Excessive eye elongation can be induced in experimental animals by visual deprivation. A tendency toward myopic refraction or shift in pediatric aphakia has been recorded in some children. In this report, we present the case of an 18-year-old patient who had developmental cataracts treated at seven years of age with bilateral cataract extraction and implantation of an intraocular lens in one eye only. Axial length of the eye with a contact lens increased compared with the eye with the intraocular lens.

The author submits the results of correction of unilateral aphakia which is a special type of anisometropia. For more than 500 examined patients with unilateral aphakia all those who had a refraction of the aphakic eye between 9.0 and 15.0 dpt were subjected to statistical analysis. The groups were classified with regard to the refraction of the aphakic eye. From the analysis ensues that optimal results of binocular vision are in the region round 12.0 and 13.0 dpt which is consistent with clinical results.

Presented are personal experiences concerning the application of artificial intraocular lenses in patients with diabetes and cataract. Operated were patients aged 50-76 years in whom implantation of an artificial lens of Alcon or Cilco was performed either to the anterior or posterior chamber. Lenses were removed from rabbit eyes using an extracapsular technique. Animals were in three groups; no treatment (NT), post-treatment (PT) and pre- and post-treatment (PPT). Treatment consisted of 2% topical indomethacin given three times a day after surgery, except for the PPT group that received 3 drops on the day prior to surgery and drops at 3 hours and 1 hour before surgery. Elongation of the eye is one of the drawbacks of implanting intraocular lenses in children. To evaluate possible effects of cataract and aphakia on eye elongation, we measured the axial length in children with unilateral aphakia (15 children with congenital cataract and 27 with traumatic cataract) and in children with bilateral congenital cataract (operated on in 14 cases and not operated on in eight cases). The intra-ocular pressure in 76 patients, undergoing Neodymium: YAG laser surgery was measured just before treatment, and one and two hours afterwards. The fellow eye served as a control. Serious elevation of the intra-ocular pressure was demonstrated, especially in capsulotomy in the aphakic eye, to a lesser extent in pseudophakic eyes. Intra-ocular pressure elevation of more than 20 mm was seen in iridectomies. Timolol 1/2% eyedrops given before Neodymium: YAG laser treatment minimized the elevation of the intra-ocular pressure. We present an initial report on flexible anterior chamber lens implantation in 14 aphakic or pseudophakic bullous keratopathy patients undergoing penetrating keratoplasty. With a minimum follow-up of 12 months, the improvement in visual acuity and graft clarity is encouraging.

In a prospective clinical study 54 postcataract membrane dissections (51 patients) were performed using a Q-switch Nd-YAG laser. Under standardized conditions all patients were pretreated with prostaglandin inhibitors and received topical steroids and beta-blockers in low concentrations, as well as carbonic anhydrase inhibitors orally. The changes in visual acuity and the response of IOP to total energy input were analyzed. There was no statistically significant correlation between postoperative rise in IOP and total energy input. The clinical relevance of the results is discussed.

The use of intraocular lenses (IOLs) to replace cataractous lenses has raised question of possible retinal damage due to exposure to ultraviolet (UV) radiation. Three groups of cynomolgus monkeys underwent cataract operations and either received normal UV-transparent IOLS or UV-opaque IOLs, or were left aphakic. All eyes were later exposed to UV light for varying periods of time. Exposure to UV light produced severe retinal lesions.
