Comparison of surgical stress between laparoscopic and open appendectomy in children.

PURPOSE: The present study aimed to evaluate laparoscopic appendectomy (LA) in comparison with conventional open appendectomy (OA) in children, with special emphasis on the extent of surgical trauma after LA and OA, and to assess whether LA had any clear advantages compared with conventional OA. METHODS: A total of 160 patients with a median age of 7.9 years (range 3-15 years) were studied. Sixty-nine of them underwent LA, and the remaining 91 underwent OA. Serum interleukin (IL) 6 and C-reactive protein (CRP) levels which are thought to play a pivotal role in the pathogenesis of surgical tra…

Ninety-four appendectomies for suspected acute appendicitis during pregnancy.

Acute appendicitis during pregnancy is a rare event, and large numbers of cases reported in the literature stem entirely from data stored in national registers. Between 1974 and 2000 relevant perioperative data on the treatment of appendicitis were collected consecutively and analyzed retrospectively. Surgical and obstetric data relating to the medical history, the clinical, intraoperative, and histologic findings, and the course of the pregnancy were recorded. Altogether, 9793 appendectomies were performed, 94 of which were in pregnant women (24.5% during the first trimester, 51% during the s

Intraabdominal abscess following open and laparoscopic appendectomy in the pediatric population.

BACKGROUND: Studies comparing intraabdominal abscess (IAA) rates following laparoscopic appendectomy (LA) and open appendectomy (OA) have shown conflicting results. METHODS: The charts of 324 children undergoing appendectomy were reviewed retrospectively to examine the incidence of IAA. RESULTS: Of the 324 appendectomies, 204 (63.0%) were completed laparoscopically and 119 (36.7%) were performed open. The conversion rate was 0.2% (1/324). Of the 15 (4.6%) IAAs, 7 occurred in the LA group (3.4%) and 8 occurred in the OA group (6.7%) [p = not significant (NS)]. The incidence of IAA for perfora…

[Laparoscopic or open appendectomy. Critical review of the literature and personal experience].

UNLABELLED: In the era of video-laparoscopic surgery there are a lot of surgeons that still continue to perform open appendectomy. This choice is the consequence of the good results of open appendectomy (clinical, cosmetic, hospital stays and hospital costs). Published trials on laparoscopic appendectomy don't show that it is superior to the open approach. The aim of this study is to critically review the literature on laparoscopic and traditional appendectomies and to report a clinical experience on 86 consecutive patients that underwent open appendectomy. PATIENTS AND METHODS: From Septembe…

Laparoscopic appendectomy does not change the incidence of postoperative infectious complications.

BACKGROUND: It is not clear whether the laparoscopic approach does decrease the incidence of postoperative infectious complications after appendectomy. METHODS: One hundred sixty-nine patients were randomized, 87 with laparoscopic (LA) and 82 with open appendectomy (OA). Patients in the OA group had a McBurney incision; LA was performed in the lithotomy position. RESULTS: Acute appendicitis was confirmed in 75% of patients. The appendix was perforated in 5 patients of the LA versus 2 patients of the OA group. No conversion to the open procedure was necessary. The median operating time was 35…

[Barrett esophagus diagnosed after appendectomy in a patient with diabetes mellitus].

A pathogenetic outline of oesophageal inflammatory lesions with special emphasis on Barrett's oesophagitis has been presented. As recorded in a history of a 65-year old patient at first the stenocardia-mimicking pains occurred a few days after appendectomy. Gastrobulboscopy performed (after coronary insufficiency had been excluded) revealed extensive inflammatory and erosive lesions of the oesophagus and multiple ulcerations of the duodenum. According to the several authors the rapid manifestation of the inflammatory lesions of the oesophagus may be related to the intubation necessary for lapa…

The role of prophylactic antibiotics in appendectomy using delayed primary closure.

A prospective randomized trial of prophylactic antibiotics in 202 patients undergoing appendectomy is reported upon. The antibiotics used were metronidazole and cephradine. Half of the patients received antibiotics preoperatively. The other half received antibiotics at operation only upon finding perforation, gangrene or pus formation. All wounds were treated by delayed primary closure. No instance of wound infection, defined as abscess formation or cellulitis, occurred in either group. Antibiotics given prophylactically in every instance appear to have no advantage in preventing wound infecti…

Comparison of stump closure techniques during laparoscopic appendectomies for complicated appendicitis - results from Pol-LA (Polish laparoscopic appendectomy) multicenter large cohort study.

Background: In general, the three main options for stump closure in laparoscopic appendectomy are clips, endoscopic staplers and endoloops. However, there is no gold standard, especially regarding complicated acute appendicitis which is generally associated with worse outcomes. Objectives: We aimed to assess the outcomes of different stump closure techniques during laparoscopic appendectomies for complicated appendicitisMethods: Our multicenter observational study of 18 surgical units assessed the outcomes of 1269 laparoscopic appendectomies for complicated appendicitis that used the three main…

Medullary adenocarcinoma of the ascending colon mimicking appendicitis after prior appendectomy.

