We performed quantitative assays for estrogen, progesterone, and testosterone receptors in 19 acoustic neuroma specimens from ten men and nine women. No patient received preoperative or intraoperative glucocorticoids, mineralocorticoids, or sex hormones. All tumors were unilateral and removed by the translabyrinthine approach. No tumor specimen was positive for estrogen or testosterone receptors. Three of ten men and seven of nine women had tumors positive for progesterone receptors (10 fmol/mg cytosol protein or greater). This between-sex difference was of borderline significance by the Mann-

Using sequence-specific radioimmunoassays the occurrence of 4 neuropeptides (cholecystokinin (CCK), gastrin, somatostatin, substance P and some of their precursors) was examined in 19 human acoustic nerve tumors and corresponding normal tissue. The normal acoustic nerve tissue obtained by autopsy contained traces of CCK, somatostatin, and substance P (less than or equal to 0.5 pmol/g), but neither proCCK, gastrin nor progastrin. In contrast 8 tumors expressed proCCK (range 0.2-4.5 pmol/g), 3 tumors gastrin (0.1-7.3 pmol/g) and 14 tumors progastrin (0.6-2.2 pmol/g). Traces of somatostatin were

Thirteen patients with acoustic neuroma were studied on a 1.5 T superconductive magnetic resonance (MR) imager. Acoustic neuromas appeared as lower signal intensity than the surrounding brain stem on T1 weighted image (W.I.), and as higher signal intensity on T2 W.I.. Axial and coronal sections of T1 W.I. were very useful in observing the tumor in the auditory canal and in investigating the anatomical relations of the tumor and the surrounding structures. MR imaging is very excellent examination to make early diagnosis of the acoustic neuroma and preoperative anatomical evaluation.

Eight male patients suffering from Menière's syndrome are presented. All patients sustained sudden or prolonged exposure to severe noise levels and suffer from acoustic trauma. The incidence of Menière's syndrome in our group of patients is significantly higher than in series reported in the normal population. Our presentation supports the implication of acoustic trauma as a possible cause of Menière's syndrome.

The objective of this study was to determine the usefulness of acoustic tracking as a supplement to perceptual judgements during remediation. One child, receiving weekly individual treatment, participated in a drill-and-practice approach to remediation for a single [w]-for-/r/ substitution error. Imitations of consonant-vowel (CV), VCV, and matched /r/ and /w/ sentence stimuli were audiotape recorded. The temporal and spectral characteristics of the recorded stimuli (i.e., the F2 transition rates and F2 values for the changing [r]) were spectrographically tracked and analyzed over a 70-day rem

The authors report a case of acoustic neurinoma presenting as intratumoral bleeding. This is the tenth reported occurrence. The literature is reviewed. All cases that have been reported have appeared with sudden onset of headache, vomiting, and decreased levels of consciousness. Aneurysm rupture, an arteriovenous malformation, or other vascular anomalies are suspected first. Preexisting unilateral hearing impairment is a valuable clue to differential diagnosis. Contrast-enhanced computed tomographic scans and cerebral angiograms are important tools for correct diagnosis. The tumor size (greate

A retrospective study was made of all operations for acoustic tumour over a 40-year period. A total of 392 cases are reported. Trends over time and with the size of the tumour are reported. In the current decade all but two tumours were completely resected, the mortality rate was 1.4%, 94% of patients had a good or excellent result, the facial nerve was preserved in 64% of cases (90% of these with acceptable function) and the cochlear nerve in 13.5% (some with hearing). There is a striking dependence on the size of the tumour of the likely outcome of operation.

