[Urinary tract infections].

From the viewpoint of prevention of nosocomial infections, the urinary indwelling catheter should be carefully managed. Under long term indwelling, prophylactic antimicrobial chemotherapy is no sense and generally bladder irrigation is also not necessary. In this paper, the characteristics of urinary tract infections due to MRSA and P. aeruginosa is described. The incidence of MRSA isolation from urine is increasing in recent several years. The distinction between infections and colonizations is important.

Urinary Tract Infections After Urogynecologic Surgery: Risk Factors, Timeline, and Uropathogens.

IMPORTANCE: Urinary tract infection (UTI) is a common and potentially avoidable postoperative (PO) adverse event after urogynecologic surgery. Understanding pathophysiology will help prevent the associated morbidity of the disease and treatment of PO-UTI. OBJECTIVE: The objective of this study is to determine the following: (1) risk factors for both PO-UTI and PO recurrent UTI (RUTI) after urogynecologic surgery, (2) temporal distribution of UTI, and (3) uropathogen identification. STUDY DESIGN: Women undergoing urogynecologic surgery were retrospectively reviewed. Urinary tract infection wa

Pasteurella aerogenes as an Asymptomatic Bacteriuria Agent.

'Asymptomatic bacteriuria' (ASB) is isolation of a specified quantitative count of bacteria in an appropriately collected urine specimen obtained from a person without symptoms or signs referable to urinary infection. Catheterized specimens are less likely to be contaminated compared with voided specimens; therefore, positive cultures of catheterized specimens are more likely to reflect true bladder bacteriuria even with low colony counts. The common pathogens for ASB are Escherichia coli, Klebsiella and Streptococcus spp. Pasteurella spp. was not previously reported as an ASB agent. ASB is im

[A case of primary mucosa-associated lymphoid tissue-type lymphoma of the urinary bladder that progressed after antibiotic therapy].

We report a case of primary mucosa-associated lymphoid tissue (MALT)-type lymphoma of the urinary bladder, which temporarily regressed after antibiotic therapy and progressed 1 year after the treatment. The patient was a 72-year-old female with a history of recurrent cystitis. She was referred to our hospital for microscopic hematuria. Urinalysis also showed microscopic pyuria and cystoscopy revealed an erythematous and edematous submucosal lesion in the right side wall of the bladder. She was diagnosed with acute cystitis and treated with antibiotics. Cystoscopy after 2 months was normal. How

Urinary tract infections in spinal cord injury: prevention and treatment guidelines.

OBJECTIVES AND METHODS: The literature on prevention and therapy of urinary tract infection (UTI) in patients with spinal cord injury (SCI) was reviewed using 3 levels of evidence. RESULTS: Antibiotic therapy is only indicated in symptomatic bacteriuria or in symptomatic exacerbations of chronic UTI. During the acute phase of a SCI, UTI's are more prevalent and bacteria are different and more resistant to antibiotics compared with the chronic phase of SCI. In SCI in general, routine screening urine cultures are not valuable as a high species turn over is seen. Intermittent catheterisation, ta

Prevention and treatment of urinary catheter-related infections in older patients.

The elderly population is more likely to have urinary catheters used than younger populations. Most patients with short term indwelling catheters (those in place less than 30 days) will be residents of acute care institutions. The frequency of catheter-acquired bacteriuria is greater with longer durations of catheterisation, female gender, failure to maintain a closed drainage system, and in patients not receiving systemic antimicrobials. Systemic antimicrobials given within 48 hours of catheter removal decrease the incidence of urinary infection, but are not currently recommended because of c

Chronic prostatitis in spinal cord injury patients.

Six spinal cord injury patients with chronic prostatitis were reviewed, all of whom had been treated with an indwelling Foley catheter during the phase of spinal shock. The 3 glass urine specimen test, the bladder wash-out test, a study of antibody coated bacteria and urethrography had limited diagnostic value. A specific diagnostic 5 glass specimen test proved to be useful and reliable. Longterm antibiotic treatment was successful in only one patient. Injection of antibiotics into the prostate gland was ineffective in the five patients in whom it was carried out. During a follow up from 1 to

[Detection and diagnosis of drug induced lithiasis].

