Mycobacterial Musculoskeletal Infections.

Although less common as causes of musculoskeletal infection than pyogenic bacteria, both Mycobacterium tuberculosis and nontuberculous mycobacteria can infect bones and joints. Although tuberculous arthritis and osteomyelitis have been recognized for millennia, infections caused by nontuberculous mycobacteria are being identified more often, likely because of a more susceptible host population and improvements in diagnostic capabilities. Despite advances in modern medicine, mycobacterial infections of the musculoskeletal system remain particularly challenging to diagnose and manage. This artic

Bilateral sternoclavicular joint septic arthritis presenting as cutaneous abscesses.

The sternoclavicular joint can be involved in ankylosing spondylitis as well as in rheumatoid and degenerative arthritis. Septic arthritis of this joint is infrequently seen, and the diagnosis of this infection can be missed until it presents with a complication. We describe a patient with bacteremia whose presentation of bilateral sternoclavicular joint septic arthritis was multiple cutaneous abscesses on her chest wall.

Necrotizing fasciitis secondary to diverticulitis.

Necrotizing fasciitis is a rare, rapidly progressive infectious process primarily involving the fascia and the subcutaneous tissue, with thrombosis of the cutaneous microcirculation. We present a case of necrotizing fasciitis secondary to diverticulitis in an immunosuppressed patient with rheumatoid arthritis.

Unique features of group B streptococcal arthritis in adults.

Group B streptococci have rarely been reported to cause serious infection in adults and even less frequently to result in septic arthritis. We reviewed our clinical and radiologic experience with septic arthritis and uncovered five cases of infectious arthritis caused by group B streptococci. Unlike previously described patients with monoarthritis and complete recovery, our patients displayed polyarticular involvement. The infection in some cases was aggressive, resulting in destruction of multiple joints. Therefore, the series supports the view that this uncommon pathogen may produce an aggre

The acutely swollen joint. First impressions may mislead.

An acutely swollen joint may be indicative of a number of disease entities. A thorough history and physical examination are the cornerstones of evaluation. Laboratory findings can be useful in diagnosis, as can response to therapy (eg, response to penicillin in gonococcal arthritis is often the only criterion for diagnosis, as the organism is difficult to culture). Patients with malignancy (especially leukemia) or who are immunosuppressed or otherwise debilitated are at particular risk for a septic cause of swelling. Infectious arthritis should be the first potential cause looked for in these

Hidden menace: Understanding the devastating consequences of dourine disease in horses.

Trypanosoma equiperdum is a protozoan parasite that causes the sexually transmitted disease known as "dourine" in horses. This chronic illness is directly spread from one animal to another during mating. Doflein proposed the name T. equiperdum in 1901. Despite being distributed worldwide, the broad use of artificial insemination technology over the past three decades has resulted in only a few cases being documented. The condition is typically fatal and is characterized by gradual emaciation, nervous system involvement, and edematous lesions of the genitals. The incubation period between expos

Ultrasonography of musculoskeletal infections in children.

Ultrasonography is an important modality for evaluation of musculoskeletal infections in children because it is rapid, nonionising and very sensitive for (infectious) fluid collections and joint effusions. Moreover, the images are not degraded by metallic or motion artefacts (as with CT and MRI) and finally, ultrasonography offers the possibility of fine-needle aspiration to confirm the infectious nature of a fluid collection without unnecessary contamination of adjacent anatomical compartments. Ultrasonography should be combined with radiography because both imaging techniques are complimenta

Prognostic outcomes of neonatal septic arthritis: a systematic review and meta-analysis.

BACKGROUND: The unique vulnerability of the neonatal joint to septic arthritis poses a substantial risk for lifelong musculoskeletal disability. Precise estimates of long-term functional outcomes are crucial for prognostication and clinical decision-making, yet robust pooled data remain scarce. This systematic review and meta-analysis was therefore conducted to determine the aggregate rate of favorable prognosis in neonates following septic arthritis. METHODS: We queried four major databases (PubMed, Embase, Cochrane Library, and Web of Science) from inception until September 2025 to identify

Synovial fluid analysis in healthy green turtles Chelonia mydas in Taiwan.

Septic arthritis is a frustrating disease in sea turtle rehabilitation because of its unclear pathogenesis, delayed onset during rehabilitation, long-term treatment requirements, and potentially poor prognosis. Radiography, blood cultures, and arthrocentesis have been used as diagnostic tools for suspected cases. However, there is currently a lack of data on the characteristics of synovial fluid in healthy sea turtles. To establish reference data for synovial fluid in sea turtles, we enrolled 14 green turtles Chelonia mydas rescued between 2019 and 2022 from 3 facilities using the following in

Approach to a patient with monoarticular disease.

PURPOSE: To reassess the diagnostic approach to a patient with a monoarticular disease in light of the up-to-date medical literature and to examine the practical utility of traditional and newer imaging tools in the setting of monoarthritis. RESULTS: The monoarticular disease can represent a medical emergency on the one hand and be a diagnostic conundrum on the other. The management rules of patients with monoarthritis have been established long ago, but various pitfalls still lead physicians off the right diagnosis at times. Septic, pseudoseptic arthritis and hemarthrosis are the most common

[Native joint infections].

Joint infections are a great challenge for the treating physicians. Infection of a native joint can result in the rapid destruction of the joint if treatment is inadequate or delayed and is associated with severe life-threatening diseases, such as sepsis. This article presents a detailed overview of the treatment of joint infections of native joints with a focus on the diagnostic approach and treatment decisions. Infections in native joints are caused either endogenously by hematogenic spreading or are caused by exogenous factors. A relevant proportion of joint infections in native joints occu

Synovial biopsies in clinical practice and research: current developments and perspectives.

Synovial biopsy techniques have developed and widely expanded over the past few years, in particular due to the development of ultrasound-guided procedures. This article reviews the different techniques, clinical applications, and the latest advances in translational research as well as current challenges and perspectives. The first part focuses on different techniques available for biopsy, along with their feasibility, success rate, tolerance, and training requirements. In the second part, clinical applications are described. Data on diagnostic performances are reported, especially regarding
