[The mechanism of apnea during difficult breathing].

The experiments on anesthetized cats demonstrated that only a combination of hypoxia and inspiratory resistive loading induced respiratory arrest. The findings suggest that both central and peripheral mechanisms of apnea exist due to functional insufficiency of respiratory muscles as a result of synaptic disorder and excitatory contractility disorder in diaphragmatic muscle fibers.

[Physiopathology of mouth breathing. Snoring and apnea].

Oral breathing causes changes in pulmonary mechanics as well as in the pressure of arterial blood gases. In response to increased nasal obstruction oro-nasal breathing occurs. The level of oro-nasal partitioning maintains an adequate level of respiratory resistance. Sleep disordered breathing, although not only related to oral breathing, is a common disorder. When upper airway resistance is increased limitation of flow occurs. Snoring indicates a mild degree of reduced airflow. Sleep apnea occurs when upstream pressure falls below a critical pressure (Pcrit). The mechanisms are reviewed.

Sleepiness and hypertension in obstructive sleep apnea.

In order to assess the complications of sleep apnea, we have reviewed a data base of 619 consecutive admissions to a university sleep disorders center. Although patients with obstructive sleep apnea (OSA) described more subjective sleepiness than patients with central sleep apnea (CSA) or primary snoring (PS), the multiple sleep latency test (MSLT) indicated similar levels of physiologic sleepiness in the two apneic groups, which was greater than among those with PS. There was no significant relationship between individual subjective estimates of habitual sleepiness and the MSLT values. Among …

Mechanisms and management of central sleep apnea.

Central sleep apnea is not a single disease but represents the final pathway in a large group of heterogeneous disorders. Control of normal breathing during sleep relies upon finely coordinated anatomical and physiological mechanisms and their destabilization leads to central apnea. The causes of central sleep apnea can be classified into 4 groups: neurologic disorders, periodic breathing, upper airway abnormalities, and idiopathic syndromes. Clinical features result from the interaction between the underlying disorder and control of respiration. Two different prototypes emerge: patients who a…

The natural history of the appearance of apnea of prematurity.

Twenty healthy preterm infants of less than 34 wk gestation were studied with continuous recordings, commencing within 8 h of birth, for up to 1 wk of age to determine the usual time course of the appearance of apnea and to classify apnea types. Airway occlusion studies were also performed on a regular basis to determine whether apneic spells were preceded or followed by a reduction in central respiratory drive. Apneic spells of greater than 15 s duration accompanied by hypoxia or bradycardia occurred in all infants before 24 h of age. The frequency of apneic spells was highest in the first 24…

Uvulopalatopharyngoplasty for treatment of obstructive sleep apnea.

Currently, uvulopalatopharyngoplasty (UPPP) is the most common surgical procedure used for the treatment of obstructive sleep apnea. Patients with clinically significant obstructive sleep apnea in whom medical treatment has failed or who are unwilling to comply with medical therapy are considered candidates for UPPP. The initial surgical results obtained in nonselected patients with obstructive sleep apnea were highly variable, approximately half of the patients experiencing more than a 50% reduction in the frequency of disordered breathing events postoperatively. Although differences in surgi…

Periodic breathing and apnea in preterm infants.

The relationship between periodic breathing and idiopathic apnea of prematurity was investigated. We recorded respiratory impedance, heart rate, pulse oximetry and end-tidal CO2 from 68 untreated infants of less than or equal to 34 wk gestation with a diagnosis of idiopathic apnea of prematurity. Mean birth wt was 1476 g (SD 420) and mean gestational age was 29.9 wk (SD 2.6). Apneas of more than 15 s duration that were associated with hypoxemia or bradycardia were identified by semiautomated analysis of computerized records. A total of 1116 significant apneic spells were identified, only one o…

Malignant hyperthermia in a child with sleep apnea.

Malignant hyperthermia is a syndrome characterized by a hypermetabolic state of skeletal muscle (1, 2). It is transmitted by genetic inheritance. The syndrome has frequently been associated with certain neuromuscular and other disorders (1, 3). A case of the syndrome occurring in a child with sleep apnea syndrome, an association not previously reported, is discussed here.

Magnesium therapy in infants with postneonatal apnea.

