Treatment of pulmonary nodule: from VATS to RATS.

BACKGROUND: The incidental detection of solitary pulmonary nodule (SPN) is currently increasing due to the widespread use of computed tomography (CT) during the follow up in oncological patients or in screening trials. A quick and definitive histological diagnosis of these nodes is mandatory as, in case of primitive lung cancer, an early detection could improve both surgical results and prognosis. The minimally invasive pulmonary resection (MIPR) is the gold standard procedure for diagnosis and treatment of small lung nodules, but it can be difficult to localize deep nonpalpable nodes that lie

Thoracoscopic Esophagectomy for Metachronous Esophageal Cancer after Heller-Dor Surgery for Achalasia with Marked Esophageal Dilatation: A Case Report.

INTRODUCTION: Achalasia is associated with an increased risk of esophageal squamous cell carcinoma even after definitive surgical treatment. Heller-Dor surgery is an established surgical treatment for achalasia; however, marked esophageal dilatation may persist despite successful symptom control and pose technical challenges during subsequent esophagectomy. CASE PRESENTATION: A 73-year-old man with a history of Heller-Dor surgery for achalasia and prior endoscopic submucosal dissection (ESD) for esophageal squamous cell carcinoma developed a metachronous lesion at a different esophageal site.

Effect of single-injection thoracic paravertebral block with liposomal bupivacaine on the quality of recovery after thoracic surgery: a protocol for an open-label, randomized controlled trial.

BACKGROUND: Postoperative pain management plays a crucial role in determining the quality of recovery following thoracic surgery. However, achieving high-quality postoperative analgesia that facilitates satisfactory recovery remains challenging due to suboptimal pain control and side effects associated with conventional analgesic strategies. This study aims to evaluate the effect of single-injection thoracic paravertebral block with liposomal bupivacaine on the overall quality of recovery in patients undergoing thoracic surgery. METHODS: This single-center, open-label, randomized controlled t

Targeted intrapleural analgesia through an integrated chest tube after thoracoscopic lung resection: a randomized controlled trial.

BACKGROUND: Chest drains are a major contributor to postoperative pain after thoracoscopic lung resection. Localized intrapleural analgesia delivered through the drain itself may provide targeted pain control without additional catheters. This study evaluated whether a drainage-analgesia integrated (DAI) chest tube improves postoperative analgesia after video-assisted thoracoscopic surgery (VATS). METHODS: In this prospective single-center, open-label randomized trial, 47 patients undergoing VATS were allocated to receive either a DAI chest tube (n=22) or standard drainage (STD) chest tube (n

Efficacy and safety of transcutaneous electrical acupoint stimulation for preoperative anxiety in thoracoscopic surgery: a randomized controlled trial.

BACKGROUND: Patients undergoing video-assisted thoracoscopic surgery (VATS) often experience preoperative anxiety, which can significantly impact the surgical process and postoperative recovery. However, the efficacy of Transcutaneous Electrical Acupoint Stimulation (TEAS) in managing preoperative anxiety in VATS patients is unknown. METHODS: A total of 82 patients scheduled for thoracoscopic surgery were randomly divided into TEAS group (n = 41) and sham TEAS (STEAS) group (n = 41). The TEAS/STEAS intervention began 3 days before the thoracoscopic surgery, with one session lasting 30 min per

Development of a classification system for potential sources of error in robotic-assisted spine surgery.

PURPOSE: Robotic-assisted spine surgery (RASS) has increased in prevalence over recent years, and while much work has been done to analyze differences in outcomes when compared to the freehand technique, little has been done to characterize the potential pitfalls associated with using robotics. This study's goal was to leverage expert opinion to develop a classification system of potential sources of error that may be encountered when using robotics in spine surgery. This not only provides practitioners, particularly those in the early stages of robotic adoption, with insight into possible sou

Feasibility and efficacy of lung ultrasound to investigate pulmonary complications in patients who developed postoperative Hypoxaemia-a prospective study.

BACKGROUND: Postoperative pulmonary complications (PPCs) and hypoxaemia are associated with morbidity and mortality. We aimed to evaluate the feasibility and efficacy of lung ultrasound (LUS) to diagnose PPCs in patients suffering from hypoxaemia after general anaesthesia and compare the results to those of thoracic computed tomography (CT). METHODS: Adult patients who received general anaesthesia and suffered from hypoxaemia in the postanaesthesia care unit (PACU) were analysed. Hypoxaemia was defined as an oxygen saturation measured by pulse oximetry (SPO2) less than 92% for more than 30 s

Electromagnetic navigation bronchoscopy-Chungnam National University Hospital experience.

