Use of the Ophtec Artificial Iris Model C1 in Patients with Aniridia/Aphakia.

In the present cases, the use of the Ophtec Arificial Iris model C1 is described in patients with traumatic or postoperative aniridia/aphakia. In one of the patients, it was combined with perforating keratoplasty because of corneal scarring. In both of the presented cases, improvement in visual acuity and a satisfactory aesthetic result without any serious complications can be shown. However, the short follow-up time must be emphasised.

Congenital anterior staphyloma associated with Peters' anomaly and aphakia in a Holstein calf.

A Holstein calf was born with a large protruding right eye and a central corneal opacity. Enucleation was the first choice of treatment. The calf had a good prognosis and was raised for milking purposes. Macroscopically, the enucleated eye was characterized by the protruded cornea, adherence of the iris to the central posterior cornea and aphakia. Microscopically, central corneal thickening and a defect in the endothelium and Descemet's membrane were observed. These data suggest that this represents a case of unilateral anterior segment dysgenesis consistent with congenital corneal staphyloma …

Intraoperative aberrometry-based aphakia refraction in patients with cataract: status and options.

AIM: To explore the application of intraoperative wavefront aberrometry (IWA) for aphakia-based biometry using three existing formulae derived from autorefractive retinoscopy and introducing new improved formulae. METHODS: In 74 patients undergoing cataract surgery, three repeated measurements of aphakic spherical equivalent (SE) were taken. All measurements were objectively graded for their quality and evaluated with the 'limits of agreement' approach. ORs were calculated and analysis of variance was applied. The intraocular lens (IOL) power that would have given the target refraction was ba…

Expression of truncated PITX3 in the developing lens leads to microphthalmia and aphakia in mice.

Microphthalmia is a severe ocular disorder, and this condition is typically caused by mutations in transcription factors that are involved in eye development. Mice carrying mutations in these transcription factors would be useful tools for defining the mechanisms underlying developmental eye disorders. We discovered a new spontaneous recessive microphthalmos mouse mutant in the Japanese wild-derived inbred strain KOR1/Stm. The homozygous mutant mice were histologically characterized as microphthalmic by the absence of crystallin in the lens, a condition referred to as aphakia. By positional cl…

Optical penalization with contact lenses for children with unilateral aphakia: an alternative to patching.

OBJECTIVES: To evaluate the efficacy of high-plus contact lenses as a form of optical penalization in children with unilateral aphakia. METHODS: An internal database review was used to identify 10 patients with unilateral aphakia after congenital cataract extraction who underwent treatment with a high-plus contact lens in their sound eye after a period of failure with traditional occlusive patching. The details surrounding the switch from patching to optical penalization with contact lens were recorded along with related changes in compliance and any demonstrable effect suggestive of an impro…

The association between myopic shift and visual acuity outcome in pediatric aphakia.

PURPOSE: The advent of intraocular lens implantation after pediatric cataract surgery necessitates an increased understanding of refractive development. The significant variation in rate and amount of refractive change among eyes, both aphakic and pseudophakic, is well recognized, although the causes of such variation remain unclear. The purpose of this study was to determine if a correlation exists between the rate of refractive growth (RRG) and visual acuity outcome in pediatric aphakia. METHODS: Multicenter, retrospective observational case series. One hundred and twenty-five eyes of 85 pa…

[Etiology of contact lens failure in pediatric aphakia. Indications for intraocular lenses?].

UNLABELLED: Due to the low rate of complications, lentectomy and contact lens fitting is the standard treatment for congenital cataract. However, contact lens fitting is not possible in all children. The authors report the main reasons for discontinuation of contact lenses in their patients. METHOD: In 134 consecutive lentectomies of 90 children, the underlying eye diseases and general diseases, the age at operation, compliance of parents and children, and social background were analyzed. RESULTS: Twenty of the 90 children had to discontinue contact lens wearing. Twelve of these children wer

[Corneal topography of patients with unsatisfactory visual acuity after epikeratophakia for aphakia].

Corneal topographic characteristics of 5 patients who obtained unsatisfactory vision after aphakic epikeratophakia, with at least nine months follow-up, were reported. All grafts were apparently decentered relative to visual axis. The contour of cornea showed highly irregular pattern of power distribution. These findings suggest that to mark visual axis before surgery, lathe corneal lenses carefully and do standard operation should be the basis of successful epikeratophakia.

Comparative study of clinical factors that predispose patients to proliferative vitreoretinopathy in aphakia.

To clarify the risk factors of proliferative vitreoretinopathy (PVR) in aphakia, the clinical features of 25 aphakic eyes with PVR were statistically analyzed and compared with a control group of 157 aphakic eyes with non-PVR rhegmatogenous retinal detachment. The statistically significant (P less than 0.05) factors that predisposed patients to PVR in aphakia were as follows: a history of vitreous loss on cataract surgery, retinal detachment developing within 3 months after cataract extraction, duration of retinal detachment longer than 3 months, break larger than three disc diameters, and cho…

The use of contact lenses to correct aphakia in a clinical trial of cataract management.

In a prospective study, 110 eligible patients (111 eyes) were randomly selected to have contact lenses for aphakia (CL). The lens of first choice was a daily-wear hard (DWH) PMMA contact lens. Extended-wear (XW) lenses were fitted to those who failed with DWH lenses. The numbers in the two groups were approximately equal. Eighty per cent of eyes were successfully wearing CLs six months post-operatively and 72% were still wearing them after five years. The mean age of both groups of patients was greater than 70 years but the DWH group was significantly younger (p less than 0.001). However, XW l…
