Implementation barriers, facilitators, and fidelity of a mobile money-based savings tool for pregnant women in the Analamanga region of Madagascar: an ex-post process evaluation.

BACKGROUND: Mobile payment services could improve access to obstetric care by easing saving, raising revenue, and pooling resources. Between 2019 and 2022, a mobile money-based savings tool for obstetric care was implemented in the Analamanga region of Madagascar following a randomized controlled trial design. We conducted an ex-post process evaluation of the intervention. METHODS: We assessed implementation fidelity and reach of the trial using five indicators from a population-based survey of 6,234 women who were pregnant during the intervention period and lived in catchment areas of 63 pub

Exploring multi-level determinants of implementation of nicotine replacement therapy guidelines for youth vaping: a mixed-methods case study.

BACKGROUND: Rates of youth vaping have become a major concern over the past decade with worries about impacts of vaping on neurodevelopment, mental health, transition to cigarettes, and unknown long-term physical sequelae. In 2023, the American Academy of Pediatrics (AAP)'s updated tobacco care guidelines encouraged primary care providers to consider using nicotine replacement therapy (NRT) products to help support youth vaping and tobacco cessation. This study sought to explore the determinants to using NRT products for youth vaping cessation per AAP recommendations amongst primary care provi

De-implementing inappropriate benzodiazepine prescribing in primary care: an overview of systematic reviews informed by behavioral frameworks.

BACKGROUND: The long-term use of benzodiazepines (BZDs) poses significant health risks, including cognitive impairment, falls, and dependency, despite guidelines recommending against prolonged use. Effective deprescribing interventions are essential, but evidence on optimal strategies in primary healthcare remains fragmented. OBJECTIVE: This overview synthesizes evidence from systematic reviews of interventions aimed at reducing inappropriate BZD prescribing in primary care. It identifies barriers and facilitators, assesses the use of behavioral theories, evaluates intervention effectiveness,

A co-creation approach to selecting and adapting adolescent tobacco and air-pollution interventions and strategies for implementation in diverse countries: insights from FRESHAIR4Life.

BACKGROUND: Evidence-based interventions to reduce adolescents' exposure to tobacco and air pollution are rarely implemented in low- and middle-income countries, often because they lack contextual fit. The 'FRESHAIR4Life' project aimed to optimise implementation of contextually adapted, evidence-based interventions in Greece, Kyrgyzstan, Pakistan, Romania, and Uganda. Selecting appropriate interventions required deliberate, inclusive, and context-sensitive decision-making. This paper outlines the co-creation approach for selecting and adapting interventions, developing implementation strategie

Exploring spillover effects following surgical de-implementation: an observational evaluation of primary care, referrals, and time to surgical intervention following reductions in the use of tonsillectomy and Dupuytren's contracture.

BACKGROUND: Reducing the use of low-value surgery is important to maintain effective, safe and financially sustainable health systems. However, following de-implementation there are inevitably wider impacts for health systems beyond the targeted reduction in surgery, which have hitherto not been explored. Here we describe the spillover effects following the reduction in the use of two surgical procedures; tonsillectomy and Dupuytren's contracture release (DCR) identified in 2019 by the Evidence Based Interventions (EBI) programme a de-implementation initiative in England. METHODS: This longit

Difference-making factors in implementing a quality improvement program for sleep apnea in stroke/TIA patients.

BACKGROUND: Effective prevention of ischemic stroke and transient ischemic attack (TIA) involves timely, guideline-concordant risk factor management. Obstructive sleep apnea (OSA), a significant but underdiagnosed cerebrovascular risk factor, affects approximately 70% of stroke and TIA patients. Untreated OSA is linked to impaired post-stroke recovery, recurrent vascular events, and increased mortality. Despite guideline recommendations to consider early post-stroke/TIA OSA screening, few patients receive sleep studies. This study explores the implementation of a multidisciplinary quality impr

Delivering effective non-invasive ventilation in amyotrophic lateral sclerosis using intensive remote support (DENIM): protocol for an embedded process evaluation in a hybrid type 3 implementation-effectiveness trial.