Medullary adenocarcinoma is a rare colorectal carcinoma that may be difficult to recognize when inflammation dominates the presentation. A 65-year-old woman with a remote appendectomy presented with 1 month of postprandial abdominal pain, weight loss, and constipation. Repeated computed tomography demonstrated marked pericecal inflammation and a blind-ending structure interpreted as possible appendicitis despite her surgical history. Diagnostic laparoscopy identified an enlarged mesenteric lymph node, and excision unexpectedly showed metastatic medullary carcinoma of colonic origin with loss o…

Female Surgeons Versus Male Surgeons in Laparoscopic Appendectomy: Equal Surgical Outcomes?

BACKGROUND: Despite advances in addressing sex disparities, inequities persist in the surgical field. Although some medical specialties have achieved a more balanced sex representation, female surgeons continue to face challenges. This study aimed to determine if a surgeon's sex influences surgical performance by comparing surgical outcomes between male and female surgeons performing laparoscopic appendectomy. METHODS: We conducted a retrospective analysis of adult patients (>16 y) undergoing laparoscopic appendectomy for acute appendicitis at our institution from 2014 to 2022. Patients were …

Incidental Appendiceal Neuroendocrine Tumor Post Appendectomy: Surgery Is Here to Stay.

Neuroendocrine tumors (NETs) arising from the appendix are rare neoplasms but carry significant consequences if missed. These malignancies are typically diagnosed after an appendectomy via histopathological evaluation of the appendix. This aspect further solidifies surgery's place in the treatment of appendicitis. A 25-year-old female patient presented to the emergency department with a three-day history of right-sided abdominal pain associated with nausea and two episodes of non-bilious vomiting. Physical examination was initially benign but later showed tenderness to the right of the umbilic…

Appendiceal diverticulum diagnosed after appendectomy: Two case reports and literature review.

INTRODUCTION: Appendiceal diverticulitis is an uncommon pathology that imitates acute appendicitis and is usually treated by appendicectomy. CASES PRESENTATION: We present two cases: a 50-year-old female patient and a 35-year-old male patient, both of whom presented with signs and symptoms of acute appendicitis and were managed accordingly. Final pathological examination confirmed the presence of an appendiceal diverticulum. CLINICAL DISCUSSION: Diverticulitis of the appendix is four times more likely to lead to perforation when compared to appendicitis and may be associated with underlying

Neuromyelitis optica spectrum disorder after post appendectomy peritonitis: A case report.

Neuromyelitis optica spectrum disorder (NMOSD), also known as "Devic's syndrome", is an autoimmune demyelination disorder. It affects the optic nerve and spinal cord, causing optic neuritis and transverse myelitis. It is associated with anti-aquaporin 4 antibodies that target the aquaporin channel on astrocytes. An 18-year-old male with a history of appendectomy 6 months ago presented with neck pain, numbness in limbs up to the umbilicus, paresis, and spasticity in the left leg. The brain magnetic resonance imaging (MRI) was normal, while the spinal cord MRI showed hyperintense foci at the T2 …

Income-Based Disparities in Outcomes Following Pediatric Appendectomy: A National Analysis.

BACKGROUND: Appendectomy remains a common pediatric surgical procedure with an estimated 80,000 operations performed each year. While prior work has reported the existence of racial disparities in postoperative outcomes, we sought to characterize potential income-based inequalities using a national cohort. METHODS: All non-elective pediatric (<18 years) hospitalizations for appendectomy were tabulated in the 2016-2020 National Inpatient Sample. Only those in the highest (HI) and lowest income (LI) quartiles were considered for analysis. Multivariable regression models were developed to assess…

Using Wearable Devices to Profile Demographic-Specific Recovery After Pediatric Appendectomy.

INTRODUCTION: Counseling patients and parents about the postoperative recovery expectations for physical activity after pediatric appendectomy varies significantly and is not specific to patients' demographic characteristics. Consumer wearable devices (CWD) can be used to objectively assess patients' normative postoperative recovery of physical activity. This study aimed to develop demographic-specific normative physical activity recovery trajectories using CWD in pediatric patients undergoing appendectomy. METHODS: Children ages 3-18 y old undergoing laparoscopic appendectomy for acute appen…

Novel Quality Improvement Targets to Address Healthcare Encounters After Pediatric Appendectomy.

INTRODUCTION: Pediatric appendicitis clinical practice guidelines (CPGs) do not typically address postdischarge healthcare encounters. This study aims to examine common indications for returns to the health system to identify novel quality improvement targets. METHODS: This retrospective cohort study analyzed patients aged 3 to 18 y undergoing appendectomy at a single institution from July 1, 2019, to July 31, 2020. The primary outcome was physical postdischarge encounters comprising emergency department (ED) visits and hospital readmissions. Indications for each encounter were categorized an…

Non-hormonal Clitoromegaly due to Clitoral Priapism Caused by Appendicitis/Appendectomy.

Clitoromegaly usually develops due to hyperandrogenism. There are a few cases of clitoromegaly described without clinical and biochemical hyperandrogenism. Clitoromegaly due to clitoral priapism and clitoral priapism after appendectomy have not been reported previously. A 7-year-old girl was referred for enlargement of the clitoris. She reported having a mild, pulsating clitoral pain starting three days after an appendectomy operation. Subsequently, painful swelling and an increase in the size of the clitoris was observed. Her growth and physical examination were otherwise normal. Causes of th…
