A retrospective study of the injury patterns seen in fatal falls downstairs.

INTRODUCTION: Fatal falls involving stairs are often associated with advanced age, intoxication, acute natural pathology and less commonly - the involvement of a third-party. The injury patterns seen in these cases are often complex and from a medico-legal standpoint, determining third-party involvement based on pathology alone can prove a significant challenge. Classifying the manner of death in such cases, can become particularly complex when the injury pattern appears inconsistent with a simple fall downstairs, or there is an absence of eye-witness testimony. AIM: To explore the circumstan

Bilateral subdural hematomas and retinal hemorrhages mimicking nonaccidental trauma in a patient with D-2-hydroxyglutaric aciduria.

INTRODUCTION: Nonaccidental trauma (NAT) is considered when pediatric patients present with intracranial injuries and a negative history of an accidental injury or concomitant medical diagnosis. The evaluation of NAT should include the consideration of possible medical causes including coagulation, hematologic, metabolic and other genetic disorders, as well as witnessed and unwitnessed accidental injuries. CASE PRESENTATION: We present a 7-month-old male with spells and incidental findings of bilateral subdural hematomas, retinal hemorrhages, and secondary macrocephaly, leading to investigati…

Understanding non-accidental trauma in the United States: A national trauma databank study.

BACKGROUND: The purpose of this study is to characterize the epidemiology, injury patterns, outcomes and trends of non-accidental trauma (NAT) in the United States using a large national database. METHODS: Children <15 years presenting after NAT were identified in the 2007-2014 National Trauma Databank research datasets. Clinical and outcome data were analyzed using descriptive statistics, chi-square and logistic regression. RESULTS: Of 678,503 children admitted for traumatic injuries, 3% (19,149) were victims of NAT. The majority (95%) were under 5 years and 71% under 1 year old. The majori

Does patient cognition predict time off from work after life-threatening accidents?

OBJECTIVE: Accidental injuries are frequent and their socioeconomic consequences enormous. The present study aimed to identify predictors of the number of days of leave taken in a consecutively selected group of accident victims who sustained severe, mostly life-threatening physical trauma. METHOD: One hundred patients with severe accidental injuries who were referred to a trauma surgeons' intensive care unit were followed up for 12 months. The main outcome measure was the number of days of leave taken that were attributable to the accident 1 year after the trauma. RESULTS: Multiple regressi…

A Rare Case of Intramediastinal Humeral Head Fracture Dislocation Through the Thoracic Inlet.

Intramediastinal fracture dislocation of the humeral head due to a blunt trauma is very rare. It may be accompanied by local and systemic injuries associated with high-energy trauma. Only one case has been reported in the literature; therefore, an appropriate treatment modality remains unclear. We present a case of a 71-year-old woman involved in an accidental fall who sustained an intramediastinal humeral head fracture dislocation through the thoracic inlet, without damaging the chest wall, causing the thoracic duct rupture and requiring three consecutive surgical procedures as treatment.

Ultrasound-guided technique allowed early detection of intravascular injection during an infraclavicular brachial plexus block.

The reported incidence of complications after peripheral nerve blocks is generally low and varies from 0% to 5%. The injuries related to brachial plexus block are perhaps more commonly reported, than after peripheral blocks of the lower extremity nerves. Recent reports suggest that expert ultrasound guidance may reduce but not completely eliminate complications as intraneural or intravascular injection. We report a case of accidental intravascular injection of local anesthetic during infraclavicular brachial plexus block, in spite of the use of ultrasound guidance technique, and negative aspir

'Rib betrays heart'--a case report.

Penetrating or stab injuries of the chest are not uncommon and could be accidental, suicidal or homicidal. However, the present case discusses the importance of a careful examination of an unusual presentation in a motor vehicle accident. The case presented with haemopericardium and haemothorax due to penetrating rib injury to the heart, subsequent to blunt force trauma on the posterior chest wall. This case is highlighted, so that it becomes a necessary tool for the forensic experts to have an unprejudiced approach at autopsy and come to a reasonable conclusion.

Life-threatening hemorrhage from inadvertent cervical arteriotomy.

Catastrophic hemorrhage followed accidental puncture of the carotid artery or its branches during attempted preoperative insertion of flow-directed balloon-tipped pulmonary artery catheters in 2 patients who underwent open cardiac procedures. Direct repair of two carotid artery lacerations was necessary in 1 patient; the second patient slowly resorbed a large extrapleural hematoma without surgical intervention. This experience suggests that patients experiencing such seemingly innocuous but potentially disastrous arterial injuries should not be heparinized, and that any procedure involving car

[Examination of Bell's palsy in consideration of the Austrian private accident insurance].