BACKGROUND: Non-invasive ventilation (NIV) is the only intervention that significantly improves survival and quality of life in motor neuron disease, extending life by 8-13 months. However, at least half of patients are unable to reach the recommended ≥ 4 h daily NIV use, and current NHS services provide insufficient follow-up for intensive optimisation. Delivering Effective Non-Invasive ventilation in Motor neuron disease using intensive remote support (DENIM) is a stepped-wedge cluster randomised trial. This protocol describes a process evaluation embedded within DENIM aiming to understand h

Implementation and evaluation of a psychological first aid program to manage post-traumatic stress injuries among Canadian police officers.

BACKGROUND: Police officers are exposed to potentially psychological traumatic events and are at high risk of developing post-traumatic stress injuries (PTSI). Development and wide implementation of best practices for managing PTSI are needed. The psychological first aid (PFA) framework encompasses trauma-informed knowledge to guide the development of best practices. Based on the framework and on a pilot performed among Canadian police officers, we propose a PFA program including: 1) PTSI awareness e-learning; 2) PFA training provided by local trainers; and, 3) peer-to-peer PFA intervention to

Integrating HIV prevention services into care settings for people with opioid use disorder (OUD): a study protocol for implementation strategy development and modeling.

BACKGROUND: The overlapping epidemics of opioid use disorder (OUD) and HIV present a critical public health challenge. Although people with OUD frequently engage with healthcare settings, uptake of HIV prevention services such as pre-exposure prophylaxis (PrEP) remains low. Integrating HIV prevention into routine OUD care could reduce new infections, but scalable, evidence-based strategies are lacking. Rhode Island offers a unique opportunity to design and evaluate such strategies using its robust data infrastructure and high OUD burden. METHODS: We will conduct a three-phase, sequential impl

Achieving health-promotion practice in primary care using a multifaceted implementation strategy: a non-randomized parallel group study.

BACKGROUND: Evidence-based healthcare recommendations exist for tobacco use, harmful alcohol consumption, low physical activity, and poor diet. However, the uptake of these recommendations in Swedish primary healthcare is poor, and the potential benefits for patients are not fully realized. Our aim was to evaluate the effect (i.e. the uptake) of a 12-month multifaceted implementation strategy to achieve a more health-promoting practice. We hypothesized that primary healthcare centers receiving this strategy would increase and sustain their health-promotion practices to a significantly greater

Methods for community-engaged data collection and analysis in implementation research.

BACKGROUND: Community engagement is widely recognized as critical to successful and equitable implementation of evidence-based practices, programs, and policies. However, there are no clear guidelines for community involvement in data collection and analysis in implementation research. METHODS: We describe three specific methods for engaging community members in data collection and analysis: concept mapping, rapid ethnographic assessment, and Photovoice. Common elements are identified from a case study of each method: 1) selection and adaptation of evidence-based strategies for improving adol

Longitudinal evaluation examining implementation and sustainment of an opioid overdose education and naloxone distribution among veterans who are unstably housed.

BACKGROUND: Rigorous implementation evaluations are needed to understand factors that influence implementation and sustainability of evidence-based interventions across contexts. In this study, we conducted a longitudinal, multi-methods, multi-site evaluation guided by the Dynamic Sustainability Framework (DSF). This evaluation focused on implementation of the Homeless Overdose Prevention-Expansion (HOPE), an opioid overdose education and naloxone distribution (OEND) trial in a permanent supportive housing program in the Veterans Health Administration (VA). METHODS: We used a multi-methods st

The peer paradox: Perspectives from HIV service providers to inform the implementation of an enhanced social network strategy for black sexual and gender minorities-a qualitative study.

