Combined spinal/caudal catheter anesthesia: extending the boundaries of regional anesthesia for complex pediatric urological surgery.

BACKGROUND: Spinal anesthesia (SA) is an established anesthetic technique for short outpatient pediatric urological cases. To avoid general anesthesia (GA) and expand regional anesthetics to longer and more complex pediatric surgeries, the authors began a program using a combined spinal/caudal catheter (SCC) technique. STUDY DESIGN: The authors retrospectively reviewed the charts of all patients scheduled for surgery under SCC between December 2016 and April 2018 and recorded age, gender, diagnosis, procedure, conversion to GA/airway intervention, operative time, neuraxial and intravenous med

The whole truth: comparative analysis of diaphragmatic hernia repair using 4-ply vs 8-ply small intestinal submucosa in a growing animal model.

BACKGROUND: Diaphragmatic reconstruction remains a challenging problem. There is limited information concerning the use of small intestinal submucosa (SIS) in congenital diaphragmatic hernia repair. A canine model was used to evaluate the use of a SIS patch in diaphragmatic reconstruction. METHODS: Eleven beagle puppies (1.6-4.2 kg, 8 weeks old) underwent left subcostal laparotomy, central left hemidiaphragm excision (2 x 7 cm, 50% loss), and reconstruction with a 4-ply group I (n = 5) or 8-ply group II (n = 6) SIS patch. Chest radiographs were taken at time of operation and 3 and 6 months po

Diverting the paralyzed larynx: a reversible procedure for intractable aspiration.

Some unfortunate patients suffer disorders which in one way or another, usually neurologically, severely restrict the larynx in its role as a protector of the lower airway. Aspiration ensues, and unless it can be successfully managed, repeated episodes of a violent pneumonitis may lead to terminal chest problems. In some patients, even the cuffed tracheostomy tubes of new and improved design do not adequately prevent aspiration over an extended time intervel, as evidenced by repeated bouts of aspiration pneumonia despite these cuffed tubes and despite optimal intensive care. For these patients

Thermal stress induces dual damages to physiology and reproductive capacity of Galaxea fascicularis during its reproductive stage.

Driven by ocean warming and intensifying marine heatwaves, increasingly frequent and severe coral bleaching events are causing declines in c … eductions in oocyte size and abundance. Downregulated genes were significantly enriched in the base excision repair and mRNA surveillance pathways, which may predict the emergence of irreversible cellular damage. Findings of this study advance our understanding of how broadcast-spawning G. fascicularis responds to varying degrees of thermal stress, and highlight …

A study on the antitumor effect and mechanism of arsenic trioxide on lung adenocarcinoma.

BACKGROUND: Lung adenocarcinoma is a common malignancy that requires new treatments. Arsenic trioxide (ATO) exerts antitumor activity in var … egulated genes, with enrichment in the mammalian target of rapamycin (mTOR) signaling pathway, base excision repair (BER), and apoptosis pathways. At the protein level, total mTOR expression did not significantly change, whereas phosphorylated mTOR (p-mTOR), APE-1, Bcl-2, and Bcl-XL were downregulated, and cleaved caspase-8 and cleaved caspase-9 were upregulated …

Quality of life before and after redo IPAA: does pouch salvage improve quality of life?

BACKGROUND: Quality of life (QoL) and functional outcomes after redo ileoanal pouch (IPAA) surgery are worse than after primary (index) pouch surgery. However, QoL in failing pouches compared to after redo IPAA has not been reported. We hypothesized that QoL after redo IPAA would be improved compared with failing IPAAs immediately prior to redo surgery. METHODS: Adults who underwent redo IPAA (1984-2024), had successful loop ileostomy closure, and at least one pouch survey were included. Our primary outcome was the Cleveland Global Quality of Life Index (CGQLI; range, 0 [lowest] to 1 [highest

Epigenetic and Epitranscriptomic Modulation by STAT1: Unraveling T Helper Cell Differentiation in NSCLC.

The lack of targetable mutations in 50% of NSCLC cases and resistance to therapies underscore the need for alternative treatments, with epig … ility from R-loops-DNA-RNA hybrids that lead to DNA breaks through transcription-coupled nucleotide excision repair. Our study examines epigenetic regulators in CD4+ T helper cells from NSCLC patients, focusing on the effects of STAT1 depletion and overexpression on R-loop formation at key gene loci specific to T helper cells. Depletion of STAT1 increased R-loop …

Somatic mutations in human ontogenesis and their impact on health.

Somatic mosaicism originates from postzygotic errors and creates a unique genomic landscape in every cell and individual. The health impacts of somatic mutations range from neutral to severe, responsible for most cancers and contributing to neurodevelopmental, neurodegenerative and other multifactorial diseases. The molecular mechanisms driving this variability are diverse, dominated by replication errors of DNA polymerase ε in CpG contexts and by spontaneous cytosine deamination followed by error-prone base excision repair. Duplex and single-cell sequencing now permit accurate quantification …

Indocyanine green for intraoperative ureteral identification in gynecologic surgery: A preliminary systematic review of feasibility, safety, and surgical outcomes.

BACKGROUND: Intraoperative ureteral injury, although uncommon, can result in significant morbidity and medicolegal consequences, particularly during complex gynecologic procedures such as endometriosis excision and prolapse repair. Conventional ureteral identification techniques, including stents and methylene blue, have recognized limitations. Indocyanine green (ICG) with near-infrared fluorescence (NIRF) imaging enables real-time ureter visualization, but clinical evidence remains scattered. OBJECTIVE: To systematically evaluate the feasibility, safety, and outcomes of intraureteral ICG for
