Foot skin can be used in traumatic nonreplantable leg amputations as an immediate neurovascular free flap for proximal stump reconstruction. The thick, glabrous skin on the sole of the foot provides durable, sensate coverage. The flap is large, avoids a secondary donor site, and can survive on either the dorsalis pedis or posterior tibial artery if one preserves the connecting deep plantar branch of the dorsalis pedis artery. Two cases of immediate stump reconstruction using the "fillet of sole" flap are presented.

After hind limb amputation in lizards, scars or short outgrowths are formed. FGFs may stimulate regeneration. Injections of FGF-1/-2 into the limb stump were made until 26 days post-amputation. We report outcomes of 28 patients after stump-lengthening procedures (SLPs) with modular tumor endoprostheses following high-thigh amputation and hip disarticulation over 11years. Mean follow up was 41.3months (range 7.4 to 133.6months). Mean Musculoskeletal Tumour Society Score was 56% (n=11); ten out of eleven patients alive used an exoprosthesis regularly. We report a case of localized squamous cell carcinoma arising in the ulceration at the site of a below-knee amputation in a patient with chronic lymphocytic leukemia on treatment with ibrutinib. The patient underwent local excision of the skin and soft tissue with histopathology showing a small focus of well-differentiated squamous cell carcinoma in the specimen. This case highlights the importance of clinical evaluation and histopathological review for underlying malignancy in the setting of amputation stump ulceration.

The treatment of pain after digital amputation consists of surgical treatment if the pain is due to excessive nociception on the nerve. With micro-surgical technique, it is possible, like Samii, to suture some of the fascicles into central direction with the other nerves fascicles, after resection of the nervroma; or to anastomose the nerve with the other nerve, if we have two nevromas on the same digital amputation stump. Results of this technique are generally progressive disappearance of the pain.

This retrospective review of the long-term problems of patients with lower extremity amputation from trauma fails to support the opinion that most traumatic amputees do well. Overall, anatomic problems involved 51% of the amputees, and social problems involved 56%. Attempting limb salvage with subsequent secondary amputation did not seem to compromise the long-term outcome for these problems. A retrospective study of 191 patients undergoing 203 Burgess type below knee amputations for end-stage peripheral vascular disease showed that the only clinical feature which associated significantly with wound healing was the lowest palpable pulse. The pulse level was noted preoperatively and was compared with the healing rates in the amputation stumps. When the femoral pulse was absent, 79% of below knee amputations failed. When the femoral pulse was palpable, only 29% wound breakdown occurred. When the popliteal or more distal pulses were palpable, only 10% failure was recorded.

An adult man underwent arm amputation for a sarcoma. Pain and three masses observed radiologically prompted surgical exploration five years later. Microscopically, the masses represented amputation neuromas. One of them was located in the lumen of an artery, in a remote organized thrombus. Intravascular growth of an amputation neuroma has not been described previously.
