Impact of Type 1 Diabetes Mellitus on Bone Health in Children.

This paper gives an overview of the impact of type 1 diabetes on bone health in children and adolescents. Firstly, we analyse studies using dual X-ray absorptiometry to assess bone mineral content and bone mineral density. Then, we discuss modern, non-invasive techniques including peripheral quantitative computer tomography (pQCT) and high-resolution pQCT for the detailed assessment of bone health aspects including bone mass, bone geometry, bone microarchitecture, and bone strength. Thereafter, we explore some of the mechanisms that are responsible for diabetic bone disease in children, like l

Bone Quality as Measured by Trabecular Bone Score in Normocalcemic Primary Hyperparathyroidism.

OBJECTIVE: The impact of normocalcemic hyperparathyroidism (NHPT) on bone quality remains largely unexplored. We aimed to investigate the usefulness of trabecular bone score (TBS) assessment in NHPT and the accuracy of TBS in predicting vertebral fractures (VFs) in NHPT. METHODS: In this multicentric cross-sectional study, we assessed the TBS in 47 subjects with NHPT, 41 with primary hyperparathyroidism (PHPT), and 39 age- and sex-matched control subjects. RESULTS: TBS values did not differ among the 3 groups. The prevalence of low TBS (TBS < 1.2) was 23.4% in NHPT, 26.8% in PHPT, and 15.4%

Current Applications and Selected Technical Details of Dual-Energy X-Ray Absorptiometry.

The application of dual-energy X-ray absorptiometry (DXA) examinations in the assessment of bone mineral density (BMD) in the lumbar spine, hip, and forearm is the basic diagnostic method for recognition of osteoporosis. The constant development of DXA technique is due to the aging of societies and the increasing importance of osteoporosis as a public health problem. In order to assess the degree of bone demineralization in patients with hyperparathyroidism, forearm DXA examination is recommended. The vertebral fracture assessment (VFA) of the thoracic and lumbar spine, performed by a highly-s

Bone mineral density and oxidative stress in adolescent girls with anorexia nervosa.

Oxidative stress appears to be involved in the pathogenesis of osteoporosis-a serious complication of anorexia nervosa (AN). We evaluated the oxidative status in adolescent girls with AN and its potential relationship with bone mineral density (BMD). Girls with AN (n = 43) and age-matched healthy controls (n = 20) underwent anthropometric and BMD examination. Markers of bone turnover, oxidative stress, and antioxidant status were measured. Participants with AN and controls did not differ in BMD at the lumbar spine (p = 0.17) and total body less head BMD (p = 0.08). BMD at the total hip was low

Evaluation of Body Composition in Paediatric Osteogenesis Imperfecta.

Osteogenesis Imperfecta (OI) is a skeletal disorder characterised by a predisposition to recurrent fractures and bone deformities. Clinically OI is defined by features such as short stature, however, less is known regarding body composition. Assess body composition, both lean mass and fat mass, in a paediatric OI population. Children with OI attending the Bone service at the Royal Hospital for Children Glasgow were included; who had a dual-energy x-ray absorptiometry (DXA) scan performed 2015-2018. Height and body-mass-index (BMI) were converted to standard-deviation scores (SDS) using UK popu

High prevalence of osteoporosis in patients undergoing spine surgery in China.

BACKGROUND: With the increase in life expectancy, a large number of patients with osteoporosis (OP) are undergoing spine surgery, which may adversely affect the surgical success rate. The prevalence of OP varies in different regions, and no data are available that represent the prevalence of OP among Chinese patients over 50 years of age who are undergoing spine surgery. It was the first multicenter study to assess OP in these patients. Aiming to obtain comprehensive data, this study combined bone mineral density (BMD) measurements and visual radiography assessment (VRA) to analyze the prevale

Evaluation of postural stability in patients screened for osteoporosis: A retrospective study of 1086 cases.

BACKGROUND: Although the interactions between balance, falls and fracture risk have been demonstrated in various aspects, the clinical focus of fracture risk evaluation has been on bone properties, e.g., bone mineral density (BMD). Accordingly, the role of balance parameters in the setting of osteoporosis assessment remained to be further explored. RESEARCH QUESTION: Is postural stability assessed by Romberg quiet stance posturography influenced by BMD or prior fragility fractures in a cohort of more than 1000 patients screened for osteoporosis? METHODS: Patients who completed both Romberg t

Estimation of bone mineral density with hounsfield unit measurement.

