Immunocompetent infants as a human reservoir for Pneumocystis jirovecii: rapid screening by non-invasive sampling and real-time PCR at the mitochondrial large subunit rRNA gene.

We tested a real-time PCR assay targeting the Pneumocystis jirovecii mitochondrial large subunit rRNA gene on 240 archival nasopharyngeal aspirates from non-immunosuppressed infants. The sensitivity of this assay appeared close to that of a conventional nested-PCR assay targeting the same locus. Because of its one-step procedure, and its sensitivity and rapidity, the real-time PCR assay is particularly suitable for screening individuals parasitized by P. jirovecii within large populations.

Review of epidemiology and clinical risk factors for severe respiratory syncytial virus (RSV) infection.

Respiratory syncytial virus (RSV) infection is the most frequent reason for hospitalization of infants in developed countries. Premature birth without or, especially, with chronic lung disease of prematurity, congenital heart disease, and T-cell immunodeficiency are conditions that predispose to more severe forms of RSV infection. Incomplete development of the airway, damage to the airway, and airway hyperreactivity underlie the increased morbidity of RSV infection in prematurely born infants. Pulmonary hypertension and cyanosis are associated with worse outcomes in infants with congenital hea

Differential immune responses to acute lower respiratory illness in early life and subsequent development of persistent wheezing and asthma.

BACKGROUND: Recent epidemiologic evidence suggests that 2 wheezing syndromes coexist in early life: transient wheezing, limited to early childhood, and persistent wheezing, which starts in early childhood and persists beyond that age. OBJECTIVE: Whether the nature of the immune response occurring during acute lower respiratory illnesses (LRIs) in infancy differs between these 2 groups of wheezers has yet to be determined. METHODS: We compared total serum IgE levels and peripheral blood eosinophil counts obtained during the acute phase of the first LRI with those obtained during the convalesc

Inflammation-induced cysts mimic honeycombing on high-resolution computed tomography in rheumatoid arthritis-associated usual interstitial pneumonia.

BACKGROUND: Rheumatoid arthritis-associated interstitial lung disease (RA-ILD) often manifests with a usual interstitial pneumonia (UIP) pattern and has a poor prognosis similar to idiopathic pulmonary fibrosis (IPF). Despite radiological similarity to IPF, RA-UIP typically shows greater inflammation. This study aimed to investigate the formation of cysts mimicking honeycombing radiologically in RA-UIP. We hypothesized that the pathogenesis of these cystic structures may differ from that of IPF. METHODS: We retrospectively analyzed patients with rheumatoid arthritis who had a UIP/probable UIP

Pulmonary arteriole narrowing in end-stage cystic fibrosis lungs occurs with and without small airway disease.

BACKGROUND: Pulmonary hypertension (PH) is an important, life-limiting co-morbidity in cystic fibrosis (CF), where multiple mechanisms such as hypoxia, inflammation and primary CF-transmembrane regulator (CFTR) dysfunction may affect vascular integrity. We aimed to characterize the structural impact of vascular wall changes in the pulmonary microcirculation, to uncover the potential need for therapeutic strategies targeting vascular disease. METHODS: End-stage inflated CF (n=6), and control (n=4) lungs were processed to lung cores (2.8cm³) and scanned with micro-computed tomography (resolutio

Increased pathogenicity and transmission of SARS-CoV-2 Omicron XBB.1.9 subvariants, including HK.3 and EG.5.1, relative to BA.2.

With the SARS-CoV-2 Omicron XBB.1.9 subvariants circulating worldwide, two XBB.1.9 variants, EG.5.1 and HK.3, spread rapidly and became dominant in mid-2023. However, the spike features, pathogenicity, and transmissibility of HK.3 are largely unknown. Here, we performed multiscale investigations to reveal the virological features of XBB.1.9 subvariants, including the newly emerging HK.3. HK.3 revealed high replication efficiency and enhanced TMPRSS2 utilization in vitro. The HK.3 spike exhibited enhanced processing, although its infectivity, fusogenicity, and human ACE2 (hACE2) binding affinit

Endobronchial Valve Insertion for the Management of Persistent Air Leak Following Pneumothorax.

Persistent air leak following a pneumothorax refers to air leakage lasting 5-7 days after the initial event. Different strategies have been used with varying degrees of success including surgical or chemical pleurodesis. Endobronchial valve (EBV) insertion is a technique where the insertion of a one-way valve obstructs the flow of air through the leak and helps in pneumothorax resolution especially if surgery is contraindicated. We present the case of a gentleman in his 60s who was admitted with a right-sided pneumothorax on the background of grade 4 glioblastoma. A 12-French chest drain was i

Influence of CT Image Matrix Size and Kernel Type on the Assessment of HRCT in Patients with SSC-ILD.

