Intravenous regional limb perfusion techniques in the anesthetized foal.

OBJECTIVE: To provide a video tutorial on how to perform IV regional limb perfusion (IVRLP) using the cephalic or saphenous vein in anesthetized foals. ANIMALS: Foals from neonatal to weaning age not amenable to standing, sedated procedures. METHODS: A pneumatic or wide-rubber tourniquet is applied around the proximal antebrachium or gaskin. The IVRLP dose of desired antimicrobial is diluted with physiologic saline to 30 mL of total volume. A 23-gauge winged infusion set or 22-gauge, 1-inch IV catheter is inserted into either the cephalic or saphenous vein distal to the tourniquet, and the e

Noninvasive imaging to confirm the technical outcomes of left atrial appendage exclusion following surgical closure.

BACKGROUND: Closure of the left atrial appendage in patients with atrial fibrillation significantly decreases the rate of future complications. Exclusion success is typically evaluated via transesophageal echocardiography, an invasive procedure typically requiring patient sedation. We investigated left atrial appendage (LAA) exclusion rates postoperatively utilizing cardiac computed tomography (CT). METHODS: We conducted a retrospective review of patients who underwent surgical LAA closure. Closure techniques were compared utilizing postoperative cardiac CT. Successful closure was defined by

Regional Blocks in the Era of the Opioid Crisis: Evaluating Their Opioid-Sparing Effect.

Perioperative pain management strategies are increasingly shifting towards opioid-sparing approaches due to the ongoing opioid epidemic, with regional anaesthesia contributing significantly to multimodal analgesia. The purpose of this narrative review was to determine if the effect of regional anaesthesia on opioid consumption has been overestimated. Literature searches performed through PubMed, MEDLINE, and Google Scholar identified relevant studies that evaluated perioperative consumption of opioids, postoperative pain, and recovery profiles for neuraxial and peripheral nerve blocks. Current

Anesthesia management of a child with Melnick-Needles syndrome associated with retrognathia and significant tracheomalacia.

BACKGROUND: Melnick-Needles syndrome (MNS) is a rare X-linked genetic disorder caused by mutations in the FLNA gene, characterized by distinctive craniofacial and skeletal dysmorphisms, hearing loss, and urological anomalies. The associated upper airway abnormalities, including retrognathia and tracheomalacia, present a high-risk scenario for general anesthesia. CASE PRESENTATION: We report the case of an 8-year-old girl with genetically confirmed MNS scheduled for urgent transurethral stenting due to obstructive hydronephrosis and recurrent sepsis. Her clinical features included severe retrog

ACR Appropriateness Criteria® Hydrocephalus-Child.

Hydrocephalus is a disorder of cerebrospinal fluid circulation that leads to ventricular enlargement and increased intracranial pressure. Presentation and imaging choice vary with age: infants often develop progressive macrocephaly because of skull compliance, whereas older children more commonly present with headache, vomiting, and lethargy. In newly suspected hydrocephalus, imaging confirms ventricular dilation, helps assess acuity, may identify an underlying cause, and informs management. In patients with cerebrospinal fluid diversion, imaging primarily evaluates suspected shunt malfunction

Perfusion index and perfusion index ratio as predictive tools for block success: A prospective observational study on ultrasound-guided supraclavicular brachial plexus block.

BACKGROUND: The supraclavicular brachial plexus block is widely used for upper limb surgeries due to its effectiveness in providing anesthesia. Conventionally, success is gauged through sensory and motor assessments, which are not only subjective but also require active patient cooperation. This poses challenges for patients who are sedated or under general anesthesia. Additionally, repeated sensory testing can be uncomfortable. Therefore, an objective and noninvasive method to assess block success is needed. Perfusion index (PI), derived from pulse oximetry, reflects peripheral perfusion chan

Clinical Disorders in Cystic Fibrosis That Affect Emergency Procedures-A Case Report and Review.