Six cases of acoustic neurinoma (AT) with preoperatively preserved hearing are presented. Their clinical features and surgical management for hearing preservation are discussed. Presenting symptoms were hearing decrease in 3 cases (#1, #2, #6), trigeminal neuralgia in 3 cases (#4, #5, #6), and hemifacial spasm in case #2. Case #3 was an incidentally diagnosed case by CT scan. Preoperative decrease of hearing in the affected ears ranged from 10 dB to 60 dB. In all cases except for case #1, CT scans revealed CP angle tumors sized 1 - 4 cm in diameter. A small intracanalicular tumor was detected

A data base of acoustic-immittance measures in normal adults is presented. The subject pool consisted of 127 adults with normal hearing and a negative otologic history. Norms are presented for hearing thresholds, ipsilateral and contralateral acoustic-reflex thresholds, tympanometry, static acoustic-admittance measures, and middle-ear (tympanogram peak) pressure.

Caution is required in diagnosing even apparently "obvious" causes of hearing loss. A patient is described who came to medical attention through referral from a lawyer for sudden deafness allegedly caused by exposure to noise in an industrial setting. Thorough evaluation revealed an acoustic neuroma. Review of the literature suggests that presentation of acoustic neuromas as sudden deafness is more common than generally recognized. Clinicians must be alert to this possibility and diligent in their evaluation of any unilateral ear symptoms.

Cardiac, respiratory and neurologic abnormalities have been identified as causes of Sudden Infant Death Syndrome (SIDS). Recurrent central apnea (no respiratory effort or nasal/oral airflow) and obstructive apnea (respiratory effort without concurrent nasal/oral airflow) in infants are considered risk factors for SIDS. However, using currently available monitoring techniques, normal activities such as yawns, stretches and swallows cannot be distinquished from short obstructive episodes lasting less than 20 s. A system was developed to more accurately detect obstructive apnea in infants using a

This paper reviews the author's personal experience with the management of 204 patients with the clinical diagnosis of acoustic neuroma. Craniotomy was performed in 181 of these cases, all of whom were proven to have acoustic neuromas. The remaining 23 cases with the clinical diagnosis of acoustic neuroma did not have tumour surgery, mainly because of advanced age or concurrent disease, although some required shunts for hydrocephalus. The management of acoustic neuromas including the selection of surgical approach depends upon the patient's age and general health, the size and growth direction

In a series of 120 patients with acoustic neurinoma, hearing preservation at removal of the tumour via the suboccipital approach was attempted in 30 ears. Hearing was preserved in 13 ears (43%), but in two, hearing was lost entirely in 3 to 4 years; thus the success rate was 36%. In 9 of the remaining 11 ears useful speech discrimination was present. Audiological tests showed increased retrocochlear loss after surgery. A surgical team should master the methods of both translabyrinthine and suboccipital surgery in order to choose the best approach for each patient.

The specific features of neurinoma of the auditory nerve were analysed clinically in 21 patients whose ages ranged from 12 to 18 years. Neurinoma of the auditory nerve located in the cerebellopontine area in children and adolescents has some specific features distinguishing it from that in adults. For instance, its incidence among boys is the same as that among girls. In most patients the neurinomas grew medially and orally, which caused gradual development of hydrocephalic-hypertensive symptoms and only in the far advanced stage of the disease did local symptoms in the form of affection of th

This study investigates the controversy regarding the influence of age on the acoustic reflex threshold for broadband noise, 500-, 1000-, 2000-, and 4000-Hz activators between Jerger et al. [Mono. Contemp. Audiol. 1 (1978)] and Jerger [J. Acoust. Soc. Am. 66 (1979)] on the one hand and Silman [J. Acoust. Soc. Am. 66 (1979)] and others on the other. The acoustic reflex thresholds for broadband noise, 500-, 1000-, 2000-, and 4000-Hz activators were evaluated under two measurement conditions. Seventy-two normal-hearing ears were drawn from 72 subjects ranging in age from 20-69 years. The results

A technique was explored for detecting the ipsilateral acoustic reflex using one acoustic signal (226-HZ tone) both to elicit the reflex and to measure the impedance of the ear. By eliminating the presentation of two acoustic signals to the same ear, artifactual responses that result from interactions between two signals are avoided. The method provided reflex threshold estimates for five normal subjects that averaged 99.8 dB SPL, in close agreement with previously reported data. A sixth subject, with otosclerosis, showed no evidence of a reflex. With some refinement, the method could become a
