Economic and educational disparities are associated with an increased risk of revision surgery following shoulder arthroplasty.

BACKGROUND: There is limited literature exploring how nonmedical factors such as social determinants of health (SDOHs) are associated with postoperative outcomes following shoulder arthroplasty. METHODS: A retrospective cohort review of the Pearl Diver Database was used to capture patients undergoing either primary anatomic total or reverse shoulder arthroplasty from the fourth quarter of 2015 to the third quarter of 2019 with at least 1 year of active longitudinal follow-up. Patients with proximal humeral fractures, stress fractures, or septic arthritis were excluded. The included patients w…

Anterior humeral approach with nerve stimulator.

INTRODUCTION: The prevalence of radial nerve injury during surgery is as high as the prevalence of radial nerve injury due to trauma. The aim of this study is to minimize the risk of iatrogenic injury of radial nerve. MATERIALS AND METHODS: Fifty patients with middle or distal diaphysis fractures of humerus and 18 patients with pseudoarthrosis at the same localizations were treated with surgery. Plate-screw fixation was performed with anterior approach in 43 patients. Eleven patients had minimally invasive plate osteosynthesis, and 14 patients had intramedullary nailing. The localization of t

A selective treatment approach to supracondylar fracture of the humerus in children.

The results of a "selective treatment" policy applied to 176 patients presenting with supracondylar fracture of the humerus are analyzed. As initial treatment 148 patients underwent closed reduction and casting, seven closed reduction and pinning, 17 olecranon traction, and four open reduction and pinning. Analysis of patients by fracture type and initial treatment demonstrated a satisfactory outcome in 46 of 75 type III fractures and in 37 of 48 patients with type II fractures treated by closed reduction and casting. Thus a policy of initially pinning all such type III fractures in our unit w

The Measurement Methods of Movement and Grip Strength in Children with a Previous Upper Extremity Fracture: A Comparative, Prospective Research.

BACKGROUND AND AIMS: Decreased range of motion of the elbow and forearm and decreased grip strength are potential findings following a childhood upper extremity fracture. Clinical follow-up is essential because spontaneous improvement is seen several months after the injury. Freehand measuring with a goniometer and hydraulic dynamometer is used to evaluate clinical result. The new methods are justified in avoiding human typewriting errors, thus improving patient safety. Nevertheless, their feasibility in child patients is unknown. This study aimed to evaluate congruence between the computer-as

Translation step-cut osteotomy for the treatment of posttraumatic cubitus varus.

BACKGROUND: Cubitus varus deformity is the most common complication after the treatment of displaced supracondylar humerus fractures. METHODS: A retrospective analysis was performed evaluating patients who had undergone a translation step-cut osteotomy for correction of cubitus varus deformity between 1993 and 2008. Postoperative radiographs to union were reviewed for all patients. Subjective information and range of motion measures were documented from the medical records. Patients and their families were also contacted through telephone and administered a questionnaire as well as the QuickD

Oblique closing wedge osteotomy and lateral plating for cubitus varus in adults.

UNLABELLED: Corrective osteotomy around the elbow can require longer recovery time in adults than in children because of the longer healing period and the propensity for stiffness. We hypothesized a lateral oblique closing wedge osteotomy with a larger contact area and fixation with a lag screw plus early motion would provide stable fixation and early motion recovery in adults with cubitus varus deformity. Twelve consecutive patients who needed surgery were treated using this procedure. They were allowed active motion exercises 1 week postoperatively. The age of the patients at the time of sur

The clinical characteristics and demographic distribution of shoulder arthroplasty in China: a nationwide hospital-based study.