Idiopathic postneonatal apnea is defined here as a sudden, unexpected attack of apnea that first occurs after the infant has been discharged from the neonatal nursery to his home. Little has been published concerning the patient's physical and clinical laboratory findings during the acute episode. The present report of 20 infants with postneonatal apnea retrospectively addressed these parameters, including all investigations conducted for magnesium deficiency. The first episode of apnea occurred at 48 +/- 12.5 days of age and was unpredicted, transient, and self-limited. In the sickest infants…

Treatment of obstructive sleep apnea by uvulopalatopharyngoplasty.

Although uvulopalatopharyngoplasty has been proposed as a treatment for obstructive sleep apnea, there is limited objective data concerning its efficacy. We have examined 40 patients with moderate to severe obstructive sleep apnea before and 3 months following uvulopalatopharyngoplasty. A standardized questionnaire and detailed overnight sleep study were performed before and after surgery. In addition, 15 patients were retested 1 year postoperatively. Uvulopalatopharyngoplasty resulted in a statistically significant reduction in symptoms, blood pressure, apnea index, and total apneic time. The…

Sleep apnea and the Arnold-Chiari malformation.

We present a polysomnogram-documented report of central sleep apnea (427 events) and moderately severe decreases in arterial oxygen saturation (to 81%) associated with the Arnold-Chiari malformation (ACM). Daytime hypersomnolence and other symptoms had significantly impaired our patient's work performance. After surgical correction of the ACM, there was marked improvement in symptomatology. A post-surgery polysomnogram revealed marked improvement in the number of central apneas (74 events) and only mild decreases in oxygen saturation (to 94%).

An evaluation of theophylline for idiopathic apnea of infancy.

19 infants admitted with a diagnosis of infantile apnea who were found to have periodic breathing were given oral theophylline to determine its effect. They were studied at a mean age of 7.1 weeks (1-16.4 week). Each infant was studied during two naps, immediately before and 7 days following the institution of theophylline therapy, which averaged 2.8 h in duration during which electro-oculograms, end-tidal CO2, heart rate, impedance respirations, and transcutaneous pO2 (tcpO2) were continuously monitored. Theophylline therapy (mean dose 2.3 mg/kg q. 6 h) was associated with a significant reduc…

Infant apnea: findings on the barium esophagram.

Barium esophagrams of 160 infants who were being examined for apneic episodes were obtained at a referral center for the investigation of sudden infant death syndrome (SIDS). The studies were standardized as closely as possible to evaluate swallowing, esophageal function and anatomy, and the gastroduodenal regions. The level and frequency of gastroesophageal reflux were carefully assessed. The most common abnormality identified was gastroesophageal reflux (54%). Other abnormalities included nasopharyngeal reflux (27%), aberrant right subclavian artery (3%), and aspiration into the airway (3%).

Obstructive sleep apnea in infants and children.

Twenty-two infants and children were found to have clinically significant obstructive sleep apnea. A history suggesting complete or partial airway obstruction during sleep was obtained on all patients, and physical examination of the sleeping patient revealed snoring, retractions, or OSA in 21 patients. Nevertheless, the mean delay in referral for 20 patients first seen after the neonatal period was 23 +15 (+ SD) months. Sixteen of 22 patients (73%) developed serious sequelae: cor pulmonale in 12 (55%), failure to thrive in six (27%), permanent neurologic damage in two (9%), and behavioral dis…

Periodic alpha seizures with apnea in a newborn.

A post-mature female newborn with a history of birth trauma developed frequent apneic spells coincident with left posterior-quadrant alpha discharges on her electro-encephalogram. Except during apneic spells, clinical seizure activity was rare. The alpha discharges occurred at regular intervals and were associated with disrupted sleep cycles and a diminution in the proportion of quiet sleep. Phenobarbital apparently abolished the apnea and alpha discharges and resulted in normal sleep cycles and state proportions. At 20 months of age the patient is neurologically and developmentally normal. Sh…

Sleep apnea in a child with the pickwickian syndrome.

A 3-year-old girl with a history of excessive weight gain from birth presented with obesity, somnolence, and cyanosis, characteristic of the Pickwickian syndrome. Obesity was familial and exogenous without endocrine or neurologic anomaly. Respiratory center sensitivity to carbon dioxide was normal. Excessive somnolence was due to the obesity, which during sleep caused airway obstruction, apnea, and awakening, finally resulting in sleep deprivation. The sleep apneas and the daytime somnolence disappeared with weight reduction, showing that obesity alone had been responsible for the disorder.