Background: Interstitial lung disease (ILD) is a frequent complication of systemic sclerosis (SSc), and its early detection and treatment may prevent deterioration of lung function. Different vendors have recently made larger image matrices available as a post-processing option for computed tomography (CT), which could facilitate the diagnosis of SSc-ILD. Therefore, the objective of this study was to assess the effect of matrix size on lung image quality in patients with SSc by comparing a 1024-pixel matrix to a standard 512-pixel matrix and applying different reconstruction kernels. Methods:

Effects of transport stress on pathological injury and main heat shock protein expression in the respiratory system of goats.

The aim of the present study was to investigate the pathological injury and the expression of heat shock proteins in the caprine lung, trachea and bronchus under transport stress. 12 healthy male goats were selected and randomly divided into three groups. The control group (non-transported group), 2 hr transport-treated group and 6 hr transport-treated group. Morphological changes as well as the expression of heat shock proteins (HSPs, mainly HSP27, HSP70 and HSP90) in three parts of the respiratory tract were examined. Our results showed swollen mucosa and congestive blood vessels in mucous l

Pleiotropic effects and pharmacological properties of penehyclidine hydrochloride.

BACKGROUND: Penehyclidine hydrochloride (PHC) is an anticholinergic drug manufactured in China. It is used widely in clinics as a reversal agent in cases of organic phosphorus poisoning and as a preanesthetic medication. Compared with other anticholinergic agents, PHC confers substantial advantages. Here, in this review, we focus on its important clinical effects for organic phosphorus poisoning, preanesthetic medication, and the protective effects on certain visceral organs. MATERIALS AND METHODS: Our bibliographic sources include the PubMed and China National Knowledge Infrastructure (CNKI)

Angiostrongylus spp. in the Americas: geographical and chronological distribution of definitive hosts versus disease reports.

BACKGROUND: Angiostrongyliasis is an infection caused by nematode worms of the genus Angiostrongylus. The adult worms inhabit the pulmonary arteries, heart, bronchioles of the lung, or mesenteric arteries of the caecum of definitive host. Of a total of 23 species of Angiostrongylus cited worldwide, only nine were registered in the American Continent. Two species, A. cantonensis and A. costaricensis, are considered zoonoses when the larvae accidentally parasitise man. OBJECTIVES: In the present study, geographical and chronological distribution of definitive hosts of Angiostrongylus in the Ame

Update on Mycoplasma hyopneumoniae infections in pigs: Knowledge gaps for improved disease control.

Mycoplasma hyopneumoniae (M. hyopneumoniae) is the primary pathogen of enzootic pneumonia, a chronic respiratory disease in pigs. Infections occur worldwide and cause major economic losses to the pig industry. The present paper reviews the current knowledge on M. hyopneumoniae infections, with emphasis on identification and analysis of knowledge gaps for optimizing control of the disease. Close contact between infected and susceptible pigs is the main route of M. hyopneumoniae transmission. Management and housing conditions predisposing for infection or disease are known, but further research

Interindividual variability in the expression of surfactant protein A and B in the human lung during development.

The surfactant complex, thanks to its multiple actions including decrease of surface- tension and antimicrobial activity, plays a fundamental role in newborn survival, lowering the risk of respiratory distress syndrome. The aim of this work was to determine if the synthesis of two surfactant proteins (SP), SPA and pro-SPB, shows some inter-individual variability during lung development in the intrauterine life. Immunoreactivity for SPA and pro-SPB was investigated in the lungs of  40 subjects, including 15 fetuses, ranging from 14 to 22 weeks of gestation, and 25 neonates, from 24 to 41 weeks.

Species of Angiostrongylus (Nematoda: Metastrongyloidea) in wildlife: A review.

Twenty-one species of Angiostrongylus plus Angiostrongylus sp. (Nematoda: Metastrongyloidea) are known currently in wildlife. These occur naturally in rodents, tupaiids, mephitids, mustelids, procyonids, felids, and canids, and aberrantly in a range of avian, marsupial and eutherian hosts including humans. Adults inhabit the pulmonary arteries and right atrium, ventricle and vena cava, bronchioles of the lung or arteries of the caecum and mesentery. All species pass first-stage larvae in the faeces of the host and all utilise slugs and/or aquatic or terrestrial snails as intermediate hosts. Ga

Observational study of sleep, respiratory mechanics and quality of life in patients with non-cystic fibrosis bronchiectasis: a protocol study.

INTRODUCTION: Bronchiectasis is a chronic disorder characterised by permanent and irreversible abnormal dilation of the bronchi and bronchioles, primarily caused by repeated cycles of pulmonary infections and inflammation, which lead to reduced mucociliary clearance and to the excessive production of sputum. Patients with non-cystic fibrosis bronchiectasis may be predisposed to hypoxemia during sleep, or to symptoms that may lead to arousals and thereby reduce the quality of life, because of the irreversible dilation of the bronchi and the presence of secretions and airflow obstruction.