INTRODUCTION: We aimed to identify variations in basic clinical features, and delineate the demographic distribution of shoulder arthroplasty (SA) in China from 2013 to 2018. MATERIALS AND METHODS: We included patients undergoing SA between 2013 and 2018 in the Hospital Quality Monitoring System (HQMS) in China. Descriptive statistics were used to assess the characteristics of patients undergoing SA with regard to sex ratio, age composition, indications, comorbidities, and geographic distribution, and the Mantel-Haenszel chi-square tests were used for trend analysis. RESULTS: A total of 3,91

Irreducible shoulder dislocation in a patient with Alzheimer's disease: A case report.

INTRODUCTION: Anterior shoulder dislocations are common injuries usually managed by closed reduction. However, irreducible cases, especially in elderly patients with Alzheimer's disease, complicate both diagnosis and management. CASE PRESENTATION: A 78-year-old female with Alzheimer's disease presented with severe right shoulder pain and reduced function following physiotherapy. Cognitive decline complicated neurovascular assessment and communication. Radiographs identified a right anterior shoulder dislocation with a displaced greater tuberosity fracture. Multiple closed reductions under sed

Synchronous bone and adrenal extramedullary plasmacytoma: a rare case report and literature review.

BACKGROUND: Adrenal extramedullary plasmacytoma (EMP) is an exceptionally rare plasma cell neoplasm with fewer than 20 documented cases worldwide. The clinical presentation, optimal management strategies, and long-term outcomes remain poorly defined. CASE PRESENTATION: We report the case of a 51-year-old woman with synchronous bone and adrenal EMP, without multiple myeloma. The initial presentation involved pathological humeral fracture following minor trauma, with the subsequent discovery of a 40 mm left adrenal mass on staging CT. Emergency orthopedic fixation and bone biopsy confirmed the

Safe corridors for external skeletal fixator pin placement in feline long bones.

OBJECTIVES: External skeletal fixation is an established technique in cats for biological fixation of long bone fractures, stabilisation of the joints, and treatment of shearing injuries and angular deformities. As appropriate and accurate pin insertion is imperative for a successful outcome, knowledge of topographic anatomy and areas that are safe (safe corridors) for pin placement is integral to successful surgery. At present, however, safe corridors have not been determined fully in feline orthopaedics, with surgeons having to rely on knowledge based on canine orthopaedics. This study was p

Anatomical Factors Contributing to Radial Nerve Excursion at the Brachium: A Cadaveric Study.

PURPOSE: The radial nerve appears to be more vulnerable to injury in the brachium than the median and ulnar nerves. The underlying mechanism for this increased vulnerability is not well explained. We hypothesize that the radial nerve has less excursion than the median and ulnar nerves because it is anatomically tethered by the lateral intermuscular septum (LIS) and that elbow positioning and LIS release will improve its excursion. METHODS: Eight paired fresh-frozen cadaveric upper extremity specimens were used. The radial, median, and ulnar nerves were transected at the level of the spiral gr

Arthroscopic management of proximal humerus malunion with tuberoplasty and rotator cuff retensioning.

PURPOSE: To report the mid- to long-term results of arthroscopic tuberoplasty and rotator cuff retensioning for proximal humerus malunion. METHODS: Between August 2001 and October 2009, 9 patients with a mean age of 49 years underwent shoulder arthroscopy with tuberoplasty and rotator cuff advancement for malunion of the proximal humerus and were included in this study. The mean delay between the initial fracture and our surgery was 19 months. We developed a systematic technique to take down the rotator cuff over the malunited proximal humerus, perform a tuberoplasty, and then retension and r

What factors can reduce the need for repeated revision for humeral loosening in revision total elbow arthroplasty?

BACKGROUND: This study aimed to determine the re-revision rate in a cohort of patients who underwent revision total elbow arthroplasty (rTEA) for humeral loosening (HL) and identify factors contributing to re-revision. We hypothesized that proportional increases in the stem and flange lengths would stabilize the bone-implant interface significantly more than a disproportional increase in stem or flange length alone. Additionally, we hypothesized that the indication for the index arthroplasty would impact the need for repeated revision for HL. The secondary objective was to describe the functio
