable venous access ports.

BACKGROUND: Given the limitations of ultrasound-guided totally implantable venous access ports (TIVAPs) placement in detecting venous abnormalities and predicting pinch-off syndrome, this study evaluates whether preoperative venography can improve surgical planning and reduce complications by identifying these anatomical risks prior to implantation. METHODS: The retrospective study analyzed 479 cancer patients undergoing TIVAP implantation from January 2019 to December 2021. Patients received either preoperative venography (n=252) or conventional ultrasound guidance (n=227) to guide surgical

Long-term Outcomes after Staged and Multimodal Treatment for Diffuse Intracranial Dural Arteriovenous Fistulas Involving Interconnected Dural Sinuses: A Single-center Case Series.

Diffuse intracranial dural arteriovenous fistulas involving interconnected dural sinuses are uncommon lesions with complex angioarchitecture and an evolving clinical course. However, long-term treatment outcomes and surveillance strategies remain poorly characterized. We retrospectively reviewed 49 consecutive patients with intracranial dural arteriovenous fistulas treated at our institution for 10 years. The study included 9 patients with diffuse intracranial dural arteriovenous fistulas involving 2 or more interconnected dural sinus compartments. Endovascular treatment was used as the first-

Use of Doppler Ultrasound During Surgical Treatment of Intradural Spinal Arteriovenous Fistula.

Spinal dural arteriovenous (AV) fistulas constitute 70% of spinal arteriovenous malformations. If left untreated, patients typically experience progressive myelopathy, and surgical treatment remains the most definitive and curative strategy. We present an operative video illustrating the use of Doppler ultrasound during clip ligation and disconnection of a spinal AV fistula. A 73-year-old male patient presented with myelopathic symptoms with weakness and diminished sensation in his bilateral lower extremities. Neuroimaging demonstrated spinal cord edema from T6 to the conus with prominent epid
