Cephalomedullary nail fixation of subtrochanteric femur fractures in older adults: Is 1 distal locking screw enough?

BACKGROUND: Subtrochanteric (ST) fractures are proximal femur fractures with strong deforming muscular forces, which complicate anatomical reduction and treatment. These fractures are fixed with intramedullary nails with 2 distal locking screws, although the need for both screws has never been researched. PURPOSE: This study's purpose was to investigate the use of 1 versus 2 distal locking screws on clinical and radiographic outcomes and complications. STUDY DESIGN/LEVEL OF EVIDENCE: Retrospective cohort, Level of Evidence III. METHODS: An IRB-approved retrospective review of a consecutive

Does an antibiotic nail made in a lubricated tube release antibiotic?

INTRODUCTION: The treatment of long-bone osteomyelitis has long been a difficult problem. Recently, antibiotic-impregnated intramedullary rods for the treatment of infected long-bone fractures have been gaining popularity but they are quite difficult to fabricate. Recently, a new technique that utilizes mineral oil to coat the inside of a chest tube mold prior to introduction of cement has been proven to ease fabrication. We hypothesized that the use of mineral oil would alter the elution characteristics of tobramycin from the intramedullary device. METHODS: Two groups of antibiotic nails wer

Addressing hindfoot arthritis with concomitant tibial malunion or nonunion with retrograde tibiotalocalcaneal nailing: a novel treatment approach.

BACKGROUND: Tibial malunions and nonunions are associated with degenerative changes about the ankle. A comprehensive literature review revealed no articles discussing treatment options for patients with tibial shaft malunion and ipsilateral ankle arthritis. The aim of our study was to evaluate a series of patients in whom tibial osteotomy and retrograde tibiotalocalcaneal nailing were used to treat both tibial deformity and ankle osteoarthritis. METHODS: Twenty-five patients underwent retrograde tibiotalocalcaneal nailing with concomitant realignment tibial osteotomy with takedown of the nonu

Intramedullary nailing of subtrochanteric fractures--does malreduction matter?

INTRODUCTION: Subtrochanteric femur fractures remain challenging injuries to treat. Historically, varus malreduction has been linked to the development of nonunion; however, there is a paucity of literature evaluating the impact of sagittal plane malreduction. The purpose of this study was to evaluate the influence of coronal and sagittal plane malreductions on time to union of subtrochanteric femur fractures treated with an intramedullary device. METHODS: A retrospective study was performed of all sub-trochanteric fractures (AO/OTA type 32) treated at a single institution. Inclusion criteria

Intramedullary nailing for open fractures of the femoral shaft: evaluation of contributing factors on deep infection and nonunion using multivariate analysis.

The purpose of this study was to use multivariate analysis to evaluate contributing factors affecting deep infection and nonunion of open femoral fractures treated with locked intramedullary nailing (IMN). We examined 89 open femoral fractures (88 patients) treated with immediate or delayed locked IMN in static fashion at the Kitasato University Hospital from 1988 to 2001. Multiple regression models were derived to determine predictors of deep infection and nonunion. The following predictive variables of deep infection were selected for analysis: age, sex, Gustilo type (I+II or III), fracture

The use of knee mega-prosthesis for the management of distal femoral fractures: A systematic review.

INTRODUCTION: Distal femur fractures (DFFs) are unusual and difficult to deal, especially in elderly patients. A consensus about a gold-standard treatment has not been reached yet. Available options include both conservative and surgical management. In elderly patients a prosthetic replacement could be a valid treatment option. Literature is lacking about the use of mega-prosthesis in this type of fractures. The purpose of the present systematic review is to examine which fracture, both acute and chronic, involving distal femur should be treated by using a mega-prosthesis. MATERIALS AND METHO

Impact of Preexisting Dementia on Mortality, Readmissions, and Complications After Surgical Repair of Hip Fractures: A Cohort Analysis.

OBJECTIVES: As the population ages, hip fractures are projected to increase. These fractures carry high morbidity and mortality in older adults. Dementia, common among these patients, may further worsen outcomes. This study evaluates the impact of preexisting dementia on mortality after surgical repair of femoral neck and intertrochanteric fractures, with the study hypothesis that the presence of preoperative dementia can result in increased mortality postoperatively. METHODS: Using the TriNetX Research Network, 68,639 patients aged 65 to 90 years who underwent surgical treatment of femoral n

hUC-MSCs via β-NGF Alleviate Cognitive Impairment After Tibial Fracture Surgery by Regulating the STMN2/NMNAT2-SARM1-NF-κB Signaling Pathway.