BACKGROUND: The CDC's Social Network Strategy (SNS) is an evidence-supported approach to increase reach for HIV testing among members of marginalized populations. Leveraging social networks could improve access to HIV services, like pre-exposure prophylaxis and antiretroviral therapy, particularly for members of Black sexual and gender minority (BSGM) groups. We explored key barriers and facilitators prior to implementing an enhanced SNS (eSNS) to increase access to a broader range of HIV prevention and treatment services among BSGM in an Ending the HIV Epidemic jurisdiction in the US South.

Why engagement matters in policy implementation: an examination of the engagement of policy actors in the implementation of a mental health and addictions strategy.

BACKGROUND: Shifts toward "new public governance" (NPG), where policy decisions and their implementation are "co-produced" by a policy network, have captured the attention of policymakers as an approach that may produce better outcomes. It is particularly promising in mental health, where it is increasingly acknowledged that effective change requires actions by multiple actors across a range of policy and system settings. In Ontario, Canada, the government's recent mental health reform, Open Minds, Healthy Minds, Ontario's Comprehensive Mental Health and Addictions Strategy, is unique in terms

Stakeholder engagement to co-design implementation strategies for integrating depression management into HIV care services in Senegal.

BACKGROUND: Depression is highly prevalent in people living with HIV (PWH), affecting their daily life and HIV outcomes. Following a successful pilot study to treat depression in PWH with Group Interpersonal Therapy, we examined its implementation potential. Despite a strong willingness for its adoption routine practice, formal integration of mental health services into HIV care remained challenging. Using Implementation Mapping, we aimed to select and specify a set of implementation strategies to integrate depression services into Senegalese HIV care. METHODS: For each step of depression ser

Normalizing daily awakening and breathing coordination at 15 heterogenous ICUs: a multicenter post-implementation survey.

BACKGROUND: Despite high post-implementation adherence, clinicians may have unresolved questions or concerns regarding use of a protocol to standardize routine daily coordination of the spontaneous awakening trial (SAT) and spontaneous breathing trial (SBT) on ventilated patients. Unresolved questions or concerns may unwittingly curtail practice normalization, impacting practice sustainment when implementation support is withdrawn. The objective of this study was to identify unresolved questions or concerns that may persist following successful implementation of a coordinated SAT/SBT (C-SAT/SB

Time required to implement a computerized patient decision aid for lupus in outpatient visits.

BACKGROUND: Patient decision aids have the potential to lower decision conflict for patients and to improve patient-physician communication. However, uptake of decision aids has been poor, in part because the time required to incorporate these into clinical practice is not well understood. OBJECTIVE: To estimate the time required for a rheumatology clinic to implement a validated decision aid for patients with lupus. METHODS: Using a cohort of eight implementation sites, study investigators identified the activities required to administer a decision aid. Site coordinators embedded within the

Can a tailored implementation programme enhance the adoption of guideline-adherent behaviour in physiotherapists and chiropractors managing patients with low back pain? An implementation study.

BACKGROUND: Low back pain (LBP) is the primary contributor to disability worldwide, leading to a significant healthcare burden. Implementing evidence-based practice (EBP) can reduce this burden, as healthcare professionals (HCPs) working evidence-based reduce the number of treatments, the use of imaging and medication compared to HCPs not working evidence-based. Clinical practice guidelines have been published to help HCPs implement research evidence into practice. Unfortunately, studies have consistently shown a lack of adherence to guidelines in LBP care. This study aimed to investigate the

Describing the determinants of fluid prescribing and fluid balance optimization in the pediatric intensive care unit: a qualitative study at a community hospital.

BACKGROUND: Critically ill children tend to receive fluid volumes exceeding physiologic requirements despite evidence demonstrating harm with increasing net positive fluid balance. However, interventions aimed at optimizing fluid balance have yet to demonstrate significant clinical benefit, likely because there are multiple drivers of this complex problem. In this study, we used qualitative inquiry to describe the current practice at a community pediatric intensive care unit and discover potential barriers and facilitators to clinical practice change. METHODS: We designed a semi-structured in
