Factors associated with outcomes after reverse total shoulder arthroplasty at midterm follow-up.

BACKGROUND: Reverse total shoulder arthroplasty has become the preferred treatment for many patients with irreparable cuff tears, osteoarthritis, and complex proximal humeral fractures. While outcomes are generally favorable, the influence of demographic and surgical factors-such as age, sex, previous surgery, and dominance of the operated arm-on post-operative results remains incompletely understood. METHODS: After institutional review board approval, a retrospective review of prospectively collected data was conducted for all primary reverse total shoulder arthroplasties performed by a sing…

Anatomic and reverse total shoulder arthroplasty for osteoarthritis: outcomes in patients 80 years old and older.

BACKGROUND: There has been an increase in both primary anatomic (aTSA) and reverse total shoulder arthroplasty (rTSA) over the last decade, with rates peaking for patients aged 75 years and older. Despite aTSA being the mainstay of treatment for patients with glenohumeral arthritis in the absence of rotator cuff insufficiency, there has been an upward trend of rTSA utilization in the elderly due to concerns about rotator cuff integrity, regardless of deformity. The purpose of this study is to evaluate outcomes including pain, function, range of motion, satisfaction, and complications in patien

Safety of Arthroscopic Versus Open or Combined Heterotopic Ossification Removal Around the Elbow.

PURPOSE: To analyze the complications of arthroscopic heterotopic ossification (HO) excision and compare them with those of open removal of HO or a combined open-arthroscopic approach. METHODS: We performed a retrospective review of elbow HO removal cases performed by a single surgeon from 1997 to 2014. In all cases studied, the intention was to restore range of motion owing to the presence of HO causing functional impairment. The arthroscopic, open, and combined treatment groups were compared. RESULTS: The study cohort consisted of 223 surgical procedures performed on 213 elbows in 211 pati

Traumatic Shoulder Dislocation Among Adolescents: Hill-Sachs Lesion Volume and Recurrent Instability.

BACKGROUND: Accurate assessment of Hill-Sachs lesions (HSLs) and their relationship to the glenoid track is essential to optimizing management following traumatic shoulder dislocation. The purpose of this study was to measure the size of HSLs by a novel method with magnetic resonance (MR) arthrography and to investigate the relationship between lesion size and instability history in adolescents (below 19 y of age) with a history of traumatic dislocation. METHODS: All shoulder MR arthrograms queried to identify HSLs over a 4-year period were retrospectively reviewed and independently evaluated

Complex limb trauma requiring revascularization: early multidisciplinary management.

Salvage of limbs with combined vascular, skeletal and extensive soft tissue injury requires the skills of several surgical specialities. Four illustrative cases are presented, three successes and one failure, and from these some conclusions are drawn. Contrary to accepted teaching, it is believed that in the emergency management of most patients with complex limb trauma, vascular reconstruction should take priority, followed by skeletal stabilization, soft tissue repair and early flap cover.

Fractures of the distal humerus operated on using the Lambda® plate: report of 75 cases at 9.5 years follow-up.

INTRODUCTION: The study sought to assess the functional results and complications of osteosynthesis using the Lambda(®) plate (Zimmer, Étupes, France) in treating fracture of the distal extremity of the humerus. MATERIALS AND METHODS: The initial series comprised 115 patients (116 fractures), treated between 1992 and 2008. Forty-one (mainly foreigners) were lost to follow-up. The final series thus comprised 74 patients (75 fractures): 44 female and 30 male, with a mean age of 46±23 years (range, 16-95 years), 22 (29%) being aged 65 years or over. According to AO classification, 22 of the frac

Long-term denosumab treatment restores cortical bone loss and reduces fracture risk at the forearm and humerus: analyses from the FREEDOM Extension cross-over group.

UNLABELLED: Upper limb fractures (including wrist, forearm, and humerus) represent a significant burden among postmenopausal women with osteoporosis. Up to 7 years of treatment with denosumab resulted in an increase in bone mineral density and decrease in fractures in upper limb sites. INTRODUCTION: Upper limb (wrist, forearm, and humerus) fractures are a significant burden in osteoporosis, associated with significant morbidity and mortality. Denosumab, a monoclonal antibody against RANK ligand, increases bone mineral density (BMD) and decreases vertebral, nonvertebral, and hip fractures. Her

A multicenter assessment of 1,177 cases of shoulder dystocia: lessons learned.

