COOH-terminal propeptides of the major human procollagens. Structural, functional and genetic comparisons.

The sequences of the carboxy-terminal extensions (COOH-propeptides) of at least one chain of all of the major human procollagens have only r … ure analyses. These results, combined with similar analyses of abnormal alpha 2(I) COOH-propeptide (osteogenesis imperfecta) permitted the identification of subsequences that are likely to be a prerequisite for COOH-propeptide functions, namely procollagen chain recognition and nucleation sites for triple helix formation. These functions are also common to the alpha 1(IV …

Fibrogenesis Imperfecta Ossium.

Fibrogenesis imperfecta ossium (FIO) is an extremely uncommon fatal bone disorder of poorly understood etiology. The pathogenesis of FIO is not well known. The fundamental skeletal defect appears to be an abnormality in organic matrix of bone characterized by defective mineralization of the abnormal collagen. FIO clinically manifests in middle-aged adults presenting with fracture and bone pain. Elevated serum alkaline phosphatase is the only and the most consistent biochemical abnormality. Although paraproteinemia is observed in one-third of cases, the pathogenic link to the disease process is

TMEM16E (GDD1) exhibits protein instability and distinct characteristics in chloride channel/pore forming ability.

TMEM16E/GDD1 has been shown to be responsible for the bone-related late-onset disease gnathodiaphyseal dysplasia (GDD), with the dominant allele (TMEM16E(gdd) ) encoding a missense mutation at Cys356. Additionally, several recessive loss-of-function alleles of TMEM16E also cause late-onset limb girdle muscular dystrophy. In this study, we found that TMEM16E was rapidly degraded via the proteasome pathway, which was rescued by inhibition of the PI3K pathway and by the chemical chaperone, sodium butyrate. Moreover, TMEM16E(gdd) exhibited lower stability than TMEM16E, but showed similar propensit

Hyperosteoidosis and hypermineralization in the same bone: bone tissue analyses in a boy with a homozygous BMP1 mutation.

Recently, homozygous mutations in BMP1 were identified as a cause of bone fragility in children with high areal bone mineral density. We examined iliac bone tissue from a 12-year-old boy with a homozygous mutation that leads to a p.Gly12Arg change in the signal peptide of BMP1, an enzyme that cleaves C-propeptide off the procollagen type I molecule. Histomorphometric analyses revealed marked hyperosteoidosis, with osteoid volume per bone volume at approximately 11 SD above the mean value for controls. At the same time, quantitative backscattered electron imaging showed drastic hypermineralizat

Critical comparison of two methods for the determination of nanomechanical properties of a material: application to synthetic and natural biomaterials.

Two methods used for determining the elastic modulus (E) and hardness (H) of a material--the original version of the well-known Oliver-Pharr Method, OOPM, and a variant of it called the Modified Slopes Method, MSM--were critically compared. The nanoindentation test results, of indenter load-versus-indenter displacement, were recorded for six series of specimens, three of commercially-available acrylic bone cements (Palacos R and Cemex XL) and three of bones (human, bovine, and mouse). In the first series, the specimens were prepared from Palacos R cement mantles retrieved from cemented total h

Craniocervical instability treated by contoured loop fixation.

Rigid posterior fixation of the skull to the third, fourth and fifth cervical vertebrae was achieved in three patients who, as a result of operation, had gross instability of the craniocervical junction. An anatomically contoured steel loop was secured to the occiput via small burr holes and to the vertebrae by sublaminar wiring. This technique has the advantage over bone grafting, either alone or with cement, in that it affords rigid stabilisation, allows early mobilisation and may contribute to eventual bony fusion.

Short-term side effects of first bisphosphonate infusion in children with different underlying bone pathologies.

BACKGROUND: Intravenous bisphosphonates are the primary pharmacological intervention for both primary and secondary paediatric osteoporosis. Following the successful use of bisphosphonates in children with osteogenesis imperfecta (OI), indications have expanded to a broad range of bone pathologies with variable underlying pathophysiology. While bisphosphonate use appears to be safe in children, data on risk factors for acute-phase reaction remain sparse. A prospective characterisation of adverse effects - including baseline musculoskeletal symptoms - has not previously been performed in childr…

Prevalence of Monogenic Bone Disorders in a Dutch Cohort of Atypical Femur Fracture Patients.

Atypical femur fractures (AFFs), considered rare associations of bisphosphonates, have also been reported in patients with monogenic bone di … The findings indicate a strong relationship between AFFs and monogenic bone disorders, particularly osteogenesis imperfecta and hypophosphatasia, but mainly in individuals with symptoms of these disorders. The high yield of (likely) pathogenic variants in AFF patients with a clinical suspicion of these disorders stresses the importance of careful clinical evaluation of A …

Pesamosca osteoplasty: surgical procedure for the spatial correction of cubitus varus or valgus post malunited supracondylar fractures of the humerus.

INTRODUCTION: Supracondylar fractures of the humerus represent a current concern in the child's and adolescent's osteo-articular pathology. … deformities were admitted in the study. The malunion due to the pathologic fractures encountered in osteogenesis imperfecta or fibrous dysplasia was precluded. The experience accumulated with the other surgical techniques used in 68 out of 96 patients (70.9%) determined us to exclusively use the Pesamosca osteoplasty following the year 2005, seeing the simplicity and the …