OBJECTIVE: Although dual-energy x-ray absorptiometry (DXA) remains the gold standard for the measurement of bone mineral density (BMD), degenerative spine and spinal instrumentation yields inaccurate results, warranting the need for optional methods. METHODS: Surgical options depend on the BMD of the patients, and to accommodate this need, we compared Hounsfield unit (HU) measurements obtained from computed tomography (CT) scans with the T-scores obtained from DXA scans to inquire whether HU measurements can screen for BMD. In this study, we also evaluated the relationship between body mass i

Allometric Modeling of Wingate Test among Adult Male Athletes from Combat Sports.

Background and objectives: Athletes from combat sports are grouped into a series of weight categories that are intended to promote fair competition. Differences in performance are partly attributable to differences in body size. Consequently, ratio standards in which a performance variable is simply divided by an anthropometric characteristic such as body mass are often used, although this application is not recommended. This study aimed to obtain allometric models to interpret Wingate Anaerobic Test (WAnT) outputs among male adult athletes from combat sports. Materials and Methods: The sample

Deriving pQCT-Type Spatial Density from DXA Images.

INTRODUCTION: Musculoskeletal overuse injuries are a serious problem in the military, particularly in basic combat training, resulting in hundreds of millions of dollars lost because of limited duty days, medical treatment, and high rates of reinjury. Injury risk models have been developed using peripheral computed tomography (pQCT)-based injury correlates. However, pQCT image capture on large number of recruits is not practical for military settings. Thus, this article presents a method to derive spatial density pQCT images from much lower resolution but more accessible dual-energy X-ray abso

Bone Features of Unaffected Skeletal Sites in Melorheostosis: A Case Report.

BACKGROUND: Melorheostosis is a rare sporadic sclerosing bone dysplasia, which commonly affects appendicular skeleton with bone hyperostosis and soft tissues sclerosis; fragility fractures are rare in melorheostotic patients. We investigated bone features at unaffected sites in a postmenopausal woman with melorheostosis of the right lower limb and with a fracture of the melorheostosis-free T11 vertebral. METHODOLOGY: Melorheostotic lesions were evaluated by plain radiography, magnetic resonance of the right lower limb, and whole-body bone scintigraphy. Dual X-ray absorptiometry, trabecular bo

A Primary Care Provider's Guide to Bone Health in Spinal Cord-Related Paralysis.

Individuals with spinal cord injury/disorder (SCI/D) are at high risk for developing secondary osteoporosis. Bone loss after neurologic injury is multifactorial and is dependent on the time from and extent of neurologic injury. Most bone loss occurs in the first year after complete motor paralysis, and fractures occur most commonly in the distal femur and proximal tibia (paraplegic fracture). The 2019 International Society for Clinical Densitometry Position Statement in SCI establishes that dual-energy X-ray absorptiometry (DXA) can be used to both diagnose osteoporosis and predict lower extre

Bone Geometry, Density, and Microarchitecture in the Distal Radius and Tibia in Adults With Marfan Syndrome Assessed by HR-pQCT.

Marfan syndrome (MFS) is a hereditary disorder of connective tissue caused by mutations in the fibrillin-1 gene. Studies have shown that patients with MFS have lower bone mass, but little is known about the other constituents of bone strength. We hypothesize that patients with MFS will have larger bone area and compromised cortical microarchitecture compared with non-MFS individuals. A total of 74 adult patients with MFS and 145 age- and sex-matched non-MFS reference individuals were included in this study. High-resolution peripheral quantitative computed tomography (HR-pQCT) at the distal rad

The Effect of Neck Flexion on Measurement of Spinal Bone Mineral Density and the Need to Apply Head Positioners During Dual-Energy X-Ray Absorptiometry.

Our purpose was to investigate any potential effect of neck flexion on measurement of spinal bone mineral density (BMD) through further reduction of spinal lordosis and whether it is necessary to apply a head positioner, in addition to a leg positioner, during dual-energy x-ray absorptiometry. Methods: Fifty-nine patients with no significant history of spinal disorders were recruited. A bone densitometry scan of the spine was obtained for all patients using a standard leg positioner in the supine position. Then, another scan of the spine was conducted using a small subnuchal cushion to flex th

Relationships between anthropometric adiposity indexes and bone mineral density in a cross-sectional Chinese study.

BACKGROUND CONTEXT: Previous studies have reported conflicting results for the relationships between anthropometric adiposity indexes and bone mineral density, based on dual-energy X-ray absorptiometry (DXA). However, few studies were published based on quantitative computed tomography (QCT), especially for Chinese population. PURPOSE: To evaluate the associations of spine bone mineral density (BMD) with body mass index (BMI), waist circumstance (WC), waist-to-hip ratio (WHR), waist-to-height ratio (WHtR) and a body shape index (ABSI) using QCT. STUDY DESIGN/SETTING: A Cross-sectional study.