Drug-induced calculi are often mis-diagnosed because of inadequate analysis of the urinary calculi. These stones can only be characterized unambiguously by global physical methods like infra-red spectrophotometry. From a series of 2,000 calculi analysed under infra-red, we identified 22, i.e. 1.1% of cases, which contained, partly or entirely, drug products. Ten other cases are still being studied. Amongst the products identified we found metabolites of glafenine (Glifanan) in 7 cases, triamterene and its derivatives (Cycloteriam) in 7 cases, metabolites of phenazopyridine (Pyridium) in 4 case

Guidelines for diagnosing and treating urinary tract infections.

Urinary tract infections are problems in both men and women in the geriatric age group, although causative factors in each sex differ widely. Diagnostic evaluation is directed at determining obstructive disease in men and underlying anatomic abnormalities in both men and women. Bacteriologic diagnosis is important, and localization studies are often helpful. Antibiotic therapy is guided by the usual standards, with special awareness of the greater incidence of chronic or degenerative disease in older patients.

State-of-the-Art Review: Diagnosis and Management of Acute and Chronic Bacterial Prostatitis.

Acute bacterial prostatitis (ABP) and chronic bacterial prostatitis (CBP) are poorly defined clinical entities, and diagnosis can be challenging. A clinical diagnosis of ABP can be made in the setting of an acute urinary tract infection (UTI) with systemic illness and evidence of prostatic involvement as defined by prostatic tenderness or fluctuance on digital rectal examination or prostatic abscess identified on imaging. Management includes a minimum of 2 weeks of antibiotics with surgical intervention reserved for refractory cases or prostatic abscess (depending on size). Chronic bacterial p

Phage vB_Ec_ZCEC14 to treat antibiotic-resistant Escherichia coli isolated from urinary tract infections.

Escherichia coli is a commensal bacterial species in the human gastrointestinal tract; however, it could be pathogenic and cause severe infections in intra and extra-intestinal sites. Uropathogenic E. coli accounts for 80-90% of urinary tract infections that can result in urosepsis and septic shock. Consequently, multidrug-resistant uropathogenic E. coli poses a considerable risk to the healthcare system worldwide. Phage therapy is demonstrated as an optimistic solution to over-the-counter antibiotics that contribute to the global issue of multidrug-resistant bacteria. This study aims to isola

First Case of Raoultella planticola Urinary Tract Infection Reported in Western Romania.

Raoultella planticola is a Gram-negative bacterium rarely involved in urinary tract infections. The patient was an 80-year-old woman with several associated diseases who presented to the hospital with fever and dysuria. Raoultella planticola was identified to be the causative agent of the urinary tract infection. Antibacterial treatment led to a full recovery within 7 days. This report highlights the presence of a rare pathogen as a causative agent in the case of a urinary tract infection and also the importance of using multiple methods in order to identify bacteria and to establish the diagn

Effects of utilization management on health outcomes: evidence from urinary tract infections and community-acquired pneumonia.

BACKGROUND: Utilization management policies are pervasive in the Medicare Part D program. We assess the effect of utilization management restrictions in the Medicare Part D program on the quality of care in two clinical areas - community-acquired pneumonia (CAP) and urinary tract infections (UTI). METHODS: In this study, we identified new cases of CAP and UTI from Medicare claims data from 2010 to 2016. We assessed the relationship between exposure to utilization management for antibiotic medications suitable for treating these conditions and adverse health outcomes, based on the Agency for H

Drivers of Prolonged Outpatient Antibiotic Therapy for Urinary Tract Infections and Community-Acquired Pneumonia.

BACKGROUND: Variability exists in treatment duration for community-acquired pneumonia (CAP) and urinary tract infection (UTI) in children and may be associated with non-clinical factors. METHODS: A retrospective study was conducted of patients treated for outpatient CAP and UTI in a children's hospital network from 2016 to 2019. Multivariable logistic regression was performed to identify predictors of long antibiotic duration (≥10 days). Hospitalization within 30 days was determined. RESULTS: Overall, 2124 prescriptions for CAP and 1116 prescriptions for UTI were included. Prescriptions were

Defining effective durations of antibiotic therapy for community-acquired pneumonia and urinary tract infections in hospitalized children.

PURPOSE OF REVIEW: Community-acquired pneumonia (CAP) and urinary tract infections (UTI) are two common childhood infections often leading to hospital admission. National guidelines for CAP and UTI in children recommend durations of antibiotic therapy of 10 days and 7-14 days, respectively. Due to concerns of rising antimicrobial resistance and an increased awareness of harms associated with prolonged courses of antibiotics, there is a renewed emphasis on reevaluating commonly prescribed durations of antibiotic therapy across bacterial infections. We describe recent clinical trials and observa