BACKGROUND: To find small pulmonary nodules or ground grass nodules (GGNs) with video-assisted thoracoscopic surgery (VATS) is very difficult. There are several conventional methods to localize small nodules or GGNs, which require additional radiation exposure and may cause some complications such as pneumothorax or hemothorax. We aimed to evaluate the effectiveness and feasibility of electromagnetic navigational bronchoscopy-guided pulmonary localization in a minimally invasive thoracic surgery field. METHODS: We retrospectively reviewed the medical records from a prospectively collected dat

Burden between Undersupply and Overtreatment in the Care of Primary Spontaneous Pneumothorax.

BACKGROUND: The optimal treatment of primary spontaneous pneumothorax (PSP) is still controversial. The purpose of this study was to analyze the incidence of recurrence, the recurrence-free time, and to identify risk factors for recurrence after PSP. METHODS: We performed a retrospective analysis of 135 patients with PSP who were treated either conservatively with a chest tube (n = 87) or surgically with video-assisted thoracoscopic surgery (VATS; n = 48) from January 2008 through December 2012. RESULTS: In this study, 101 (74.8%) male and 34 (25.2%) female patients were included with a mean

Long-term recurrence after surgery for recurrent primary spontaneous pneumothorax: pleural intervention versus surgical access.

The relative contributions of pleural intervention and surgical access to long-term recurrence after surgery for recurrent primary spontaneous pneumothorax remain uncertain. In this single-center retrospective cohort study, 315 patients younger than 35 years who underwent surgery for recurrent primary spontaneous pneumothorax after a second ipsilateral episode were included. Surgical treatment consisted of wedge resection combined with pleural abrasion or pleurectomy, performed through uniportal video-assisted thoracoscopic surgery or muscle-sparing mini-thoracotomy. The primary endpoint was r

Lung volume reduction surgery is safe and feasible after initial endobronchial valve treatment for emphysema patients.

OBJECTIVES: Bronchoscopic lung volume reduction with endobronchial valves is a guideline treatment leading to improved pulmonary function, exercise tolerance and quality of life, in patients with advanced emphysema, severe hyperinflation and no collateral ventilation. After valve treatment, loss of the initial lung volume reduction effect can occur, as well as local valve-induced complications such as persistent haemoptysis. In these cases, a surgical lobectomy can be considered to achieve similar efficacy outcomes. We evaluated the safety and feasibility of a video-assisted thoracoscopic surg

Review of patients discharged post thoracic surgery with chest drain in situ and nurse-based follow-up clinic.

Persistent air leaks and prolonged drainage are recognized complications of thoracic surgery, increasing hospital stay and costs. Patients can be discharged with a chest drain and followed up in a nurse-led clinic. We reviewed such patients and the rate of readmission after discharge to assess the effectiveness of the drain follow-up clinic. We conducted a retrospective review of our prospective database spanning 22 months, from March 2019 to January 2021. The analysis focused on the indication and duration of chest drainage, complications, and readmission for any reason. 62 patients (represen

Enhanced Recovery Protocol Associated With Decreased 3-Month Opioid Use After Thoracic Surgery.

BACKGROUND: Enhanced recovery protocols (ERPs) have been shown to decrease inhospital opioid use after thoracic surgery. However, the impact on opioid use after discharge has not been reported. We hypothesized that prolonged opioid use would decrease after implementation of a comprehensive ERP. METHODS: Records from all patients undergoing elective pulmonary, pleural, and mediastinal operations at a single institution (2015-2018) were abstracted from a prospective ERP database and The Society of Thoracic Surgeons institutional database. Records were reviewed for documentation of opioid use at

Day case parathyroidectomy: is this the right way for the patients?

BACKGROUND: Minimally-invasive video-assisted parathyroidectomy (MIVAP) can be considered as the primary treatment of choice for single parathyroid adenoma. Often, this technique is performed in a day surgery setting and is associated with regional anaesthesia (RA). Many studies have already reported the feasibility and safety of MIVAP in day surgery. Here our focus has been on the patient's personal experience with these procedures through an assessment of their recovery at home. METHODS: We conducted a prospective observational study in the University Hospital of Pisa Day Surgery Unit. Fort

Endoscopic lymph node dissection for thymic malignancies: lateral thoracic intercostal and subxiphoid approaches.

The significance of lymph node dissection for thymic malignancies is currently unclear. Recently, endoscopic surgery has been indicated for the surgical treatment of thymic malignancies. We discussed the range of possible lymph node metastasis following surgery for thymic malignancies, depending on whether the lateral thoracic intercostal or the subxiphoid approach was used. While the video-assisted thoracoscopic surgery (VATS) lateral approach cannot be used for lymph nodes in the anterior region on the opposite side, it is simple enough to be used for deep-region lymph nodes. Taking an appro