Cystic fibrosis (CF) is a multisystemic disease caused by a genetic defect, namely a mutation in the CFTR gene, that results in the production of an abnormal protein that regulates the flow of chloride ions through epithelial cells, leading to the dehydration of secreted mucus and changes in its biological properties. Chronic inflammation and recurrent respiratory infections progressively damage lung tissue, leading to respiratory and cardiorespiratory failure. This study aims to present a clinical case and explore the clinical changes in CF that may influence the provision of pre-hospital fir

Impact of virtual reality distraction during colonoscopy vs intravenous deep sedation: Results of a single-center randomized controlled trial.

BACKGROUND AND STUDY AIMS: Colonoscopy is associated with discomfort that requires intravenous sedation (IVS). The aim of this randomized controlled trial (RCT) was to explore the feasibility of virtual reality distraction (VRD) for colonoscopy using two primary endpoints: cecal intubation rate and the rate of rescue with IVS. PATIENTS AND METHODS: Patients scheduled for elective colonoscopy with IVS were randomized in a 2:1 ratio in favor of VRD, with rescue IVS by propofol if needed. VRD involved use of a device providing a visual and auditive experience similar to clinical hypnosis. RESUL

A RETROSPECTIVE STUDY COMPARING THE SAFETY OF KETAMINE-MEDETOMIDINE AND TILETAMINE-ZOLAZEPAM-MEDETOMIDINE FOR CHEMICAL RESTRAINT OF CAPTIVE PANTHERA SPP.

Chemical restraint, whether remote via dart or via hand injection is essential for many veterinary procedures in captive and wild Panthera spp. but carries risk both to the animal and staff involved. Anecdotal patient safety concerns have been raised for the use of Tiletamine-Zolazepam (TZ), particularly in tigers, that have since been disputed. This investigation assesses the incidence of multiple reported anesthetic complications, including ataxia, respiratory depression/apnea, seizures, bradycardia and retching/vomiting, in captive Panthera spp. immobilized with Ketamine-Medetomidine or TZ-

An Emerging Paradigm for Safer and Faster Recovery: A Narrative Review on Opioid Sparing Anesthesia in Surgery.

Opioid-free anesthesia (OFA) replaces opioid use with many non-opioid drugs, such as dexmedetomidine, lidocaine, esketamine, regional techniques, and enhanced recovery after surgery (ERAS)-aligned strategies to control pain while minimizing opioid-related adverse effects. Across surgical procedures, we conducted a narrative review of the literature, which showed that OFA is consistently associated with lower postoperative nausea and vomiting, faster recovery of gastrointestinal function, and reduced rescue opioid use, with similar post-anesthesia care unit (PACU) stay and pain scores in many t

A Comprehensive Review of a Mechanism-Based Ventricular Electrical Storm Management.

The electrical ventricular storm (VES) is defined as multiple sustained ventricular arrhythmias arising in a short time, often refractory to standard antiarrhythmic treatment. The three pillars of the physiopathogenesis of the VES are autonomic dysfunction, triggers, and an altered ventricular substrate. Incessant or highly recurrent ventricular arrhythmia impacts the hemodynamic status by worsening heart failure and increasing mortality. A stepwise, team-based, and tailored therapeutic approach is required to stop ventricular arrhythmia and regain the hemodynamic and electric stability of the

Oral Health Care for Children and Youth With Developmental Disabilities: Clinical Report.

Oral health is an essential component of overall health for all individuals. The oral health of children and youth with developmental disabilities (CYDD) involves unique characteristics and needs of which pediatricians and pediatric clinicians can be aware. Risk for oral disease in CYDD is multifactorial and includes underlying medical conditions, medications, and ability to participate in preventive oral health care and treatment, and lack of access to providers is common for this population despite being eligible for Medicaid. Pediatric clinicians are uniquely positioned to support the oral

Effect of Dexmedetomidine on Perception of Paresthesia during Subthalamic Nucleus Deep Brain Stimulation Surgery for Parkinson's Disease.

BACKGROUND: Deep brain stimulation (DBS) has become a well-established treatment for the management of Parkinson's disease (PD). The most common method of lead targeting utilizes microelectrode recording (MER) and intraoperative macrostimulation to confirm accurate placement of the lead. This has been significantly aided by the use of dexmedetomidine (DEX) sedation during the procedure. Despite the frequent use of DEX, it has been theorized that DEX may have some effects on the MER during intraoperative testing. The effect on the perception of sensory thresholds during macrostimulation in the
