Lansoprazole compared with ranitidine for the treatment of nonerosive gastroesophageal reflux disease.

BACKGROUND: Traditionally, proton pump inhibitors are used primarily for patients with esophagitis. However, patients with nonerosive reflux disease may also benefit from these powerful medications. OBJECTIVE: To compare the safety and symptom relief efficacy of lansoprazole with ranitidine therapy and with placebo. METHODS: In 2 randomized, double-blind, multicenter trials of 901 patients with symptomatic reflux disease, which was confirmed by endoscopy to be nonerosive, received lansoprazole, 15 or 30 mg once daily; ranitidine, 150 mg twice daily; or placebo for 8 weeks. RESULTS: Analysis

Differences in plasma and tissue aluminum concentrations due to different aluminum-containing drugs in patients with renal insufficiency and with normal renal function.

The gastrointestinal absorption of aluminum from orally administered aluminum-containing drugs is well documented. Retention of the absorbed aluminum leads to elevated levels of this metal in the tissue of patients with renal failure. We studied two groups of dialysis patients who had received equal amounts of the different aluminum-containing phosphate-binders Aludrox and Antiphosphat. It has recently been shown that Antiphosphat releases only a few aluminum ions in an environment of low pH. Consistent with this finding, we found the aluminum levels to be significantly higher in the plasma, b

Effects of combined treatment with triletide and cimetidine in gastric and duodenal ulcer patients, and a retrospective comparison with cimetidine alone.

A study was carried out in 15 patients with gastric or duodenal ulcer to assess whether the efficacy of the antisecretory agent cimetidine might be improved by the addition of a cyto-protective agent, triletide, to the treatment regimen. The patients were treated for an average of 11 +/- 5 days with 0.2 g parenteral cimetidine per day plus 1.5 g oral triletide per day. Endoscopy was performed before and after treatment, as well as routine haematology and haematochemistry. Intensity of symptoms and signs of the illness was scored and recorded daily during the first 5 days of treatment, as well

Indications for the use of parenteral H2-receptor antagonists.

Development of acute mucosal ulceration is a complex series of catabolic interactions. Hospitalized patients with duodenal or gastric ulcer, pathologic gastric hypersecretory states (such as Zollinger-Ellison syndrome), gastric outlet obstruction, esophagitis, severe gastritis or duodenitis, sepsis, trauma (particularly head injury or burns), and some patients receiving high-dose corticosteroids are at risk of developing acute stress ulcers. Treatment should be initiated as soon as the patient is identified as being at risk, because measures designed to prevent bleeding or perforation are more

Doxycycline Trap: A Case Report on the Unexpected Development of an Oesophageal Ulcer.

The phenomenon of drug-induced mucosal damage has not been reported much in the literature, although over 100 drugs are known to cause this condition. Tetracyclines contribute the most to mucosal damage. The common symptoms are difficulty swallowing, pain while swallowing, and possibly retrosternal pain. The diagnosis is based on upper endoscopy. The therapeutic approach involves discontinuing the causative drug, switching to parenteral fluid intake, and administering proton pump inhibitors and antacids. Proper drug intake in an upright position with plenty of fluids is key to preventing mucos…

Gastroprotective Therapy.

A range of gastroprotective drugs are available for the treatment of esophagitis and gastroduodenal mucosal injury including acid suppressants (ie, histamine-2 receptor antagonists, proton pump inhibitors), coating agents, prostaglandin analogs, and antacids. Of these, the proton pump inhibitors are the most effective drugs for the medical treatment of upper gastrointestinal injury. However, proton pump inhibitors are not effective for all causes of upper gastrointestinal injury. The choice of gastroprotective drug should be guided by the cause and location of gastrointestinal injury and the p…

[Effects of histamine H2-receptor blockers and dalargin on reparative processes gastroduodenal mucosa in patients with duodenal ulcer].

The effect of synthetic opiate peptide dalargin, histamine H2-receptors blockers (cimetidin, famotidin) and combination of dalargin with famotidin on proliferative activity of gastroduodenal mucosal epithelium has been studied autoradiographically (3H-thymidine) in 75 patients with duodenal ulcer. Compared to spasmolytics and antacids, the studied drugs (dalargin, cimetidin and famotidin) significantly increased the number of DNA-synthetizing cells in the epithelium of fundal stomach, duodenal cap and ulcer borders. Maximal effect was noticed in combined use of dalargin and famotidin. A correl…

Peptic ulcer disease.

Patients with peptic ulcer disease are commonly seen by family physicians. The incidence of duodenal ulcer seems to be declining in the United States. Avoidance of inciting factors and the use of antacids and cimetidine are indicated in the treatment of duodenal ulcers. New histamine H2 receptor antagonists and site-specific mucosal barrier agents are now available. Gastric ulcer differs from duodenal ulcer in many ways, and gastric carcinoma must be considered in the differential diagnosis. Other important entities include Zollinger-Ellison syndrome, "stress" ulceration, and peptic ulcer dise…

Cimetidine in elderly patients: review of uses and risks.

Cimetidine is widely prescribed but has numerous potential side effects, especially in elder patients, who require a lower dosage, e.g., 300 mg twice daily. Antacids may be more effective than cimetidine in preventing acute gastrointestinal bleeding in critically ill elderly patients. Cimetidine increases the risk of toxicity of other commonly used drugs such as phenytoin, diazepam, chlordiazepoxide, propranolol, theophylline, warfarin, and carbamazepine. Elderly patients taking cimetidine are at risk for neuropsychiatric changes, which may be temporarily reversed by physostigmine. Other side …