The purposes of this review were to describe deliveries complicated by shoulder dystocia (SD) at three tertiary centers and discern the differences between SD with and without brachial plexus injury (BPI). The inclusion criteria for this multicenter, retrospective study were singletons, delivered vaginally with SD. To discern the risk factors for SD with and without injury, a case (SD and BPI) versus control (3 SD without injury at the same institution) design was used. Multiple linear regression was employed. Over a 7-year period, among 46,637 vaginal deliveries, SD occurred in 1,177 cases (2

Comparison of anterior and lateral approaches in the treatment of extension-type supracondylar humerus fractures in children.

Eighty-four patients who underwent open reduction and Kirschner wire (K-wire) fixation for supracondylar humerus fractures through anterior or lateral approach with or without additional medial incisions were compared with regard to complications and end results. A total of 46 patients were operated through the anterior and 38 through the lateral approach. In lateral approach cases, medial incision was added only in those patients in whom the medial condyle and therefore the ulnar nerve were not easily distinguished due to excessive oedema. All the fractures were Gartland type III extension fr

Skeletal traction and delayed percutaneous fixation of complicated supracondylar humerus fractures due to delayed or unsuccessful reductions and extensive swelling in children.

The functional and radiological results of 13 supracondylar humerus fractures of 13 children were evaluated to determine the efficiency of delayed percutaneous fixation of the fractures after a period of skeletal overhead traction. All the fractures were complicated by delayed reduction, extensive swelling or unsuccessful reduction manipulations. As the unfavourable preexisting factors subsided under skeletal traction, anatomical reduction and fixation of fractures by percutaneous pinning were performed under general anesthesia. The average follow-up period was 21 months. Bauman and lateral ca

Flexion-type supracondylar elbow fractures in children.

Flexion-type supracondylar elbow fractures are uncommon in children. Minimally displaced fractures were treated in plaster cast in 7 children, and 22 children with a displaced fracture were treated by closed reduction and percutaneous pinning. Results after an average follow-up of 6.3 years showed that the treatment of choice yielded excellent or good results in 86.2% of the patients. All patients were satisfied with the end result and had normal use of their elbow.

Ischaemia and the pink, pulseless hand complicating supracondylar fractures of the humerus in childhood: long-term follow-up.

A series of 26 children was referred to our specialist unit with a 'pink pulseless hand' following a supracondylar fracture of the distal humerus after a mean period of three months (4 days to 12 months) except for one referred after almost three years. They were followed up for a mean of 15.5 years (4 to 26). The neurovascular injuries and resulting impairment in function and salvage procedures were recorded. The mean age at presentation was 8.6 years (2 to 12). There were eight girls and 18 boys. Only four of the 26 patients had undergone immediate surgical exploration before referral and th

Reverse V osteotomy of the distal humerus for the correction of cubitus varus.

We reviewed 22 children with cubitus varus who had been treated by a reverse V osteotomy and fixation by cross-pinning and wiring. The mean pre-operative humeral-elbow-wrist angle was -16.9 degrees (-25 degrees to +9 degrees ) and at the latest follow-up it was +7.3 degrees (-2 degrees to +14 degrees ). No child had a lateral prominence greater than 5 mm after correction. An excellent result was achieved in 20 children and a good result in two. We believe that this osteotomy has the advantages of better inherent stability, the avoidance of a prominent lateral condyle after correction and firm

A retrospective review of long anterior fusions to the sacrum.

BACKGROUND CONTEXT: In the setting of tumor, infection, or trauma, a corpectomy of the L5 vertebral body may be necessary. However, the space has an irregular trapezoidal shape, and the failure to account for this may lead to improper fitting of the titanium cages or the allograft struts when performing a reconstruction. PURPOSE: The purpose of this study was to evaluate the failure rate of implants used to reconstruct the anterior lumbar spine when an L5 corpectomy has been performed. METHODS: A retrospective review of the medical records and radiographs of 19 consecutive patients undergoin
