We describe a case of a 7-year-old child with Down syndrome who presented with loud snoring and cessation of breath during sleep and was found to have a large calculus (20 mm X 12 mm X 12 mm) in her left tonsil by CT scan for which tonsillectomy with adenoidectomy were done. This is one of the youngest reported cases in the literature.

The anatomical and embryological facets of the persistent craniopharyngeal canal combined with a meningoencephalocele are reviewed, and the case of a 6 year old girl is reported. Postnatal dyspnea and cyanosis improved after perforation of nasopharyngeal cyst, and adenoidectomy was performed at the age of 8 months. The second episode of meningitis led to its radiological assessment and correct diagnosis. Surgical treatment is limited because the location of the pituitary in this child is not known.

The Velocardiofacial (VCF) syndrome is a relatively frequent cromosomopathy that usually associates various otorhinolaryngological features, as hipenasal speech, typical facies and auricular anomalies. We report a patient with VCF syndrome that before being diagnosed had undergone adenoidectomy with a postoperative worsening in speech. Otorhinolaryngological clinical features of the VCF syndrome are discussed and a diagnostic protocol is proposed to achieve an early diagnosis and to prevent iatrogenic interventions in these patients.

Sixty-four parents completed questionnaires pertaining to their knowledge, attitudes and experiences both before and two months after their children underwent adenoidectomy and grommet insertion. Although a positive image of and high satisfaction with both procedure and inpatient stay were demonstrated, many parents failed to appreciate the existence, nature and frequency of possible complications. Parents should be given quite explicit descriptions of the most common postoperative problems.

Extramedullary plasmacytomas are extremely rarely diagnosed in children. We report two cases of extramedullary plasmacytoma detected coincidentally in the adenoidectomy specimens of children younger than 4 years. We show that these children are disease-free after local excision as the only treatment. Surgical treatment without anti-neoplastic therapy in pediatric plasma cell dyscrasias of the pharyngeal lymphoid tissue may be a sufficient curative therapy.