BACKGROUND: Perioperative neurocognitive disorders (PND) are common postoperative complications, particularly in elderly patients, marked by learning and memory deficits with limited treatment options. Human umbilical cord mesenchymal stem cells (hUC-MSCs) hold promise due to their neuroprotective and immunomodulatory effects, but their underlying mechanisms remain unclear. METHODS: This study established a PND model in aged mice using tibial fracture intramedullary fixation surgery, followed by intravenous hUC-MSCs administration. Subsequently, behavioral tests, pathological examination, pro

May-Thurner Syndrome Presenting as Extensive Left Iliofemoral Deep-vein Thrombosis with Inferior Vena Cava Extension Following Lower Limb Fracture Fixation: A Computed Tomography Venography.

May-Thurner syndrome is an anatomical condition in which the left common iliac vein is compressed between the right common iliac artery anteriorly and the lumbar spine posteriorly, predisposing to predominantly left-sided deep-vein thrombosis (DVT). It is frequently subclinical and typically becomes evident only at the time of a thrombotic event, often with a disproportionately extensive thrombus burden. We report the case of a 32-year-old male who developed acute swelling and pitting edema of the left lower limb approximately 18 days after intramedullary nail fixation of a left femoral shaft

Novel Allograft in the Load-Bearing Portion of the Femoral Head.

Background: An osteochondral defect in the hip can be a painful and limiting pathologic process. The damaged joint may progress into premature osteoarthritis, further limiting a patient's functionality. Case Report: A 24-year-old male presented to the clinic with left hip pain. The patient had been involved in a motor vehicle accident 3 years prior to presentation to our clinic. His injury from the high-speed accident required intramedullary rod fixation for a right-sided (contralateral) subtrochanteric femur fracture. The patient complained of left groin pain when in a sitting position, with

Treatments for the Fifth Metacarpal Neck Fractures: A Network Meta-analysis of Randomized Controlled Trials.

The fifth metacarpal neck fractures (commonly termed boxer's fractures) are the most common type of metacarpal fractures. Many types of treatments are available in clinical practice, some of which have already been compared with other treatments by various researchers. However, a comprehensive treatment comparison is lacking. We estimated the comparative efficacy of different interventions for total complications, through a network meta-analysis of randomized controlled trials. We conducted a systematic search of the literature through October 2015. The outcome measurements were the total comp

The biomechanical effect of torsion on humeral shaft repair techniques for completed pathological fractures.

In the presence of a tumor defect, completed humeral shaft fractures continue to be a major surgical challenge since there is no "gold standard" treatment. This is due, in part, to the fact that only one prior biomechanical study exists on the matter, but which only compared 2 repair methods. The current authors measured the humeral torsional performance of 5 fixation constructs for completed pathological fractures. In 40 artificial humeri, a 2-cm hemi-cylindrical cortical defect with a transverse fracture was created in the lateral cortex. Specimens were divided into 5 different constructs an

Optimizing surgery of metaphyseal-diaphyseal fractures of the fifth metatarsal: a cadaveric study on implications of intramedullary screw position, screw parameters and surrounding anatomic structures.

AIMS: Many advocate screw fixation of fractures to the metaphyseal-diaphyseal junction of the fifth metatarsal base, better known as Jones fractures (JF), to facilitate quicker ambulation and return to sport. Maximizing screw parameters based on fifth metatarsal (MT5) anatomy, alongside understanding the anatomic structures compromised by screw insertion, may optimize surgical outcomes. This study aims to (1) correlate the proximity of JF to the peroneus brevis (PB) and plantar fascia (PF) footprints and (2) quantify optimal screw parameters given MT5 anatomy. MATERIALS AND METHODS: 3D CT-sca

Can the use of an evidence-based algorithm for the treatment of intertrochanteric fractures of the hip maintain quality at a reduced cost?

In March 2012, an algorithm for the treatment of intertrochanteric fractures of the hip was introduced in our academic department of Orthopaedic Surgery. It included the use of specified implants for particular patterns of fracture. In this cohort study, 102 consecutive patients presenting with an intertrochanteric fracture were followed prospectively (post-algorithm group). Another 117 consecutive patients who had been treated immediately prior to the implementation of the algorithm were identified retrospectively as a control group (pre-algorithm group). The total cost of the implants prior