In western Europe peripheral facial palsy is the most common cranial nerve disorder. The constitutional palsy may be ideopathic or combined with other disorders, above all Lyme's disease and otogenic zoster. The traumatic palsy is nearly always combined with other severe injuries and above all caused by accidents.Constitutional palsies have a less degree of partial recovery compared to the traumatic forms. The expert has to evaluate the actual status of palsy at the time of examination and also to consider prognostic outcome. The Austrian private accident insurance covers the risk of injuries

Severing of skull cap: A rare trauma attributed to transportation of bulldozer.

Bulldozer fatalities are usually due to accidental crushing of the body at the workplace. However, severance of the skull cap simulating a chopping injury to head is rare in the literature. Medico-legal investigation may be posed with different challenges when carrying out an autopsy of a victim with this devastating head injury. The police will seek an opinion about the type of weapon responsible for the injuries, the nature of injuries produced and manner of death. In the present case, the victim was hit at dusk by a protruding part of the bulldozer blade as he crossed in front of a bulldoze

Simultaneous bilateral femoral neck fractures after electrical shock injury: a case report.

Simultaneous bilateral fractures of the femoral necks are rare injuries, especially when there is no underlying pathological condition. We report a 20-year-old man who sustained bilateral femoral neck fractures resulting from an accidental electric shock with 440 V direct current. Simultaneous bilateral femoral neck fractures after electrical shock injury without falling from a height are rather uncommon in clinic. The main cause of the fracture may be muscle contraction. This case highlights that even in the absence of primary and secondary bone disease, bilateral fractures of the femoral nec

The care and cost of snowmobile-related injuries.

From January 1, 1991, through May 1, 1993, we identified 42 patients from our prospective computer-based trauma registry (38 males, four females; mean age, 25 years) who were hospitalized after snowmobile accidents. The primary reason for hospitalization varied: bone fracture (n = 18), blunt abdominal trauma (n = nine), closed head injury (n = five), and miscellaneous injuries (n = 10). The mean injury Severity Score was 9.3 (range, one to 43; median, nine). Twenty-six patients (62%) required emergent operation. Mean hospital stay was six days (range, one to 16 days). Thirteen patients had com

[Caustics-induced lesions of the gastrointestinal tract].

Ingestion of a corrosive agent results in a life-threatening clinical condition that requires emergency intensive care and often surgical intervention. Treatment is multifactorial and varies with the extent and depth of the injury. We report a case of a young 24-year-old man with accidental ingestion of sulphonic acid. After 1 month he developed a progressive dysphagia. Early endoscopic examination showed grade III injury to both the esophagus and stomach with severe and undilatable stricture of the antrum. Therapy included total parenteral nutrition and antibiotics. Repeat endoscopic examinat

Shotgun injuries in pediatric patients.

Firearms, particularly high-powered automatic weapons, injure progressively younger children in urban environments. At Arkansas Children's Hospital, 32 children have been admitted for gunshot wounds in 12 years. Air guns (3), handguns (19), and shotguns (10) caused the injuries. Shotgun wounds occurred during hunting accidents (2), flights from houses (3), and mistaken identity in the home (5), usually by family members (5). All victims of shotgun wounds were boys; seven were black. Their ages ranged from 6 to 20 years. Two injuries involved the abdomen, one the flank, one the chest, one the a

Analysis of injury- and violence-related fatalities in the Ohio Medicaid population: identifying opportunities for prevention.

BACKGROUND: To identify the leading causes of injury- and violence-related deaths in demographic subgroups of the population in Ohio, by Medicaid status. METHODS: We used linked Ohio Medicaid and death certificate files, 1992 to 1998, and obtained the probability (p) of dying from a specific mechanism of injury--given death from injury--by Medicaid status, using multinomial multivariable logistic regression analysis. Probabilities were rank-ordered to identify the leading causes of death in each subgroup. RESULTS: The leading cause of injury-related deaths was homicide among Medicaid deceden

Experiences from Burn Registry Pilot Study in Northern India: Patient Profile and Implementation Perspectives.

In India, burns represent a serious public health concern due to high death and disability rates. A national burn registry was proposed under the National Program on Prevention and Management of Burn Injuries. This study reports experiences from the first pilot burn registry in India, presenting patient profiles and implementation perspectives. Five burn centers were selected to implement a prospective, multicentric burn registry in northern India. Every burn patient at the study sites who met the inclusion criteria was recruited. Patients' sociodemographic and burn injury profiles, determinan

Case report: Injury plausibility for skull fractures in two kittens.

A forensic postmortem examination (FPME) was performed on two kittens. In each case, investigators were given conflicting stories describing how the kitten died. The goal for each FPME was to determine the cause of death with the hope of also determining which story was more plausible. In both cases, animal control officers responded to a call to retrieve a deceased kitten. In the first case, the kitten's owner reported the kitten had died when thrown to the floor by the owner's boyfriend; the boyfriend reported he had accidentally dropped the kitten. In the second case, an ex-girlfriend of th
